Washburn Center's case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family's needs and coordinate care across multiple providers. These services are provided for children age 3 through 17. Case Management is a unique field which will provide you with an expansive understanding of family mental health. As such this role is well suited for Social Work experienced individuals or Mental Health qualified Professionals. Additionally, this role is a highly collaborative function, and you will experience exposure to all programs at Washburn Center and work in collaboration with the various experienced professionals at Washburn to ensure the best care for the children and families in your care. The case management professional will be actively present in the community and the home life of the client. We encourage independent working for all our case managers to allow for the best client care and services. ROLE RESPONSIBILITIES:1. You will be managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.2. At Washburn Center for Children we are committed to provide culturally based, individualized targeted Children's Mental Health Case Management to children/students and families in their home, schools and community based on each family's needs. 3. You will coordinate mental health and other health related services. The purpose of this involvement in the child and family's life is to improve or maintain child/student's emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services. 4. You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations. DAY TO DAY ACTIVITIES:Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:Assessment - for your clients you will complete a functional assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.Service Plan Development - You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis. Dependent on each individual case you will Refer, Coordinate and Monitor Services **All clinicians are to ensure Minnesota and HIPAA data privacy requirements are followed.** Education and experience required:Due to the unique nature of the work, we are considering individuals with:
· A bachelor's degree in one of the behavioral sciences or a related field including, but not limited to: social work, psychology, human services, or sociology from an accredited college or university· An LSW (License of Social Work) is required, should you have a BSW· Fluency in both English and Spanish· 1 year of experience with mental health compromised children and families· Valid Driver's License and access to reliable transportation is required· A successful candidate should be able to demonstrate knowledge of cultural diversity and the role of culture in the therapeutic process· A flexible, resourceful approach to the work with experience or in-depth knowledge of home and community-based services Salary Bracket: $46,000 - $50,000 (Salary is based on several factors including but not limited to licensure, work experience and education)

What is School Based Therapy at Washburn? Our compassionate, experienced therapists support children and their families through challenging times by offering individual therapy to help with a wide range of concerns, including school adjustment problems, depression, anxiety, difficulty dealing with loss or family changes, abuse, traumatic events, attention problems, parent-child conflicts and acting-out behaviors. Our counseling focuses on building on a child's strengths and supporting them in gaining skills to be successful at home, in school and in the community. As a school-based mental health clinician you focus will be on assessment, treatment, and prevention services to children and their families as well as consultation and training for the school staff at Bloomington Elementary School.

Clinical Treatment - Clinicians will provide a combination of the following school-based services within the school, clients' homes, and across the community, as agreed upon with the school staff and the Washburn Center supervisor, including:* Conduct diagnostic assessments* Develop and implement a treatment plan with the family based on a diagnosis* Provide individual and family therapy* Participate in IEP meetings* Care coordination, including collaboration with psychiatrists and other professionals working with the child* Create a discharge plan
***All school-based clinicians will be expected to maintain monthly services consistent with budgeted quota expectations, an average of 15 billable sessions per week.
Ancillary Services - Clinicians will also provide ancillary services as agreed upon with the school staff and the Washburn Center supervisor, including:* Conduct child group therapy, skills training, and/or prevention groups* Provide care coordination, including collaboration with parents, school staff, and other agencies* Participate in classroom observations* Provide teacher consultation and training* Attend collaboration and planning meetings with school staff, program directors, and grant coordinators to review the status of the program and manage referrals in order to facilitate program development and the coordination of the program at the school
Documentation - Clinicians will be expected to maintain required documentation as specified in our Clinical Policy Manual, including:* Diagnostics assessments* Treatment plan for each client* Daily progress notes* Completion of screening instruments (SDQ) and any other required outcome instruments* Discharge summaries* Timely record keeping of all clinical activities and grant related services as defined* Timely response of communications from the program director, the supervisor, or program support to ensure clear communication throughout the program
All clinicians are to ensure Minnesota and HIPAA data privacy requirements are followed.
Supervision and Professional Development - Clinicians are expected to:* Attend weekly supervision and team meetings, present case information and formulation, and identify questions for team meetings and supervision* Receive regular clinical supervision as required under Rule 29* Provide clinical and administrative supervision for interns as approved by the program supervisor based on licensure and experience and assist with providing training, feedback, and learning experience for program interns* Maintain required CEUs As Washburn Center for Children serves the community it is important that our therapists are able to demonstrate cultural humility and responsiveness in their therapeutic process and in their interpersonal relationships. Specimens handled in this laboratory include surgical, autopsy, and research material. The International Board Certified Lactation Consultant (IBCLC) is recognized as an expert in lactation knowledge and experience in breastfeeding situations. The Lactation Consultant is available and collaborates with nursing units to provide a comprehensive lactation program which includes lactation services, education services, and continued program assessment. Lactation Consultants proactively talk with families before or assist soon after delivery with any perceived concerns. The IBCLC actively participates in Discharge Rounds. This Lactation consultant position functions collaboratively covering for lactation team members as needed at both hospitals and within the OB Clinic setting.
The lactation consultant is a clinical nurse expert working collaboratively to improve the quality of patient care and facilitate optimal outcomes in the establishment and maintenance of breastfeeding and lactation. As an expert the lactation consultant is an integral member of the healthcare team trained to counsel mothers and families on initiation, exclusivity, and duration of breastfeeding and to assist amidst any difficulties or high-risk situations. The lactation consultant focuses on health promotion and encourages self-care and parental decision-making prenatally and postnatally. The lactation consultant functions as a clinical nurse expert resource and educator, playing a vital role in educating patients, families, and multidisciplinary team members on evidenced based practices in breastfeeding and lactation to provide anticipatory guidance as well as to empower mothers and families to manage breastfeeding challenges as they arise. Facilitates breastfeeding mothers and families in reaching their breastfeeding and lactation goals to ultimately improving the care of mothers and infants. Qualifications
Graduate of a baccalaureate nursing program, as recognized by the Accreditation Commission for Education in Nursing (ACEN), Commission on Collegiate Nursing Education (CCNE), National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). If graduated from a nursing program that was not accredited by ACEN, CCNE, or NLN CNEA, at least one year of RN experience in an applicable care setting or one year is required.

Otolaryngology at Mayo Clinic is ranked 4th in the U.S. News & World Report. Our department is actively engaged in multidisciplinary partnerships across the institution. Clinically we have several highly functioning multidisciplinary clinics and teams which are able to accrue patients effectively and rapidly to prospective clinical trials with significant experience in a wide range of sample collection and rapid processing. We have four full time clinical research coordinators within our department, relationships with two biostatisticians to support our research efforts, and strong relationships with the Mayo Clinic Comprehensive Cancer Center for clinical trial activation. As a high-volume surgical team, we have access to both significant archival and fresh tissue from a wide range of tumor types. We are actively investigating tumor microenvironments, methylation markers, circulating tumor DNA, machine learning algorithms to analyze digitized histology, tumor microenvironments, and imaging modalities.

We are hiring full-time Care Coordinators to provide coordination of services and resource support to our clients in the community to get people what they need when they need it. As a Care Coordinator, you will: Coordinate all incoming client referrals in the Community Mental Health Outpatient programs at East Metro and/or Wilder Center, complete intake process, opening and closing of clients, and scheduling. Work with clients and families to ensure high satisfaction with program services. Work in a coordinated way with community partners, providers, clients and their families to support the full range of health needs including mental health and physical health. Provide backup support for Operations Support Specialist and other admin staff.
Wilder is a first-implementer Certified Community Behavioral Health Clinic (CCBHC). As part of this innovative model of mental health care, you will collaborate closely with other service partners in ensuring coordinated, integrated care for clients and their families. Job Qualifications

The registered nurse (RN) in the Internal Travel Team at Mayo Clinic will rotate across Mayo Clinic sites in Arizona, Florida, Minnesota and Wisconsin as determined by patient care needs.
RNs hired in this role will be required to complete orientation coursework including Foundations of Critical Care Nursing (FCCN) and ACLS ( if not already certified).
PCCN certification may be substituted for FCCN coursework.
RNs hired in this role will be assigned to ICU, PACU, or Procedural areas. It is an expectation that RNs will float to PCU and Med/Surg areas as needed.
The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. The registered nurse (RN) in the Mayo Clinic Department of Nursing Central Staffing Pool will provide contingency staffing support across Mayo Clinic nursing sites as determined by staffing and patient care needs. Qualifications

Minimum of one year RN experience in a PCU (ICU step-down) or ICU setting is required.

Develops and implements treatment plans for patients, including disease management education, counseling and support to patients and family members involved, exercise prescription, proper exercise training and technique, safe use of exercise equipment, identification of cardiovascular symptoms, cardiac medication education, and modification of cardiovascular risk factors. Monitors and evaluates each patient's treatment plan and must work collaboratively and effectively with the medical team regarding each patient's care. The patient population is diverse including adult, geriatric, and pediatric.

Bachelor's degree in exercise science, exercise physiology, recreational therapy, cardiac rehabilitation, health fitness or other equivalent allied health field is required.
Previous training and/or experience in cardiac rehabilitation, cardiopulmonary stress testing, cardiovascular and pulmonary physiology, cardiovascular pathology, cardiac medications, exercise prescription is required (i.e. experience from a formal internship is highly desired).
Basic knowledge of rest and exercise 12-lead electrocardiogram and rhythm abnormalities is required.
Must be physically able to demonstrate/lead various exercises and assist patients with various exercises, as well as with the use of various exercise equipment.
Master's degree is preferred. Health and Wellness coaching experience and certification is preferred. Demonstrates critical thinking, decision making, problem solving and communication skills. Involvement with local and/or national professional organizations (AACVPR) related to position is encouraged.
Licensure or Certification:
Clinical Exercise Physiologist (CEP) Certification through the American College of Sports Medicine is required within 1 year of hire date.
Basic Life Support and Advanced Cardiac Life Support required within 6 months of hire.
Minnesota Department of Health Limited-Scope X-ray licensure preferred within 2 years of hire.
