The DPC will have a variety of responsibilities, including but not limited to: • Managing Pharmacist scheduling and payroll for the district, including partnering with the DSPL regularly to ensure that District Support Pharmacists (DSPs) are scheduled appropriately to support pharmacy operations • Managing Vaccine Clinic Scheduling for flu season including inputting clinics into the schedule ideally 3 weeks out, checking the scheduling tool daily as many clinics drop in with a week or less from the date they would like to host, revising pharmacist schedule to cover clinics and contacting each pharmacist who was moved with the goal of reducing and preventing client escalations. If a pharmacist secures additional clinics on their own, the DPC will assist in finding coverage for the schedule if PM or Staff Pharmacists cannot cover it themselves. • Handling administrative activities (e.g., expenses, payroll, supply ordering, etc.) • Pulling, formatting, and analyzing reporting to highlight district and individual pharmacy opportunities for DL, RX, DSPL and PMs • Following up on outstanding issues (exception reporting, CRS audits, IT/service ticket escalations, etc.) • Managing calendar and email requests for the DL, Rx while they are engaged in store visits • Serving as "change champion" for the district, liaising with CSC partners to track and monitor progress on implementation of pilots and new program rollouts, while providing critical feedback to CSC project teams Primary Job Duties & Responsibilities Include: • Set and maintain Pharmacist schedule for stores in the district, including holes list and shift coverage, DSP Pharmacist calendar, Pharmacist vacation approvals and coverage coordination, and sick call coverage. Review communications from CSC (Weekly my Work Communication) and Region Director to brief DL, RX on key priorities and upcoming milestones. Set up video conference support and other supports, as needed, for various district conference calls and meetings. • Pull, manipulate, and analyze store- and district-level pharmacy reporting to help identify opportunities across the market. Complete district/region trackers, as required, and follow up with store teams on exception reporting, including training, mCC escalation management, etc. • Support staffing for the district, as needed, including opening and maintaining STARSource requisitions, screening candidates for exempt (RPh) and hourly (Rx Technicians) roles and attending career fairs/hiring events, as needed. Follow up on candidate progress and status with the Hiring Support Center. • Travel the market, as needed, with the DL, RX or independently to support pharmacy operations and planned/unplanned events (e.g., remodels, inventories, program rollouts, etc.). Access RxConnect to support planned inventories and program rollouts, as needed. Serve as a champion for change, promoting new programs and services, and helping colleagues understand the importance of agility in driving positive outcomes. Trainees will have 3 months to build up a minimal panel of 30 and 6 months to work up to a 50 member panel; Sharing and maintaining information with client's multidisciplinary team and implementing activities per CMP, including Community Supports; Support client engagement in support including coordination or medication review and or reconciliation, scheduling appointments, appointment reminders, coordinating transportation, accompany client to critical appointments, identify and address other barriers to client's engagement in services; Ensuring regular contact with the member and their family member(s), guardian, caregiver, and/or authorized support person(s) as part of care coordination; Engage and help client participate in and manage their care; Coaching members to make lifestyle choices based on healthy behavior - goal is for members to successfully monitor and manage their health; Supporting members in strengthening their skills to identify and access resources to assist them in managing and prevention of chronic condition; Linkage to resources based on member's needs such as smoking cessation, self-help recovery, etc.; Provide transitional care for clients during discharge from hospital or institutional setting including developing a transition care plan (Targeted Care Plan Update), and coordination of care to provide adherence support and referrals to appropriate resources and community supports, as needed; Identify supports needed for client; Collaboration with Community Supports provider and other community-based organizations to coordinate services; Provide appropriate education of the client and/or their family support/authorized support about care instructions for the person served; Assist members in accessing additional benefits and related documentation such as, Social Security Insurance (SSI), CalFresh, cash aid, and obtaining required documentation to apply (ID, birth certificate, immigration status, financial records, marriage/divorce records, proof of medical conditions, etc.; Develop, establish, and maintain professional and collaborative working relationships with internal and external care team; Network with community and stakeholders to remain current on issues and activities as they impact coordination of care for clients; Coordination of care with health plans; Attend required training to maintain provider certification and current industry knowledge; Performs administrative tasks including timely record keeping and data entry; Maintains up to date, adequate records and other documentation necessary for the collection of data and statistics pertaining to program outcomes, demographics, and information as required by funders; Collaborate as an active member of a team; Actively model and communicate the mission and vision and supports a corporate culture of empowerment, team building, and open communication; Maintains compliance with all applicable county, state and federal laws and regulations, funder and program requirements; Performs other duties as assigned. The successful candidate has: Associate's Degree in the social service field with one (1) year of experience in care coordination/case management/Community Health work preferred OR minimum 2 years of case management/community health work and care planning experience in lieu of Associate's Degree.
