    Coordinates/oversees a designated group of patients being treated by assigned Physical Therapy Assistants (PTA) and any related clinical supervision of such PTAs and provides appropriate therapy evaluations, treatments and modalities in accordance with established VNS health policies and procedures. Works under general supervision. • Establishes patient plan of care (POC) including treatment interventions, appropriate treatment outcomes and frequency and duration of treatment in collaboration with the physician. Coordinates patient POC and collaborates with other health care team members, patient and family/caregiver for optimal patient benefit. • Assesses, evaluates and identifies patient rehabilitation needs and potential using accepted practice standards. • Assesses/makes determination for direct physical therapy services to be provided by a PTA in accordance with VNSNY protocol. Supervises assigned PTA's, making co-visits and overseeing their schedules, caseloads, clinical skills and patient care delivery to ensure compliance with state, federal and VNS Health policy. • Establishes a discharge plan as part of the patient care continuum. • Provides care in accordance with established POC, including patient and family/caregiver teaching. • Evaluates the need for equipment orders and instructs in use of equipment as is appropriate for patient function and safety. Follows up to ensure resolution of problems/issues and adheres to VNS Health policies/procedures on equipment problems. • Assesses patient status and effectiveness of treatment interventions; modifies goals and Rehab POC as appropriate. • Identifies any changes in clinical and psychological status of patient and reports findings to appropriate members of the health care team to ensure quality care of VNS Health patients. • Refers assigned patients to other VNS Health services, as appropriate. • Initiates and maintains timely communications with the health care team to ensure coordinated quality patient care. Documents all evaluation findings, treatments and patient responses and communications regarding patient care within the timeframe established by VNSNY protocol. • Inputs/enters patient clinical information into appropriate Agency systems (i.e., pen-based, cyber, etc.) to ensure coordinated documentation and patient care. • Acts as coordinator of care when indicated according to VNS Health policy and procedures. • Participates in Rehabilitation Department and Agency meetings, in-service programs, Quality Assurance reviews, and interdisciplinary team meetings as requested. Participates in performance improvement via participation in co-visits and conferences with Rehabilitation Department supervisors. Collaborates as required in assigned service delivery team events. • Contributes to cost-effectiveness of services and programs of the Agency by maintaining knowledge of third-party payer regulations, and adhering to them. • Keeps abreast of the field of physical therapy, assumes responsibility for professional growth and maintains high level of clinical knowledge and skills. Supports the philosophy, mission, and vision of the Agency through attitude and work ethics. • Performs all duties inherent in a supervisory role. Ensures effective staff training, interviews candidates for employment, evaluates staff performance and recommends hiring, promotions, salary actions, and terminations, as appropriate. • Participates in special projects and performs other duties as assigned. Staffing ratios vary and can be 1:1 to 1:3 with 1:2 being the usual assignment. The ICU RN's are required to have a wide array of knowledge, skill and ability to competently and safely provide critical care to our patients. Frequent ongoing education, new skill acquisition and skill validation are provided and supported by an experienced ICU CNS for new and experienced ICU nurses. Newly hired RN's will have between 2-14 weeks of orientation depending on prior experience. Our preceptors are purposely selected for their teaching ability and formally trained to meet the training needs of new staff. Specific attention is provided for new graduate ICU RN's as they participate in our Vizient AACN Nurse Residency Program and AACN's essentials of Critical Care Orientation online education. Experienced RN's are encouraged and supported with educational and financial assistance to achieve critical care certification. ICU staff are supported by an interdisciplinary care team which in part includes unit based respiratory, physical, speech and occupational therapists. Interpreter, nutrition, palliative, chaplaincy and ethical services are readily available as well. Our ICU has onsite and eICU pulmonary critical care intensivists and their team of physician extenders 24/7.     About the Role The Touching Hearts at Home Franchise Operations Coordinator supports the day-to-day operations of the franchise office and strengthens franchisee relationships across the network. This role manages communication touchpoints, franchise documentation, event logistics, reporting, and general office operations to ensure franchisees feel supported, informed, and connected. Key Responsibilities Franchisee Support & Network Coordination Route and track incoming inquiries to the appropriate departments. Manage franchisee acknowledgements (anniversaries, milestones, recognition). Track non-legal customer, caregiver, or client dispute resolutions. Coordinate owner forums, peer groups, and special-topic calls, including scheduling, reminders, agendas, and attendance tracking. Documentation, Compliance & Reporting Maintain franchise documents, including agreements, renewals, amendments, and required filings. Collect and track Certificates of Insurance and monitor renewal deadlines. Support onboarding documentation for new franchise owners. Prepare weekly, monthly, and quarterly reports; maintain digital files; assist with audits and data requests. Conference, Training & Event Support Assist with National Conference logistics: registrations, sponsor coordination, swag inventory, printed materials, and on-site organization. Support Confirmation Day and Training Week through scheduling, communication, materials prep, and hospitality. Office & Operational Support Manage daily in-office operations, calendars, meetings, and internal communication. Maintain office supplies, equipment, vendor relationships, and general organization. Serve as the professional point of contact for calls, emails, and visitors. Coordinate printing and assembly of operations manuals, training binders, and franchise resources. Manage inventory of branded materials, swag, and supplies. Assist leadership with project coordination, follow-ups, and improving internal processes. What You Bring Strong organization, accuracy, and follow-through-details stay tight. A warm, professional communication style. Ability to juggle multiple deadlines and keep projects moving. Proficiency in Microsoft Office and willingness to learn new platforms. Administrative or operations experience; franchise experience is a plus. A proactive, service-oriented mindset with the ability to anticipate needs. Salary commensurate with experience.
