Job Summary
The Registered Nurse in Personal Care Services provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.
Specific Job Duties/Responsibilities
Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client's care team from admit through discharge.
Completes clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.
Ensures the patient/client's eligibility and medical necessity for services as defined by payer source and agency policy.
Develops and revises individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.

Position Overview
The Operations Apprentice will work alongside the farm's management team, including the owners, to learn all aspects of organic mushroom production and daily farm operations. Key Responsibilities
Assist with daily mushroom production tasks, including compost substrate preparation (mostly watering hay bales), spawning, casing, harvesting, packaging, and packaging prep (making/folding boxes).
Eventually learn and support the operation of climate-controlled growing rooms and monitoring systems.
Help maintain organic certification and food processing facility certification standards by following cleaning, sanitation, and record keeping practices.
Operate forklift and skid steer loader frequently under minimal supervision. Ability to use skid steer loader or willingness to learn.
Participate in routine maintenance of facilities and equipment. Help unload semi-trucks of inputs (peat moss, hay bales, chicken manure, etc.).
Adhere to all food safety and workplace safety protocols.
Qualifications
High school diploma or equivalent required; agricultural or technical coursework is a plus. Associate's or bachelor's degree in horticulture, botany, and/or conservation a plus.
Strong interest in sustainable farming and food production, ideally someone who is specifically interested in growing produce and understands the basic business/industry expectations and standard operating procedures.
Ability to perform physical tasks, including lifting 30-50 lbs repeatedly, standing most of the time on concrete, walking, twisting, climbing, and working in varying conditions. This is a job for those who wear Red Wings to work, not wingtips. ;) Everyone from the owner to part-time harvesters washes floors, hauls hoses, and cleans various work stations throughout the day.

A background in logistics, supply chain management, or warehouse management would be beneficial.
