The Community Care Manager provides trauma-informed, community-based support to survivors of sex trafficking. This role focuses on direct client care, case coordination, and resource navigation for individuals recovering from sexual exploitation. Work with the client, family, and other involved individuals, i.e. school personnel, probation officer, mentor, DHHS, medical personnel, etc. For the purposes of coordination of care, follow community partner, PIHP, CMH, and agency guidelines when providing treatment with patients receiving Medication-Assisted Treatment or other medical treatment. Provide education to clients, family, and other involved parties on MAT. Provide individual, family therapy, and case management as needed to each client/family on their caseload, with a minimum of one to two hours face to face contact each week. Coordinate the transfer/discharge process for the client and family, informing the Associate Director and the Admissions Department of projected transfer/discharge planning. Continuous Marketing of SA and Community programs, facilitate communication and good working relations between the agency and the community, including police, church, neighbors, volunteers, employers, medical personnel, and friends of individual clients. Complete all service plans, contact records, and all other necessary paperwork, in a timely manner. Develop Service Plans, Discharge Summaries, progress notes, and other documents as required by contract. Monitor the quality of overall treatment services to clients on their caseload; and evaluate the adequacy of these services to meet treatment objectives. Also, administers and assists in the coordination of GAIN and GPRA measures. Participates in regularly scheduled staff meetings, therapist meetings, and any others as deemed necessary by the Community Leadership. Job Description Youth Treatment Specialist (PT) - Boelkins Program What you'll do Working in one of our residential homes alongside our experienced care team, you'll implement individual treatment plans tailored to meet the needs of boys and girls ages 10-16 in our program.
