Position Summary: The HR Generalist (or Coordinator) plays a key role in supporting the day-to-day operations of the Human Resources department. This position is responsible for processing employee transactions, maintaining HRIS data integrity, generating and distributing reports for internal stakeholders, and supporting compliance and onboarding functions including I-9 documentation and background checks. Employee Transactions & HRIS Management:
Process employee life cycle transactions (hires, terminations, promotions, transfers, etc.) in the HRIS.
Maintain accurate and up-to-date employee records in the HRIS and personnel files.
Perform routine audits of HRIS data to ensure accuracy and compliance.
Reporting & Data Distribution:

Assist with data collection and reporting needs for internal departments.
Compliance & Recordkeeping:

Background Checks & Onboarding Support:

Qualifications
2+ years of experience in an HR support or coordinator role.
Working knowledge of HRIS systems (e.g., Paycom, Workday, ADP, UKG, etc.).

Position Summary: The case manager will provide support and service linkages to residents within the program. Coordinate services and ensure quality care to the residents.
Essential Responsibilities:
Responsible for the operation of the Supported Housing program and assigned caseload. Drive self and/or drive agency vehicle throughout boroughs to visit apartments routinely, to assess, observe and evaluate physical plan and residents functioning abilities and needs.
Act as a liaison with community health, mental health, educational, vocational and recreational programs.
Supervise and collect urine samples. Ensure procedure for accurate testing.
Provide linkage services with detoxification and rehabilitation treatment centers.
Assist consumers in the housing selection process including participation in the selection of adequate furnishings, working with real estates agents securing service from utility companies (telephone, electric), etc.

Responsible for reporting and documentation of all incidents to immediate supervisor and/or the Director of MHS within 24 hours.
On-call 24 hours a day, seven (7) days per week to respond to emergencies.
Weekly supervision of assigned staff, to delegate tasks and provide professional direction and technical assistance.
Required to participate and attend meetings/trainings at various sites that require following different route of travel to worksite.
Able to lift/carry packages and boxes of supplies; as well as clients if necessary.

ESSENTIAL ACCOUNTABILITIES:
Each DSP must be able to perform the following functions in order to ensure the life safety, care and well-being of the people served by WellLife Network.

6. Responsible for crisis intervention and incident reporting procedures.

The Care Coordinator will be responsible for providing comprehensive care management services to individuals across various healthcare settings in NYC or Long Island. POSITION SUMMARY:

In conjunction with the Physician and/or Medical Director/Assistant Medical Director develop and maintain a network of referral sources for primary and specialty care for use as needed to address medical referrals from the program.
Review lab and other medical reports and take actions as appropriate, i.e. contact primary care provider, to ensure appropriate follow up.
Provide psychoeducation to patients and families about diagnoses, medications, and treatment options
Collaborate with therapists, case managers, and other clinical staff to ensure coordinated care.
Meet the treatment team on a regular basis to keep them current regarding treatment plan and be apprised of any issues that may affect it.
Maintain thorough, accurate, and timely clinical documentation in accordance with OMH regulatory standards.
Participate in clinical team meetings, case conferences, and ongoing quality improvement initiatives.
Ensure care provided aligns with current best practices, ethical standards, and state/federal regulations.
Consult with affiliated psychiatrist and/or primary care /specialty physician re: medical services and/or diagnosis/treatment referrals that he/she provides.
Monitor client's psychiatric needs through direct observation and when necessary make appropriate referrals for psychiatric hospitalization or other levels of care.
Monitor treatment provided by collateral(s)/other team members by observing them for the purpose of giving instructions, providing feedback and adjusting practices.
Participate and attend meetings/ trainings at various sites.
Participate in OMH approved orientations and trainings.
As requested, provide training to clinical staff, i.e.: medication management, health care maintenance and/or special topics of interest
Must be able to communicate information verbally and in writing for the purposes of giving instructions, providing feedback and adjusting work practices.
Maintain computer literacy; E H R platform, order connect etc.
Maintain confidentiality at all times.
Represent agency and clinical services at inter-agency meetings and conferences.
Participate in all audit processes, including preparation of Plan of Corrective Action (POCA), if necessary.
Qualifications
Licensed and registered as a Nurse Practitioner in New York State is required.
1-2 years of experience providing psychiatric care to children and adolescents, preferably in an outpatient or community mental health setting.
Familiarity with MHOTRS regulations and service delivery models a plus
Current DEA registration and NPI number.
CPR/BLS certification.

POSITION SUMMARY: Ensures the facility is maintained in a clean and sanitary condition and in a state of good repair. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities.
ESSENTIAL ACCOUNTABILITIES:
1. Maintains a clean, sanitary environment. Sweeps, mops floors and disinfects bathrooms if events (i.e.: major floods) require it.
2. Responsible for ensuring that assigned site(s) is in good repair (i.e.: paint, plaster, repair furniture and property as needed). Ensure that site(s) are in compliance with all fire and life safety codes by conducting on-site inspections.
3. Complete facility specific inspection checklists on monthly basis including but not limited to vehicle, fire extinguisher, emergency light, clothes dryer vent, boiler/water temperature, and stair chair/wheelchair lift.
4. Completes work orders as assigned by supervisor (via text, verbally, in-person & hard-copy) and maintains record of all completed work.
5. Keeps appropriate records on all work performed and ensures that all necessary paperwork is completed.
6. Responsible for ensuring that emergencies and unsafe conditions at the site(s) are immediately addressed.
7. Keeps sidewalks, yards, stairs, driveways, and steps clean of debris. Duties may require shoveling snow, as well as salting and sanding areas.
8. Responsible for the storage and inventory of all necessary maintenance equipment and submission of required reports.
9. Keeps bathrooms stocked with paper towels, toilet tissue and soap (site specific).
10. Tracks inventory of janitorial supplies and submits required reports (site specific).
11. Required to lift heavy objects such as furniture, garbage, appliances, etc. up to fifty pounds (50 lbs.).
12. Must be able to bend, crouch and work in a kneeling position.
13. Must be able to carry & set a ladder, ascend and descent ladders, stairs and stepstools in accordance with OSHA guidelines.
14. Must be able to access confined spaces and perform work as needed in areas such as attics, crawlspaces, beneath sinks & kitchen cabinetry.
15. Checks air conditioning/heating systems, hot water heater, pipes for leaks (water, gas & sewer), laundry equipment (including vents & appliances, and other site equipment for signs of wear and tear and alerts supervisor to the condition.
16. Attend certificate and non-certificate classes as well as other trainings as required.
17. Required to work during emergencies such as snow/inclement weather, weekends & holidays.
18. Must attend monthly staff meeting.

21. Position may require driving an agency vehicle.

Position Summary: The Peer Specialist will provide direct mental health services to recipients participating in rehabilitation and recovery services. The Peer Specialist will perform a range of tasks which are designed to assist recipients in regaining personal control over their lives and over their own recovery processes. Essential Accountabilities:
Teach skills to individuals and groups.
