    Background Our therapeutic shelter is a safe and supportive residential facility that provides case management and intensive treatment services for adult men age 18 and over. This is a short-term program (10-14 days on average) for those persons contemplating a step towards recovery and who are feeling challenged by residing in the community and maintaining abstinence, or for those who have completed formal treatment and are awaiting placement in the next level of care. Twenty-four-hour treatment services are provided by a professional, culturally sensitive staff. As a member of the treatment team, the Residential Aide (Shift Monitor) is responsible for the well-being and safety of the clients while assisting with and supervising client activities. As a Residential Aide, you will remain awake on all designated shifts. Hours: Per diem: weekends, first and second, 3rd shift Priority given to those that would like to work -Third Shift - 12am-8am weekends Location: Waterbury, CT Pay rate: $17/hr. Duties & Responsibilities Develops positive working relationships with supervisor, co-workers and members of the clinical team. Develops appropriate therapeutic relationships with clients • Abides by the Code of Ethics. • Facilitates assigned educational groups and community meetings. • Supervises visiting hours. • Models appropriate professional behavior • Collects/administers urinalysis and breathalyzer tests. • Supervises meal and snack times. • Supervises self-administration of medication. • Routine bed checks, house rounds and documentation of client counts. • Makes clear and detailed entries in staff log and appropriate files. • Attend staff meetings. Requirements/Experience: High School Diploma, prefer minimum 2 years college in counseling or related field. Position Summary: This position will be responsible for the direct nursing needs of the clients in the Older Adult Care Team and other programs, or nurse related initiatives as needed, and documentation of nurse care management services into the EMR. The Registered Nurse will partner with agency nursing staff to provide guidance and oversight on the medical/health/wellness assessment of clients to identify health care needs; develop health care goals/objectives as part of the treatment plan and connect patients to primary care and medical specialty services, as needed. Develops and delivers wellness, health promotion, and health education and prevention initiatives, and provides clinical guidance and oversight of all nursing documentation. Essential Responsibilities: Conducts home visits, as needed to assess clients and engage clients in coordinated/managed primary and behavioral health treatment. Assists in coordination of care with medical providers, health care agencies, specialty providers and hospitals for admission and discharge Assesses client's ability to maintain in the home completing ASCEND/PASSR applications if needed, referring to specialty care, and communicating to staff and client family the needs of the client to remain in the home. Conducts medication reconciliation between psychotropic and medical medications, particularly after patient hospitalizations. Develops and maintains working relationships with primary and specialty care providers including inpatient facilities. Monitors and reports on primary care performance ensuring outcomes and deliverables are met and leads to improvement efforts. Provides education and clinical support to agency nursing staff. Educates nursing staff on integrated treatments ensuring medical/ health/wellness goals and objectives are completed for agency clients. Provides training on medical disease, treatments & medications to clients and staff, as needed. Assists in the organization and facilitation of the agency medical clinics (Flu Clinic, PPD Clinic, wellness activities). Develops and/or revises clinical and administrative policies ensuring adequacy of practices and standards and compliance with oversight bodies, payers and licensing regulations. Monitors documentation of medical/health/wellness assessments completed by nursing staff to assist in risk management for organization. Tracks required health care assessment and screenings. Audits client charts to ensure that medication reconciliation between psychotropic and medical medications, particularly after patient hospitalizations. Works with Residential management to ensure that all medication management policies and procedures are being adhered to and followed. Attends and provides consultation at team meetings.
