Job DescriptionJob Summary: Provides coaching, supervision, and casework services to clients in a Group Home while managing all aspects of the program within the home.
Supervisory Responsibilities:
Trains new Child Wellness Coaches and Coordinators in accordance with VQ, state, and federal requirements. Provides coaching and supervision of clients in individual and group settings as part of a multidisciplinary team to achieve treatment goals. Provides supportive individualized coaching and training in daily living skills such as personal hygiene, nutrition, budgeting, and food preparation and purchase. Additionally, actively supporting the member in planning and achieving their treatment plan goals.


Duties/Responsibilities:
Responsible for overall compliance of the program while adhering to licensing standards, contract, TI Model, and VisionQuest policies and procedures. Ensure all shifts are covered and and all staff ratios are adhered to (this may include covering a shift for a coach or coordinator). Being available after hours for if staff need assistance with incidents, crisis episodes or staff shortages. Presents clear, informative, accurate, and prompt observations (written and verbal) including documentation of member participation and other relevant information as primarily assessed through member report and/or observation. Must maintain member records according to established policies and procedures. Conducts and participates in audits. Serves as liaison between member’s team, stakeholders, and VisionQuest. Maintains professionalism in all interactions and documentation. Provides supervision, direction, and support to all wellness coaches and clients. Intervenes in crisis situations and demonstrates ability to provide individualized crisis management through appropriate de-escalation techniques. Observes and reports health safety functions including medication related issues, fire safety and emergency response, infection control, and any other incidents outlined in the policies and procedures that may risk the safety of our members. Maintains accountability of the members and Wellness Coach to our program structure and scheduled activities. This may include providing collaborative leadership and role-modeling. Completes all necessary documentation for members during their residence, this may include development of individualized treatment plan, intake and discharge documentation, and any other documentation required by the agency, state, and federal policies. Transports clients in agency vehicles as necessary. Wellness Coach may be asked to utilize their own personal vehicle, if approved according to the company’s transportation policy. Attends and participates in in-service training and Wellness Coach meetings. Will assist in orientation, training and evaluation of new and current Wellness Coach Ensures that all equipment and vehicles are being maintained. This may include general upkeep of equipment and vehicle. An individual may qualify for the position if the individual is able to “perform safely” the essential functions of the position with or without a reasonable accommodation. “Perform safely” means to perform without causing a direct threat to the health or safety of the individual or others that cannot be eliminated by reasonable accommodation, as defined by 42 U.S.C. #12111(b) and the EEOC’s regulations, 29 C.F.R #1630.2(r). Required Skills/Abilities:

Possesses basic knowledge of the behavioral attributes of mental health disorders that children and adolescents may be diagnosed. We are currently seeking a Wellness Program Coordinator for our Connections for Living (Memory Care) Department at our community, The Commons at Dallas Ranch, in Antioch, CA.

Schedule: Tuesday - Saturday, 8am to 4:30pm

At MBK, we believe that every resident has the potential to experience joy and pleasure, connection with others, purpose, and self-esteem at every point on the spectrum of the cognitive changes we call dementia. The purpose of the Wellness Program Leader in memory care is to uncover and develop each resident's potential to have these positive daily experiences and to grow. This goal is achieved by creatively planning and implementing a daily Wellness Program using the structures and content provided by MBK.

Essential Job DutiesUnderstand each resident's wellness needs and document them in the Service Plan - 20% :Review the Lifestyle Survey for each new resident prior to move-in and create a plan to welcome and integrate the resident into the Wellness Program beginning on move-in day Identify resident's interests, needs, and potential roles in the first 30 days after move in and incorporate them into the neighborhood's Wellness Program plan Document Wellness Program services provided in each resident's Service Plan Ensure that every resident has large group, small group, and/or individual experiences included in their Service Plan Assist the team to find better ways to meet residents' needs when they express distress through verbal or nonverbal communication Plan and implement Wellness Program Daily Flow - 60% :Plan individual, small group, and large group experiences by reviewing Lifestyle Surveys and finding common needs and interests Create and execute daily Wellness Program using the Daily Flow structure Optimize the use of MBK-provided content, ie. iN2L, Eversound, Joy for All Companion Pets, SingFit, etc. Provide training and support to ensure that Wellness Programs are conducted when the Wellness Program Leader is not present, including evenings and days off Provide training and supplies to enable all memory care team members to support residents' greatest potential - 10%:Encourage and instruct all memory care team members to build relationships with residents, communicate effectively with them, and support their abilities Ensure that adequate and appropriate supplies are always available for team members to initiate individual, small group, and large group experiences Perform administrative tasks necessary to maintain role requirements - 5%: Attend required Teams calls and trainings Contribute to inter-community discussions and projects on Teams groups and channels Contribute content to monthly community newsletter Effectively manage program supplies and services budget Support community sales and marketing efforts by effectively demonstrating programs and providing materials to post on social media - 5%: Highlight Wellness Programs during tours or when other guests are present Take videos and photos of Wellness Programs and provide them for posting on the community's Facebook page or other social media
