How you'll contribute A Discharge Coordinator who excels in this role: * Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge * Creates and maintains up to date aftercare coordination community resources * Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients' provider and therapist. * Provides resource information to patients and families as appropriate for future care needs. * Actively communicates with clients, families, aftercare providers, and referral sources. What we're looking for Applicants should have a Bachelor's Degree in Social Work, Counseling, Psychology, or at least three years of clinical experience within an inpatient behavioral health setting.
