The minimum qualifications for Special Education Teacher:

This role frequent travel. You're expected to drive for several hours each day, normally conducting 3-5 home visits (dependent on travel times).
Job requirements
What You'll Bring
Must hold an active Registered Nurse (RN) License in the state of employment
Minimum two (2) years of experience in direct care of elders or adults with disabilities
Ability to travel (usually by automobile) within assigned geographic area; valid driver's license and automobile insurance is required
Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a home care setting
Proficiency in common software and web-based applications (e.g. MS Office, case management systems, PCP order forms etc)
Experience with Minimum Data Set (MDS) assessment tool a plus.
Assess client status upon initial home visit to identify needs in order to develop a comprehensive case management plan.
Using basic teaching principles, counsel and instruct the patient, family and caregiver in meeting nursing and related needs.
Document in an accurate and timely manner. Maintain a clinical record for each morbidity patient.
Completing the MDS-HC and other nursing assessments while coordinating all other applicable clinical assessments.
Training, evaluating, and supervising caregivers.
Monitoring the health status of all members to ensure that all needed care is properly delivered.
Proficiency in common software and web-based applications including Microsoft Outlook, Excel, Word, PowerPoint, and Teams.
Additional state-specific qualifications may be required.

Conduct home visits to assess and create personalized care plans.
Train and support caregivers, ensuring they can meet their loved ones' needs.
Monitor and report on clients' health, coordinating with their primary care physicians.
Facilitate smooth transitions and ongoing care for clients.
Maintain compliance with all relevant regulations and standards.
