Job Summary
Plans, organizes and delivers physical therapy programs that help clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions clients/patients/residents who are affected by injury or disease. Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's/resident's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients/residents.
May treat individuals of all ages with varying diagnosis and disorders. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients/residents. Follows and adheres to accepted clinical practice guidelines of professional organization.
Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department, Good Samaritan Society and Sanford Health. Will be interacting with individuals outside of the department including but not limited to: providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. The Senior Regulatory Expert is a recognized thought leader and subject matter expert (SME) who is responsible for driving Assent's objective of being a market leading software and services solution for a wide variety of customers and industries. The Brooklyn Bridge Alliance for Youth (BBAY) is a joint powers agreement between the City of Brooklyn Center, Brooklyn Park, Hennepin County, and the school districts of Osseo, Brooklyn Center, Robbinsdale and Anoka-Hennepin, Hennepin Technical College & North Hennepin Community College. Our mission is to coordinate a system of high-quality, accessible, and fully resourced opportunities that lead to an increase in high school graduation and pathways to college or career and safety & wellbeing.
The BBAY uses a collective impact framework for shared action. The Youth Engagement Specialist's position will be responsible for a robust recruitment and outreach strategy for youth in Brooklyn Center and Brooklyn Park at their schools and across partner organizations to participate in conversations, opportunities, initiatives, and programs available at the Brooklyn Bridge Alliance for Youth and its partner organizations.
The primary charge of the Youth Engagement Specialist is to provide school-based, youth-engagement facilitation by:
* Building relationships with youth and their families to help recruit and connect them to opportunities with the Reimagine Black Youth Mental Health Initiative, and other opportunities as they emerge.
* Outreach & Recruiting with the intention of including youth most disenfranchised.
* Facilitating youth-centered spaces, events, and activities.
* Supporting the work of the Reimaging Black Youth Mental Health (RBYMH) Coordinator.
The Reimagine Black Youth Mental Health initiative centers youth from across the Black diaspora to participate in a variety of activities that tend to their wellbeing in the moment and give them opportunities to strengthen lasting support for their wellbeing overtime. Some of these activities include community-building field trips related to Black culture or mental health, overnight retreats related to Black Joy or policy making, attending conferences to understand and identify policies that support well-being, collective work with Initiative Advisory Council, and recruiting other Black youth to participate in demonstrations of change. The specialist will support these efforts and leverage adults to help build a network of partners who will also recruit and support youth in participating.
This position is an essential part of the Alliance Team, which includes an Executive Director, Data and Quality Coordinator, Youth and Community Engagement Initiatives Coordinator and the Black Youth Mental Health Coordinator. This position will report to the BYMH Coordinator.
The ideal candidate has a passion for serving and a relationship with the Brooklyn Center and Brooklyn Park community. This individual is also responsible for maintaining a clean and neat area in the pharmacy and work areas. Additionally, this individual is also responsible for greeting all customers and offering assistance.
A few of the primary responsibilities include:
Providing excellent customer service to customers by building loyalty and repeat business.
Champions Thrifty White programs to patients such as Ready Refill, Medication Synchronization and HealthyPackRx and assists with enrolling patients.
Receives, unpacks and checks merchandise to verify all merchandise is received and in acceptable condition. Stocks, cleans and sorts pharmacy product.
Responsibilities may include health aids department maintenance.
Able to learn and implement new and changing technologies on an ongoing basis.
Able to maintain flexibility in new or changing work roles and the modification of job responsibilities as required.
Willing to help anywhere in the store or nearby locations as needed.
Good attendance and punctuality is required in order to fulfill the essential job functions.

Responsibilities include obtaining and organizing patient information for outreach clinics, assisting with scheduling, coordinating outreach provider schedules and calendars, and keeping them up to date. The role also manages clinical organization by coordinating and scheduling procedures, retrieving necessary data, and completing data entry. Candidates must have a high school diploma or equivalent. Medical Assistant (MA) certification is preferred, but not required. Previous experience in health care is preferred.
