How you'll contribute
The Physical Therapist (PT) assumes responsibility and accountability for a designated group of patients and provides physical therapy evaluations, modalities, and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care. Assists patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
Observes and evaluates treatment affect, accomplishment of short and long term goals. Recommends change to physician, if needed.
Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
Formulates and updates patient specific plan of care for physical therapy
Reports patient progress and barriers to discharge in each patient team conference
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation.
Demonstrates knowledge, competency and proficiency of physical therapy modalities.

Roles & Responsibilities:
Primary Duties:
· If required for the position, train and qualify as a vessel operator for HSMST, QST A/B/I, FACT, and SDST Targets. When assigned to these small boats, personnel will serve as captains; when aboard larger vessels, such as the SL-120, they will serve as mates. Tasks typically involve assisting higher level technicians in activities such as build-up or replacing seaborne target components, marine engines, troubleshooting systems and subsystems, repairing simple machinery equipment; using tools and common test equipment; and receiving technical guidance, as required.
· Under supervision, apply technical working knowledge to perform tasks on various types of marine engine on seaborne target systems and subsystems (QST A/B/I, FACT, HSMST, and SDST Targets).
· Follow established preventative maintenance programs utilizing technical service manuals and schematic charts for marine engines and related marine equipment.
Additional Duties:
· Operate and be qualified (technically and medically) in forklift operations, overhead crane operations, marine travel lift, and other target specific equipment.
· Maintain equipment logs, maintenance action forms and worktime tracking sheets.
If required for the position, qualify and maintain Portable Command and Control Unit (PCCU) Remote Control Operator (RCO) certification.

• Attend staff meetings and trainings as scheduled and required. Support Leaders/Directors in implementation of meetings and trainings.
• Adheres to YMCA program standards regarding safety and risk management.
• Act as representative of the YMCA of the Foothills at all times. Conveys information on programs and schedules and refers members to other programs as appropriate. Articulate the mission, cause and rally cry of the YMCA of the Foothills.
• Record and maintain daily attendance and other documentation as required in a timely manner.
• Ensure that classroom and program spaces are clean, organized and safe.
• Follows YMCA policies and procedures, including those related to medical and disciplinary situations, child abuse prevention, emergencies, dress guidelines and cleanliness standards.

Qualifications
QUALIFICATIONS:
• Minimum two (1) year of experience working or participating in a youth group/program setting. YMCA program participation preferred.
• HS Diploma or GED Equivalent required. College completion in a related field preferred; equivalent work experience will be considered.
• Minimum 18 years of age. • Proven ability to coordinate, implement, and/or lead enrichment/recreational activities.
• Self-starter who is committed and possess good communication and teamwork skills.
• Energetic, youth-friendly and creative person with a high level of enthusiasm about providing meaningful opportunities and activities for youth.
• Ability to work effectively with people of different backgrounds, abilities, opinions and perceptions.
• Ability to communicate verbally to large and small groups of members.
• Possess imagination, a sense of humor, patience, conviction, sincerity, enthusiasm, initiative, self-control, adaptability, willingness to learn, integrity and ingenuity.
• Must possess the four essential characteristics for dealing with the growth of youth into adulthood: good health, intelligence, moral character and personality.
• Ability to follow and accept instruction and relate well with peers.
• Familiarity working with schools and community-based agencies preferred.
• Must be able to model the core values, Caring, Respect, Responsibility and Honesty.
• Must have commitment to the mission and cause of the YMCA and uphold its values and ethics.
• Must have clearance through fingerprinting TB testing, drug testing, criminal records and child abuse index check.
