What You'll Do In This Role: Perform blood glucose monitoring, inhalation treatments, and other physical prescribed medical treatments/diagnostic test. Maintains medication and resident's records, responsible for electronic input of resident's data in EHR system. Transmit requests including scans and/or faxes medication refill requests per policy to ensure proper medication administration to assigned residents. Communicates and coordinates care with residents and their families. Documents and leads in the documentation of resident care. Inspects the environment, identifies environmental problems and initiates requests for correction of environmental problems. Identifies, reports and documents incidents and accidents; participates in investigation of incidents and accidents as necessary. Completes records and reports as needed. Identifies clinical and operational problems, and works with team to identify and implement solutions. Reviews, transcribes and implements physician orders. Communicates with resident's physicians as directed. Implements, practices and assures staff adheres to infection control measures. Lead CNAs (Certified Nursing Assistant). Assisting with daily needs such as bathing and dressing. Participates in the orientation of new employees. Participates in continuing education programs to maintain competency. Support and participate in community and departmental quality assurance programs. Ensures that unit practices are consistent with regulatory requirements. Perform all other duties as assigned. Key Responsibilities * Business Development & Relationship Management: * Proactively develop and execute a strategic marketing plan to grow and maintain a consistent volume of quality referrals from hospitals, physicians, case managers, social workers, and other healthcare providers. * Function as the primary sales and marketing representative, identifying new referral sources and cultivating new and existing relationships. * Analyze market data and referral trends to identify opportunities for growth * Utilize Customer Relationship Management (CRM) software to track referral source activity, manage a pipeline, and report on key performance indicators. * Develop and maintain strong, collaborative relationships with referral sources, including hospital staff (nurses, social workers, discharge planners, physicians) and hospice agencies. * Represent the Inpatient Hospice House at community events, health fairs, and professional conferences to promote awareness and understanding of hospice services. * Provide educational in-services and presentations to healthcare professionals on hospice care, admission criteria, and the benefits of the Hospice House of Hampton Roads. * Referral Management: * Respond promptly to incoming referrals from hospitals, hospice agencies, and other healthcare facilities. * Conduct comprehensive clinical assessments of prospective patients to determine their eligibility for inpatient hospice care as well as the organization's admission criteria. * Collaborate with attending hospice providers, discharge planners, social workers, and case managers to gather necessary medical information and ensure appropriate patient placement. * Facilitate timely admissions and execute on the census goals. * Clinical Assessment and Care Transition Coordination: * Conduct timely, comprehensive pre-admission clinical assessments of prospective patients in referring facilities (hospitals, agencies, etc.) to determine their clinical appropriateness for inpatient hospice care. * Review patient medical records, including physician orders, history and physicals, and therapy or nursing notes, to ensure they meet admission criteria. * Collaborate effectively with the interdisciplinary team at the referring facility to gather necessary clinical and financial information. * Clearly articulate our facility's services, programs, and clinical outcomes to referral sources, patients, and their families. * Patient and Family Education & Support: * Serve as the primary point of contact for patients and their families regarding inpatient hospice needs. * Educate patients and families on the hospice philosophy, available services, and goals of care in a clear, compassionate, and culturally sensitive manner. * Address questions and concerns, manage expectations, and end of live goals working with the administrator for a collaborative approach or solution. * Coordination of Care: * Ensure nursing assessments, judgments, and treatment decisions are based on patient needs and wishes before admission to the Hospice House. * Ensure all required admission documentation is completed accurately and submitted promptly. * Coordinates with interdisciplinary teams, including physician approval of plans for continuity of care, and coordinates community hospice agencies. * Administrative & Compliance: * Maintain accurate records of referral activities, patient interactions, and admissions. * Stay up-to-date on all regulatory requirements and levels of care (Medicare Hospice Benefit, Medicaid, private insurance). * Adhere to all HIPAA regulations and maintain patient confidentiality. * Participate in staff meetings, educational programs, and quality improvement initiatives. * Clinical Support * Displays a willingness to support policies and procedures and use appropriate channels for change of such policies * Conducts and maintains documentation of care conferences and in-services. * Immediately reports accidents, incidents, lost articles, or unusual occurrences to the Administrator. * Identifies, investigates, and promotes plans of correction to improve resident care with the Administrator. * Attends pertinent continuing education programs, participates in community education projects, and shares information with staff. * Answers correspondence, email, and voicemail appropriately and promptly. * Ensures that verbal and written communications are understandable. * Adheres to the HHHR code of ethical behavior and models our core values of respect, passion, and professionalism at all times.
