The Internal Auditor will increase the value of services, ensure stronger compliance with regulations and policies, and reduce company risk. The Internal Auditor will be given the authority to discover opportunities for improvement and drive enduring change. The position will report directly to the CEO and FO, who will assist as necessary in helping the role make a real and lasting difference in our business operations.
What You'll Do In This Role
Assist in conducting financial and operational audits in accordance with established audit plans and priorities.
Perform audit testing under supervision, including walk-throughs, sample testing, and documentation of results.
Prepare and maintain audit work papers that clearly document procedures performed, evidence obtained, and conclusions reached.
Assist in evaluating internal controls and identifying opportunities for process improvement.
Support compliance reviews to ensure adherence to internal policies, procedures, and applicable federal and state regulations.
Participate in process observations and interviews with staff to gain an understanding of operational workflows.
Assist in identifying potential control gaps, inefficiencies, or documentation issues and communicate observations to the CFO and CEO.
Support the development and refinement of audit programs, templates, and documentation standards.
Assist with external audit preparation by compiling schedules, supporting documentation, and responding to auditor requests.
Maintain confidentiality of organizational records, systems, and audit information.

Responsibilities:
Our Maintenance Mechanic performs the work involving maintenance or craft occupations to keep the electric and mechanical equipment and appliances in apartments and on-campus operating and in good repair. The Outreach Liaison serves as a vital link between the Hospice House, hospitals, hospice physicians, hospice agencies, and the community. This role is responsible for identifying, assessing, and facilitating the admission of eligible patients into the Hospice House, particularly for general inpatient services (GIP) when appropriate and respite care. The IPU Outreach Clinical Liaison provides education to patients, families, and healthcare professionals about hospice philosophy, services, and benefits, ensuring a seamless transition of care and promoting the organization's mission of compassionate end-of-life care and family support.
Key Responsibilities
* Business Development & Relationship Management:
* Proactively develop and execute a strategic marketing plan to grow and maintain a consistent volume of quality referrals from hospitals, physicians, case managers, social workers, and other healthcare providers.
* Function as the primary sales and marketing representative, identifying new referral sources and cultivating new and existing relationships.
* Analyze market data and referral trends to identify opportunities for growth
* Utilize Customer Relationship Management (CRM) software to track referral source activity, manage a pipeline, and report on key performance indicators.
* Develop and maintain strong, collaborative relationships with referral sources, including hospital staff (nurses, social workers, discharge planners, physicians) and hospice agencies.
* Represent the Inpatient Hospice House at community events, health fairs, and professional conferences to promote awareness and understanding of hospice services.
* Provide educational in-services and presentations to healthcare professionals on hospice care, admission criteria, and the benefits of the Hospice House of Hampton Roads.
* Referral Management:
* Respond promptly to incoming referrals from hospitals, hospice agencies, and other healthcare facilities.
* Conduct comprehensive clinical assessments of prospective patients to determine their eligibility for inpatient hospice care as well as the organization's admission criteria.
* Collaborate with attending hospice providers, discharge planners, social workers, and case managers to gather necessary medical information and ensure appropriate patient placement.
* Facilitate timely admissions and execute on the census goals.
* Clinical Assessment and Care Transition Coordination:
* Conduct timely, comprehensive pre-admission clinical assessments of prospective patients in referring facilities (hospitals, agencies, etc.) to determine their clinical appropriateness for inpatient hospice care.
* Review patient medical records, including physician orders, history and physicals, and therapy or nursing notes, to ensure they meet admission criteria.
* Collaborate effectively with the interdisciplinary team at the referring facility to gather necessary clinical and financial information.
* Clearly articulate our facility's services, programs, and clinical outcomes to referral sources, patients, and their families.
* Patient and Family Education & Support:
* Serve as the primary point of contact for patients and their families regarding inpatient hospice needs.
* Educate patients and families on the hospice philosophy, available services, and goals of care in a clear, compassionate, and culturally sensitive manner.
* Address questions and concerns, manage expectations, and end of live goals working with the administrator for a collaborative approach or solution.
* Coordination of Care:
* Ensure nursing assessments, judgments, and treatment decisions are based on patient needs and wishes before admission to the Hospice House.
* Ensure all required admission documentation is completed accurately and submitted promptly.
* Coordinates with interdisciplinary teams, including physician approval of plans for continuity of care, and coordinates community hospice agencies.
* Administrative & Compliance:
* Maintain accurate records of referral activities, patient interactions, and admissions.
* Stay up-to-date on all regulatory requirements and levels of care (Medicare Hospice Benefit, Medicaid, private insurance).
* Adhere to all HIPAA regulations and maintain patient confidentiality.
* Participate in staff meetings, educational programs, and quality improvement initiatives.
* Clinical Support
* Displays a willingness to support policies and procedures and use appropriate channels for change of such policies
* Conducts and maintains documentation of care conferences and in-services.

Under the direction of the Therapeutic Recreation Supervisor, implement the therapeutic recreational needs of the residents to include physical, social, emotional, vocational, spiritual and cognitive needs.
What You'll Do In This Role
Implement individual recreational program for each resident as part of their overall plan of care.
Provide the documentation for programs & services.
Provide pertinent input to Therapeutic Recreation Supervisor regarding changes and modifications in the activity plan of care that will assist in overall program and service improvements.

What You'll Do In This Role
* Screen, interview, select and train new team members.
* Conduct performance evaluations and discipline employees in accordance with established policies and procedures.
* Maintain employee files, documenting such personnel standards of conduct issues such as tardiness, absences, counseling sessions, etc.
* Ensures all policies and procedures are adhered to.
* Prepare payroll for submission. Monitor and report department overtime. Complete employee schedule within budget guidelines.
* Coordinates daily "Showtime" meetings utilizing training and customer service goals.
* Participate in all scheduled Dining Services Manager meetings and Nutrition Care Team Leadership meetings. Conduct monthly "Chat" sessions as assigned.
* Represent Dining Services in all areas of Westminster Canterbury with a positive manner, using established customer service guidelines.
* Conduct audits during meal service to monitor staff compliance with safety and sanitation guidelines.
* Participate in the Performance Improvement Program as assigned.
* Complete month-end inventory.
* Conduct mandatory in-services and ensure completion of all compliance requirements, i.e., annual PPD's, COVID testing, etc.
* Demonstrate knowledge of all therapeutic diets and the proper food/condiments served with each diet. Demonstrate knowledge of proper food portioning of all items for residents.
* Demonstrate knowledge of infection control and sanitation with regard to food temperatures, hand washing techniques, use of gloves and dating of food items.
* Accurately identify standard temperatures for refrigerated food storage. Demonstrate knowledge of proper chemical usage and MSDS.
* Follow prescribed work assignments daily, maintaining proper time frames and completing all required duties, and all other duties as assigned.

Position Schedule: Part-Time, Nights (4-7PM), Weekends
Reports To: Nutrition Care Supervisor, Team Leader and/or Nutrition Care Manager
The primary purpose of this position is to create an enjoyable experience for residents and their guests of the Lighthouses of the Hoy Center and Enhanced Services at Westminster Canterbury on the Chesapeake Bay. Each of these areas will service anywhere between 12 and 30 individuals at a time by providing quality food and service at all times. The Nutrition Care Partner will be assigned to one of the areas to prepare, cook, serve and/or deliver food in accordance with the current federal, state and local standards, guidelines and regulations, along with our established policies and procedures. This position follows established culinary instruction in preparation of meals, maintains meal schedules, assists in clean up, kitchen sanitation and maintenance and setting up and taking down the dining areas. Other job duties include delivering food trays to residents' rooms, preparing daily milkshakes and snacks and taking daily inventory of food and kitchen supplies in the Lighthouses of the Hoy Center and Enhanced Services. You will be part of a program that is unique to the needs and desires of our residents and the strengths, talents and skills of your team.
Job Duties
1. Responsible for preparing and cooking small quantities of a variety of made to order breakfast items (i.e. omelets, scrambled, fried, poached eggs, pancakes, waffles) and sandwiches.
2. Prepares milkshakes, smoothies and snacks, and will occasionally be involved in preparing breads, cookies, appetizers for resident afternoon events.
3. Delivers food trays to residents' rooms
4. Takes daily inventory of food and kitchen supplies in the Lighthouses of the Hoy Center and Enhanced Service kitchens.
5. Prepares garnishes as directed and utilizes them to enhance plate presentation.
6. Represents Dining Services in all areas of Westminster Canterbury with a positive attitude, using established customer service guidelines, displaying a positive dining experience by displaying a cheerful demeanor when serving residents and following established procedures for food handling, plate presentation and service.
7. Demonstrates knowledge of all therapeutic diets and the proper food/condiments served with each diet.
8. Demonstrates knowledge of proper food portioning of all items for residents.
9. Provides food and beverages for Quench Station and other afternoon activities
10. Demonstrates knowledge of infection control and sanitation with regard to food temperatures, hand washing techniques, use of gloves and dating of food items.
11. Completes temperature logs of food and kitchen equipment utilizing the HACCP Manager Data System.
12. Completes production waste logs, PAR level sheets and all forms relating to the meal service in a timely manner.
13. Demonstrates knowledge of proper chemical usage and SDS.
14. Cleans and organizes work area to provide a clean, safe and neat environment for residents, self and staff.
15. Maintains assigned dining areas by cleaning and resetting tables prior to each meal period.
16. Rotates and stocks work area prior to each meal period.
17. Participates in monthly inventory process of the Lighthouses of the Hoy Center and Enhanced Services, as assigned by supervisor.
18. Demonstrates knowledge of fire safety procedures and assists with evacuation of kitchen and dining areas in the event of fire or other emergency.
19. Builds and develops relationships with Nursing, Therapy and Housekeeping, all collaborating to provide the resident with a high quality dining and living experience.
20. Independently handles basic problems. Confers with Nutrition Care Supervisory staff regarding unusual problems.
21. Demonstrates dependability and meets established criteria for good attendance and punctuality.
22. Adheres to Dining Services dress code, maintaining a clean and neat appearance at all times.
23. Attends all Westminster Canterbury mandatory in-services and mandatory Nutrition Care Team meetings as scheduled.
24. Performs other duties as assigned.
PHYSICAL REQUIREMENTS
• Must be at least 18 years old.
• Must be able to lift up to twenty five (25) pounds or more.
• Must be able to push and pull carts/objects.
• Must be able to stand up to 8 hours consecutively.
• Must be capable of standing and maneuvering independently and safely around work area.
• Sufficient manual dexterity to operate a fire extinguisher and to manipulate kitchen equipment including stovetop, oven, waffle iron, griddle, Panini, milkshake machine, blenders, can openers, knives, etc.

The RN Case Manager (Home Health) provides ongoing consultation and education for patients/family members regarding all aspects of a terminal diagnosis. Communicate and collaborate with other members of the hospice interdisciplinary team. What You'll Do In This Role
* Provide initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psycho-social and environmental needs and activities of daily living. Assessment includes risk for pathological grief, cultural and spiritual implications, and verbal and non-verbal communications patterns.
* Implement the individualized plan of care and recommend revisions to the plan as necessary, including managing discomfort and providing symptom relief and providing specialized nursing skills related to palliative and end-of-life care.
* Consult with and educate the patient/family regarding the disease process, self-care techniques, end-of-life care, and the process for dealing with issues of ethical concern.
* Initiate appropriate preventive and rehabilitative nursing procedures.
* Prepare clinical and progress notes that demonstrate progress toward established goals.
* Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team.
* Use the case management approach and refer to other services as needed.
* Inform the physician and other personnel of changes in the patient's needs and outcomes of intervention.
* Determine scope and frequency of services needed based on acuity and patient/family needs.
* Supervise LPNs and paraprofessionals providing services to the patient according to regulatory guidelines.
