An Acute Targeted Case Manager shall have one of the following groups of minimum qualifications: Master's degree in social work, psychology, rehabilitation, activity therapies, counseling; OR Bachelor's Degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science AND three years of direct experience in behavioral health, of which two years must be experience working with or providing services to adult individuals with serious mental illness or to children with serious emotional disturbance or at risk for same. Experience as a case manager is preferred, but not required; OR Bachelor's degree in nursing and 3 years of direct mental health care experience. An overview of the responsibilities of the Acute TCM position include: Responsible for assessing client functioning and developing comprehensive Targeted Case Management (TCM) plans for individuals transitioning from psychiatric facilities. Coordinates discharge planning with hospital liaisons, links clients to housing, benefits, and treatment services, and ensures intensive monitoring during the first 90 days post-discharge. Oversees all services received by clients, ensures needs are met through natural and community supports, attends interdisciplinary meetings, and maintains a current directory of resources. Participates in a rotating 24-hour crisis response system, provides immediate support, and ensures all alternatives to hospitalization are explored. Acts as a strong advocate for clients, removing barriers, resolving issues, and ensuring access to appropriate services and resources. Maintains accurate records, ensures regulatory compliance, prepares for audits, and demonstrates strong knowledge of case management principles and community resources. This position requires the employees to perform their daily job duties and responsibilities in an structured work environment. ESSENTIAL DUTIES AND RESPONSIBILITIES Includes the following, (other duties may be assigned as deemed necessary by management): General Office Duties Answer phones and direct calls and emails in a professional manner. Screen calls for appropriate referrals and complete applicable documentation. Monitor WCSI security/visitor camera. Greet and direct visitors to their WCSI contact or appointment location. Implement WCSI's visitor protocol, signing in/out all visitors, formal tracking of visitors, and administer visitor badges. Process incoming and outgoing mail daily. Assist in maintaining WCSI's Food Pantry Administrative Duties Electronic Data Filing- Responsible for accurately filing documents in the appropriate section of the client's record within the department's 72-hour standard. Process requests to have client documents distributed via email, mail or fax to clients, family members and providers. Follow established processes for recording outgoing documents. Verify that all documents are delivered to the appropriate recipient and if sent by email, ensure that documents are transmitted via encrypted email to maintain HIPAA compliance. Provide coverage for all Clerical positions as needed and assigned by Clerical Supervisor Prioritize work assignments to meet deadlines. Additional Responsibilities Distribute office supplies and other necessary items to Program staff as requested. Prepare intake packets and assist with additional projects as assigned. Assist with preparing for agency/program events. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. SPECIFIC DUTIES: 1.Provide supervision of residents at all times, unless the resident is using independence time. 2.Maintain a supportive, therapeutic relationship with each resident. 3.Be aware of each resident's service plan, history, medications, current needs and any special accommodation/s they require. 4.Actively participate in residents' program planning process and support residents' independence in choosing goals. 5.Always act as a role model, maintaining appropriate boundaries and demonstrating effective communication skills. 6.Support individuals by providing assistance and training, at varying levels based on need, in all areas of daily living, including but not limited to the following: Hygiene, grooming, toileting and bathing Care of clothing, laundry Cooking, nutrition, dietary needs, meal preparation Leisure activity, recreation and social skills building 7.Maintain clear and consistent expectations of each resident while maintaining a safe, supportive environment. 8.Implement, on a consistent basis, direct service interventions as defined in the individual's service and psychiatric rehabilitation plans. 9.Oversee and actively assist in the completion of household chores ensuring the home is always neat and clean. 10.Maintain open and ongoing collaboration and communication with all team members including other RTAs, supervisors, therapists, psychiatrist, nurses and others working on the team. 11.Maintain ongoing communication with each resident's family/significant others utilizing diplomacy and tact and creating a welcoming environment. 12.Ensure any family concerns are shared immediately with a supervisor. 13.Participate in household shopping, as applicable. 14.Coordinate, facilitate and engage in regular social activities both on and off campus with the residents. 15.Transport and accompany residents to appointments and activities, as assigned. 16.Provide program/shift coverage as assigned. 17.Upon successful completion of training, monitor residents taking their prescribed medications. 18.Provide crisis intervention in a calm therapeutic manner when indicated. 19.Upon successful completion of training and in accordance with PATH policies and procedures, implement approved manual restrictive procedures only if necessary to prevent a resident from imminent harm to themself or others. 20.Follow the ‘When to Call a Supervisor' protocol which includes immediately contacting a supervisor, or the on-call supervisor, regarding any incidents, issues or concerns. 21.Participate in scheduled meetings, including stakeholder's meetings, case conferences and community meetings, as well as staff meetings and supervision. 22.As assigned, schedule necessary medical and dental appointments for residents. 23.Assist residents in maintaining overall health by: becoming knowledgeable about their health histories, accompanying them to appointments and emergency services as needed, and encouraging individuals to follow healthy living choices and wellness recommendations from their doctors. 24.Collaborate with jobs, schools or other programs residents attend. 25.Ensure all required documentation is completed in timely, comprehensive and legible manner and in accordance with PATH policies and procedures. This includes, prior to the end of shift, ensuring shift notes are completed and document residents' progress towards their goals, as well as completing incident reports for any non-routine incidents, including noting when supervisor was informed. PHYSICAL DEMANDS: Ability to perform crisis management interventions including restraints once training is successfully completed. Ability to walk distances during psych rehab activities and other community outings with residents. The program operates 365 days/year, 24 hours/day. QUALIFICATIONS: Bachelor's or Master's Degree from an accredited college or university in a human services field, i.e., social work, counseling, psychology, criminal justice 1 year experience in a Behavioral Health system Ability to use an Electronic Health Record Good interpersonal and communication skills Active Driver's License and vehicle required SPECIFIC DUTIES: Provide crisis assessment, intervention, education, and support in person, with assigned team member, within a designated time frame to referrals received by DBH/CBH. Utilize approved evidence-based practices and tools when providing crisis assessment and intervention, including SAMHSA crisis intervention guidelines and resolution models. Assure all interventions are person-centered and driven by the individual and their family/supports. Obtain from the family/supports, if release of information allows, an understanding of the events that led to the crisis from everyone's perspective. If the individual is currently receiving services, collaborate with other treatment providers, if release of information allows. Assist the individual in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, and education through linkage to resources and additional supports. To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed, and the plan has been developed. Assure appointments are scheduled at a time and place convenient for the individual. To complete or assist others in completing involuntary commitment petitions. To arrange and follow through with voluntary or involuntary hospitalization admissions. To follow all procedures regarding the initiation of a commitment. Link the individual to the CRC or other 24/7 crisis staff and remain with them until the connection and collaboration is completed. Develop with the individual and their family/supports, if available, a safety/ crisis plan obtaining everyone's perspective, strengths and ideas. Be knowledgeable of local community resources and services and provide care coordination. Provide up to 72 hours of intervention by scheduling follow up appointments at a time convenient for the individual, which includes evening and weekend appointments in the home and community.
