CORE RESPONSIBILITIES
Examine and thoroughly document oral conditions, including those of the head and neck region, through the use of clerical measurement, radiographs and other assessment methods
Perform thorough removal of hard and soft deposit with ultrasonic/hand instrumentation and polishing of supra gingival crowns
Review and record patient medical histories
Develop individualized oral hygiene programs for home care
Educate patients, the general public and special population groups particularly the children regarding oral health
Maintain inventory of dental hygiene supplies
Assist other dental staff when necessary
Requirements
MINIMUM QUALIFICATIONS
High school diploma or GED
Graduate of an accredited Dental Hygiene Program
Current Delaware dental hygiene license - Dental hygienists can apply for license by reciprocity if they hold a current Dental Hygienist license in another jurisdiction and have practiced as a Hygienist at least three of the past five years.

RESPONSIBILITIES:
Checks in patients (in person or via Curbside Check-In) in a timely and friendly manner

Responsibilities of Nurse Supervisor
Engage in outreach initiatives in collaboration with the Rural Heath Outreach Program Manager as directed by the Associate Director of Clinical Operations and Director of External Affairs.
Maintain the Mobile Health Unit (MHU) in accordance with Joint Commission standards, including stocking and maintenance of diagnostic equipment, supplies and medications needed for patient care. Perform and document daily completion of QC logs and maintain POCT before beginning patient care.
Assist providers and medical assistants with patient care, e.g., take vital signs, perform EKGs, collect specimens, irrigate ears, dress wounds, splint and tape injuries, perform selected diagnostic tests, and assist with pap smears, physical exams, and minor procedures when necessary.
Oversee clinical care on the MHU and out in the community to ensure culturally competent, high quality, evidence-based care in accordance with Westside's standards, policies, and procedures.
Work with the Nurse Manager and Clinical Floor Supervisor to complete the weekly staffing schedule, clinical services delivered in the community, monitor attendance and adequately communicate all last-minute schedule changes to all affected personnel.
Proactively and continuously reviews patient medical records to identify any care needs requiring intervention.
Educates patient/family about medical conditions (existing or newly diagnosed) to assist them in appropriate self-management.
Attend community meetings and participate in community events/fairs as directed
Maintains compliance with all professional and organization requirements to meet the positions standards
Lead team outings and outreach during nurse led health screening events
Support Westside's Dover Health Center when not on the Mobile Health Unit

RESPONSIBILITIES:
Room patients, this includes but not limited to: escort patients to examination rooms, prepare them for the provider by completing medication reconciliation, obtaining and recording in the electronic medical record system the patient's medical history, chief complaint, assess vitals, complete care gaps and any additional care as needed or requested.

RESPONSIBILITIES
Protect the security of medical records to ensure that confidentiality is maintained in accordance with HIPAA regulations.
Collect, sort, and distribute paper mail within the office.
Evaluate and process incoming documents for provider review and scan into the electronic medical records (EMR) system in accordance with Westside Family Healthcare HIM Department policies.

Core Responsibilities of the Director of Finance and Accounting
Supervise the Accounting and Finance team
Develop and plan department objectives with the CFO and staff including short- and long-range accounting programs in budgeting, profit analysis, accounting systems, financial reporting, and internal auditing

Core Responsibilities
The Clinical Floor Supervisor is responsible for oversight of the Medical Assistant team with responsibilities including:
Direct the daily activities of the Medical Assistant staff, including the assignment and completion of tasks, quality oversight and readiness, and preparation. Ensure work is completed efficiently and in accordance with Westside's standards, policies, and procedures. Delegate tasks to MAs as appropriate.
Resolve coverage issues related to scheduled and unscheduled absences.
Provide support to site MA team, including supporting orientation and ongoing training when needed, and continuously practicing, enforcing, and educating the MA team regarding workflows and workflow changes resulting from the addition or revision of policies and processes.
Work in conjunction with the Nurse Manager to maintain vaccine and supply inventory and serve as a main contact for Vaccines for Children/317 programs, except where Lead CFS is present and responsible.
Participate in quality improvement activities and initiatives with a focus on sustainability.
Ensure daily point-of-care testing is completed per manufacturers recommendations.
Oversee the testing of all assigned equipment to ensure equipment is working properly and notify the Clinical Floor Supervisor immediately when equipment is not functional.
Complete MA timesheets, performance evaluations, performance improvement plans and formal corrective actions documentation as needed, except where Lead CFS is present and responsible.
Facilitate the timely completion of MA competencies for the site MAs and address any identified staff deficiencies, except where Lead CFS is present and responsible.
Participate in the recruiting, interviewing, orientation, and training of new MA team members.
Act as the liaison with the Lead Clinical Floor Supervisor/Nurse Supervisor/Manager for issues that arise with the MAs.

RESPONSIBILITIES OF NURSE MANAGERS

RESPONSIBILITIES OF NURSES
Manage a caseload of patients identified as at risk for health deterioration, sentinel events and/or poor outcomes using the nursing process and established protocols/workflows. Ongoing evaluation of care coordination efforts by measuring interventions effectiveness and fostering a continuous improvement mentality. Partners with the patient and family, the primary care provider, and other team members to implement an interdisciplinary plan of care.
Participates in daily huddles with the Clinical Operations team, and other team meetings/trainings as scheduled

Works with patients and families to ensure both medical and psychosocial needs are met in order to promote health and well-being and to identify any barriers that inhibit the patient from adhering to the prescribed plan of care
Assesses and documents progress towards goals based on provider's plan of care; utilizing clinical judgment, established protocols, review of patients' self-monitory tools and trends in clinical data (i.e., HbA1c, LDL, BP)
Promotes timely access to appropriate care, increasing the utilization of preventative care and healthy behaviors to improve the health of the population at risk
Provides ambulatory care support as a member of the care team, which may include, but is not limited to walk-in and/or telephone triage utilizing established protocols

Core Responsibilities
The Clinical Floor Supervisor is responsible for oversight of the Medical Assistant team with responsibilities including:
Direct the daily activities of the Medical Assistant staff, including the assignment and completion of tasks, quality oversight and readiness, and preparation. Ensure work is completed efficiently and in accordance with Westside's standards, policies, and procedures. Delegate tasks to MAs as appropriate.
Resolve coverage issues related to scheduled and unscheduled absences.
Provide support to site MA team, including supporting orientation and ongoing training when needed, and continuously practicing, enforcing, and educating the MA team regarding workflows and workflow changes resulting from the addition or revision of policies and processes.
Work in conjunction with the Nurse Manager to maintain vaccine and supply inventory and serve as a main contact for Vaccines for Children/317 programs, except where Lead CFS is present and responsible.
Participate in quality improvement activities and initiatives with a focus on sustainability.
Ensure daily point-of-care testing is completed per manufacturers recommendations.
Oversee the testing of all assigned equipment to ensure equipment is working properly and notify the Clinical Floor Supervisor immediately when equipment is not functional.
Complete MA timesheets, performance evaluations, performance improvement plans and formal corrective actions documentation as needed, except where Lead CFS is present and responsible.
Facilitate the timely completion of MA competencies for the site MAs and address any identified staff deficiencies, except where Lead CFS is present and responsible.
Participate in the recruiting, interviewing, orientation, and training of new MA team members.
Act as the liaison with the Lead Clinical Floor Supervisor/Nurse Supervisor/Manager for issues that arise with the MAs.
In addition to the Clinical Floor Supervisor responsibilities, the Clinical Floor Supervisor will also perform the duties and responsibilities of the Medical Assistant as defined in the Medical Assistant job description.
Maintain Medical Assistant certification and CPR/BLS certification.
