How you'll contribute
The Physical Therapist (PT) assumes responsibility and accountability for a designated group of patients and provides physical therapy evaluations, modalities, and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care. Assists patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.
Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
Observes and evaluates treatment affect, accomplishment of short- and long-term goals. Recommends change to physician, if needed.
Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
Formulates and updates patient specific plan of care for physical therapy
Reports patient progress and barriers to discharge in each patient team conference
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate. Performs appropriate documentation.
Demonstrates knowledge, competency and proficiency of physical therapy modalities.
Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families.
Manages and operates equipment safely and correctly. Maintains department cleanliness and safety.
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
Communicates appropriately and clearly to physicians, staff, and administrative team.
Participates in patient/family conferences and team conferences.
Demonstrates an ability to be flexible, organized, and to function under stressful situations.
Maintains a good working relationship both within the department and with other departments.
Consults other departments, as appropriate, to collaborate in patient care and performance improvement activities.
Meets current documentation standards and policies. Documents goals on the Interdisciplinary Care Plan on admission of the patient and provide updates regularly.
Participates in educational staff performance improvement in-services. Assists with orientation and in-service training for certified department staff members. Participates in guidance and educational programs.
Ensures that patient charges are accurate and entered on a timely basis.
Participate in infection control, departmental equipment training, and organizational safety and fire safety programs.

*BAYADA Home Health Care* is currently seeking a *Part Time* *Admissions Coordinator* to join our *Hospice Triage Services* team for *Weekend Coverage.* The Admission Coordinator (AC) is responsible for receiving and coordinating hospice referrals from physicians, hospitals, facilities, and other community resources. The AC coordinates all hospice referrals and is the lead coordinator of all client referral/admissions activity. This role collaborates with different departments to obtain necessary billing and clinical preadmission information and build effective working relationships within the work group to benefit our clients and referral sources. The position works with and supports the families and clients directly at the beginning of their hospice experience, which may include explaining benefits, coordinating care, and sharing knowledge about the services provided. The AC will positively impact the experiences of our clients and families, as well as increase our response time to referral sources, all while creating a team environment between sales and operations.
*MINIMUM QUALIFICATIONS:*
* Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability.
* Four (4) year college degree.
* Minimum two (2) years of experience and/or training with hospice. Experience in a medical office environment, customer service role, and intake or inside sales experience are preferred.
* Licensed Practical Nurse (LPN) certification is preferred but not required.

* Process client referrals, coordinate client admissions, collaborate closely with hospice branch operations and sales team, and enter client admission data after all hospice requirements have been met and documented.
* Collaborate with different BAYADA functional offices to obtain necessary billing information, clinical information, referral source/account preferences, and build effective working relationships within the work group for the benefit of our clients.
* Perform an initial screening/verification of benefits according to Medicare, Medicaid, and third-party payor guidelines, coordinate equipment with contracted vendors, and gather supporting information and/or documents pertinent to client admission to the program.
* Ensure that operations remain in accordance with BAYADA policies and procedures, and applicable local, state, and federal regulations.
* Accept all new referral calls and monitor all referral fax lines and portals.
* Contact the client and family, liaison and/or Program Manager, referral source, and branch staff within one (1) hour of the referral.
* Enter all information gathered accurately into HCHB.
* Contact the insurance company for eligibility and authorization. Obtain sufficient client and insurance information to confirm the hospice service line.
* Perform an initial screening to determine suitability for admission to Hospice care according to Medicare, Medicaid, and third-party payer guidelines.
* Maintain discretion, as the position could be exposed to highly confidential information.
* Explain benefits to families and clients directly at the beginning of their hospice experience, working with and supporting them.
* Coordinate care and share knowledge about the services provided and hospice philosophy with the clients and their families.
* Positively impact the experiences of our clients and families, increase our response time to referral sources, and create a team environment between sales and operations.
* Confirm the benefit period and explain hospice services, benefits, and philosophy to clients, family members, and significant others.
* Ensure maximum reimbursement through initial payer source, benefits, eligibility verification, and authorization processes.
* Provide accurate insurance and admission information to admission staff to ensure appropriate disclosure of rights and responsibilities, financial liabilities, and benefit plan details to clients and their families.
* Completes required payer source verification forms.
* Schedule an appointment with the client and family for a hospice informational meeting.
* Secures a hospice order for evaluation/admission. Confirm and obtain Do Not Resuscitate.
* Coordinate hospice durable medical equipment/ comfort kits in place upon client arrival and with hospital staff upon discharge to ensure a smooth transition for the client when arriving home.
* Confirm and audit client charts in HCHB, ensuring all necessary documents are signed, dated, and uploaded to HCHB.
* Participate as a member of the marketing team by following up on pending referrals, developing and maintaining positive relationships with referral sources, and assisting in developing strategies and plans for site and company growth.
* Provide the Billing Coordinator, or designee, with insurance and billing documentation, client location, and client level of care as needed for final confirmation prior to admission.
* Maintain communication with pending clients, families, and referral sources and routinely follow the progress of any non-admissions.
* Participate in the service office morning stand-up calls.
* Ensure operations remain in accordance with BAYADA policies and procedures, and applicable local, state, and federal regulations.
* Attend in-services and continuing education programs when appropriate and assigned. Remain current on innovations, developments, new procedures, and techniques as they may pertain to referral information.
* Promote BAYADA's philosophy, mission statement, and administrative policies to ensure quality of care.
* Maintain client and staff privacy and confidentiality pursuant to the HIPAA Privacy Final Rule.

The TMC builds a book of business through deposit growth, maintaining and cross-selling existing clients, and prospecting new-to-bank relationships.
The TMC on the Government Banking Treasury Management Sales team will work to identify ways to improve the client experience and enhance Capital One's brand in the marketplace. Seen as a trusted advisor, the TMC works closely with government clients to understand their business, optimize Treasury work flows, and minimize financial and reputational risk. They focus on creating and delivering customized client centric solutions through a consultative approach.
This TMC will focus exclusively on Government Banking clients that includes State and Local Municipalities, Public School Districts and Public Universities. Familiarity with the public sector vertical and Government clients' specific banking and financial operations is desirable. This role functions as both the Treasury Management Consultant and Relationship Manager for client relationships and works to generate new-to-bank relationships, while developing existing clients by delivering value-added solutions and working across the product and technology organization to optimize products for evolving client needs.
Leveraging their network and collaborating across business lines, the TMC influences strategy to meet the needs of clients while using sound judgement in assessing risk. The TMC is also responsible for developing marketing and portfolio relationship plans specific to their market to drive profitable portfolio growth and net new revenue generation. They will act as the lead for a team of functional partners who will help support Treasury Management, deposit sales processes, and build ongoing relationships.
The TMC values and fosters a spirit of teamwork and cooperation across value streams. This position works independently and uses initiative in carrying out client analysis and proposals requiring minimal guidance from the sales leader.

Independently develops and maintains relationships with clients to maintain and expand existing business
Understands treasury management product risk and ensures Capital One credit policies and other risks (e.g., reputational, legal) are adhered to and mitigated
Establishes appropriate expectations for onboarding including execution of documentation and ensure ramp of sold services
Conducts independent entities to generate new, high-quality and profitable Treasury Management and deposit business to achieve or exceed specific YoY growth goals
Takes a role in market by actively engaging in local industry associations, conferences, networking groups and community associations
Identifies working capital efficiency opportunities for an entity and deepens relationship accordingly; ensures awareness of market and industry trends; identifies dissatisfiers and escalates appropriately
Identifies and refers ancillary business opportunities to appropriate partners
Designs customer-centric treasury management solutions, and provides consultative expertise on the liquidity management cycle
Identifies both internal and external prospects. Develops and maintains relationships with prospects to profitably expand business
Provides thought leadership within and outside the market through, but not limited to, attending and speaking at industry conferences, etc.
Engages with internal marketing in the design of sales collateral and company-sponsored marketing
