Help seniors remain healthy and independent at home with your reliable and friendly help! As a Home Support Provider, you would help people stay healthy and independent at home by doing housekeeping and household chores, simple meal prep, laundry assistance, and run errands.
***This position has a $1,000 Retention Bonus for part-time schedules, paid during the first year of employment in quarterly installments***
Part-time (20+ hours weekly) position available with flexible hours
Weekday, daytime hours - between 8 a.m. and 4:30 p.m.

Requirements
Able to lift/carry household items & groceries, ascend and descend flights of stairs, and other tasks requiring similar levels of physical stamina
A minimum of one year of experience in a home health setting and/or caregiving experience
Valid driver's license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and state minimum liability insurance
Ability to effectively use agency device to track work time, mileage, and communicate with agency staff

What You'll Do:
Handle incoming and outgoing non-professional level phone inquiries and transfer calls to appropriate staff.
Update patient demographic information in the Electronic Medical Record (EMR).
Respond to inquiries and provide information such as business hours, delivery services, price information, prescription status, and/or refill availability.
Ensure that all newly enrolled mail-order patients have been completely and successfully signed up.
Actively promotes and educates patients on all pharmacy services, including the mail order process and auto-refill.
Complete third-party rebilling process, including contacting insurance companies for authorization.
May perform Lead duties by acting as a liaison between staff and the direct supervisor. Manages breaks, lunches, sick calls and overtime. Clinical Psychologist: $48.08/hour

What You'll Do:
Respond to requests from primary care providers to address behavioral health needs of medical clinic patients to maintain a warm hand-off for most patient encounters.
Provide brief intervention for presenting problems and triage for ongoing treatment services as needed. Completes encounters and associated documentation, including coding and billing.
Present case studies and reports to funding sources or as requested by outside organizations.
Measure change and adjust treatment as needed, using appropriate screening tools.
Provide cognitive behavioral therapy to individuals, families, groups, or communities as assigned. Integrate cognitive behavioral interventions in multiple settings.
Enter documentation into the electronic medical record (EMR).
Respond to crisis situations, assess the urgency of patient's needs, provide care, and obtain appropriate services as needed.
Assist patients and their families in obtaining additional services and other resources as needed. Documents all referrals made to other resources.

As a dietitian, you will have the opportunity to:
Conducts nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions.
Utilizes motivational interview to identify desired goals and outcomes of nutrition care in collaboration with patient/client and health care professionals. Considers the variables that may influence the patient/client's food related behaviors including culture and social determinants of health.
Evaluates and provides evidence based nutrition recommendations to address patient/client medical needs. What You'll Do:
Plans and manages the daily functions of the clinic front office.
Formulates and implements policies, procedures, and processes that meet the standards and requirements of the organization and various regulatory agencies.
Manages provider schedules to ensure correct appointment type is assigned, schedule is full, and daily targets are met. Verifies and adjusts schedule when providers are not available.
Maintains and books appointments for the Credit Manager and Behavioral Health Consultant.

Manages AIDET patient communication surveys, compiles and communicates results, and makes any process or procedure changes necessary to improve results. Also manages TSF and Arcadia Tracking on a daily basis.
Manages the processing of all Release of Information requests. Coordinates with Risk Management regarding the healthcare information to be provided prior to the release.
Coaches, mentors and trains direct report staff. Assesses competencies of staff, provides continuous performance management and conducts employee performance evaluations. Addresses performance and/or behavior issues, clarifying expectations and providing feedback.
Participates in the recruitment and selection of staff. Provides orientation to outline job requirements and expectations, policies and procedures, and proper use of tools and equipment.

Qualifications:

This position will provide coverage for pharmacies in the Yakima Valley at various locations from Yakima to Kennewick.
What You'll Do:
Enter patient data in the system including patient name, insurance, allergies, doctor and diagnosis. Enter the prescription in the system, selecting the brand, product/drug, and appropriate strength to align with the instructions given by the prescribing Provider.
Fill or dispense the product from the pharmacy stock and delivers to patient.
Process prescription refills verifying refill availability.
Maintain awareness of stock level and enters order for restocking based on defined minimum inventory levels. Verify quantity and submits order to the Wholesaler.
Verify product orders received with invoice and purchase order. Place a sticker on each item and places on the shelf.

What You'll Do:
Plans and manages the daily functions of the clinic front office.
Formulates and implements policies, procedures, and processes that meet the standards and requirements of the organization and various regulatory agencies.
Manages provider schedules to ensure correct appointment type is assigned, schedule is full, and daily targets are met. Verifies and adjusts schedule when providers are not available.

Coaches, mentors and trains direct report staff. Assesses competencies of staff, provides continuous performance management and conducts employee performance evaluations. Addresses performance and/or behavior issues, clarifying expectations and providing feedback.
Participates in the recruitment and selection of staff. Provides orientation to outline job requirements and expectations, policies and procedures, and proper use of tools and equipment.

What We Offer
$23.94-$29.32 DOE with the ability to go higher for highly experienced candidates
Essential Functions/Responsibilities/Duties:
Enters patient data in the system including patient name, insurance, allergies, doctor and diagnosis. Enters the prescription in the system, selecting the brand, product/drug, and appropriate strength to align with the instructions given by the prescribing Provider.
Fills or dispenses the product from the pharmacy stock and delivers to patient.
Processes prescription refills verifying refill availability.
Maintains awareness of stock level and enters order for restocking based on defined minimum inventory levels. Verifies quantity and submits order to the Wholesaler.
Verifies product orders received with invoice and purchase order. Places a sticker on each item and places on the shelf.
Monitors and checks expiration dates on drugs in stock and prepares expired stock for return to Wholesaler. Processes credit invoices associated with the returns.
Returns or discards dispensed prescription to stock if not picked up by patient in 30 days. Enters information in the system for reverse billing.
Completes third party billing form and contacts Provider if prior authorization is required.
Performs other duties as assigned.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA) and YVFWC requirements.
Qualifications:
High School Diploma or General Education Diploma (GED).
OR Pharmacy Technician license; Alternatively, may have an active Pharmacy Intern License
Effective verbal and listening communication skills with a customer-oriented approach.
Knowledge of medical terminology.
Ability to perform administrative tasks such as answering phones, filing, faxing, scanning and copying.
Ability to work as part of a team in a fast-paced environment.
Ability to organize work while performing multiple tasks requiring attention to detail.
Basic proficiency with a variety of computer systems including Microsoft Office preferred.
Basic knowledge of a minimum of one pharmacy software system and ability to learn new systems.
This job will require a pre-employment drug screen, which would take place after extending a conditional offer of employment.

The Behavioral Health Consultant provides primary care-based behavioral health services to clients with complex or chronic needs depending on the assigned program as determined in the treatment or care plan of the client. Counseling or therapeutic services are provided as part of a team, with primary care-based services being integrated into the care plan directed by the primary care provider. We offer this position at either PsyD/PhD clinical psychologist, or independently licensed Master's-level mental health therapist.
This is a part-time, 20-hour/week position.

Essential Functions/Responsibilities/Duties
Responds to requests from primary care providers to address behavioral health needs of medical clinic patients to maintain a warm hand-off for most patient encounters.
Provides brief intervention for presenting problems and triage for ongoing treatment services as needed.
Completes encounters and associated documentation, including coding and billing.
Presents case studies and reports to funding sources or as requested by outside organizations.
Measures change and adjusts treatment as needed, using appropriate screening tools.
Provides cognitive behavioral therapy to individuals, families, groups, or communities as assigned. Integrates cognitive behavioral interventions in multiple settings.
Enters documentation into the electronic medical record (EMR).
Responds to crisis situations, assessing urgency of patient's needs, provides care, and obtains appropriate services as needed.
Assists patient and family in obtaining additional services and other resources as needed. Documents all referrals made to other resources.
As assigned, participates in various community meetings and serves as a liaison to numerous community agencies and organizations, including school districts, hospitals, law enforcement, Division of Children and Family Services (DCFS) staff, and tribal authorities.
Presents mental health/behavioral health findings and recommendations in verbal or written format to appropriate professionals either upon request or as part of a regular case presentation for peer review or clinical meetings.
Leads staff training sessions on new therapeutic techniques and practices. Provides training and education on clinical topics to team members, staff, and the community, as needed.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications
Clinical Psychologist Requirements
Education: Doctor of Philosophy (Ph.D.) or Doctor of Psychology (Psy.D.) in Clinical Psychology.
Experience Preferred: One year's clinical experience working within a multidisciplinary team in primary care or other medical settings providing diagnostic and psychological assessments for primary care-based positions, or one year's clinical experience working with children, adolescents, and families for specialty behavioral health settings.
Licenses/Registration: Licensed Psychologist within the state of practice. First Aid/Cardiopulmonary Resuscitation (CPR) Certification obtained within 45 days of employment.
Master's Level Therapist Requirements
Education: Master's Degree in Social Work, Psychology, Counseling, or related field.
Experience: Two years experience postgraduate/master's experience in the direct treatment of persons with mental illness or emotional disturbance; such experience must have been gained under the supervision of a mental health professional. Washington State designation as a Mental Health Professional (MHP) may be substituted for this experience requirement. One year's full-time experience in the treatment of children under the supervision of a children's mental health specialist. Washington State designation as a Child Mental Health Specialist may be substituted for this experience requirement.
Licenses/Certificates/Registration: Current Cardiopulmonary Resuscitation (CPR)/First Aid certification (or obtained within 45 days of employment). One of the following licenses is required for this position (must apply for within one week of hire if the license is not current):
Licensed Advanced Social Worker (LASW)
Licensed Independent Clinical Social Worker (LICSW)

Home Healthcare and Hospice Community Services Job In Keene, NH
HCS is seeking experienced Home Health and Hospice Aide / LNA to make home visits. Provide personal care and assistance with ADLs as well as light housekeeping essential to the patient's health and social well-being. Activities shall be under the supervision of the appropriate professional therapist when the Aide carries out simple procedures as an extension of physical, speech or occupational therapy. Full-time opportunity working 30 hours per week with benefits.

Requirements
Successful completion of approved LNA training which fulfills Medicare and State regulation
Minimum of one year experience of active employment as LNA with long term care, hospital or homecare settings;
Current registration by the New Hampshire Board of Nursing and evidence of twelve (12) hours of nursing related in-service education annually;
Current Basic Life Support (BLS) certification;
Must have successfully completed the LNA competency test with a score of 80% or better; and
