We're looking for a compassionate and skilled Afterhours Registered Nurse (RN) to join our Advanced Illness Management (AIM) team in Canonsburg, PA. In this vital palliative care role, you'll provide after hours clinical support to patients and families facing serious illness-through phone triage, in-person visits, or virtual interventions.
This is an On Call After Hours position - 7 days on/7 days off.
5pm-8am during the week, and 5pm Friday through 8am Monday.

Key Responsibilities
Triage and respond to urgent calls from palliative care patients and families after hours
Conduct initial nursing evaluations to determine eligibility for palliative care services
Provide clinical support via phone, video, or in-person visits as needed
Collaborate with physicians, nurse practitioners, and the interdisciplinary team (IDT)
Assess and manage patients' physical, emotional, psychosocial, and spiritual needs
Document care accurately and timely in the electronic medical record (EMR)
Communicate changes in condition to the appropriate care team, including PCPs and supervisors
Assist with medication, DME, and supply needs in accordance with care plans
Ensure continuity of care and contribute to advanced illness care planning
Auditing patient charts as needed
About You:
Registered Nurse / RN Requirements:
Education:
Associate or bachelor's degree in Nursing
Experience:
2 years of prior skilled Registered Nurse experience
Demonstrated ability to provide quality patient care
Nursing experience in palliative care, hospice, home health, oncology, or geriatric care preferred
Prior experience in triage nursing, emergency care, or crisis response is a plus
Additional, preferred experience to help you be successful role: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing
Licenses, Certifications and/or Registration:
Active and unrestrictive RN license to practice in the state without any board actions
CPR Certification

Duties and Responsibilities:

See patients right away as part of the VEG Triage and VEG Spikes, our unique, customer-centered way of doing things-like ensuring people see a doctor right away and allowing families to stay with their pets
Gather the signalment, presenting complaint, vitals, and physical exam
Conduct physical examinations of animals, along with other required investigatory measures such as ultrasounds, X-rays, bloodwork, and biopsies
Complete a diagnosis of each pet's condition to create a diagnostic and therapeutic plan
Communicate diagnosis and treatment options directly with customers
Prepare estimates and present them to customers
Create and review medical records to ensure accuracy
