ESSENTIAL JOB FUNCTIONS Implements landscape designs in residential and public areas Prepares soil for planting grass, flowers, shrubs, and trees Installs sod, spreads seed, and re-plants a variety of vegetation Mixes compost and applies fertilizer as needed to support plant health Waters plants, flowers, and other greenery to ensure proper growth Mows lawns, trims shrubs, prunes trees, and removes dead or damaged plants Clears roads and pathways of weeds, sand, debris, and trash Collects and properly disposes of litter and refuse as needed Maintains and repairs irrigation systems, including pipes and valves Digs and lines ditches, trenches, and culverts for proper drainage Installs decorative and functional materials such as stone, brick, or rock Maintains a clean and organized work area; safely disposes of fertilizer and other materials Ensures all work is performed in accordance with safety standards and best practices Participates in disaster preparedness and emergency response, including fire drills and snow removal Performs additional duties as assigned QUALIFICATIONS Minimum of one (1) year of experience in landscaping and the operation of landscaping equipment; experience with snow removal equipment, preferred Proficient in reading, writing, and following both verbal and written grounds maintenance processes and instructions Possession of a valid driver's license issued by the Commonwealth of Virginia. Client clinical record documentation is done per agency policy. * Communicates with other team members regarding the Plan of Care and continues to work with team members for the duration of the patient's case. * Effectively communicated with referring physician, either verbal or written, and provides accurate treatment notes and other necessary documentation. * Documents information regarding interpretation of Hospice care policy, agency philosophy, objectives, and long-range plans. * Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician. * On-going observational recording/reporting of changes in patients condition as soon as possible to the patient's physician, and other team members, * Makes recommendations for involving other services, as deemed necessary. * Advises and consults with the patient's family in the patient's therapy program. * Attend staff meetings at least 4 times/year. * Documents all communication with other personnel to contribute to patient's case conference notes. * Uses equipment and supplies effectively and efficiently and request supplies needed for patient care. * Maintains confidentiality of patient information as well as co-worker's personal information if applicable at all times. * Implements infection control and safety measures as appropriate. * Consistently addresses patients concerns. Promotes education for patients, family members and caregivers. * Demonstrates current knowledge regarding professional career. * Attends mandatory in-services and maintains 100% compliance with mandatory data-based training. * Participates in orientation of Hospice/Home Health care staff members. * Immediately reports abuse/suspected abuse to supervisor, and DHHS or APS. Job Duties * Provide comprehensive Physical Therapy evaluations and treatments, under referral from the attending physician, to overcome specific losses of function, impaired safety, and pain. * Open cases, completing Oasis assessment, if on intake the patient is assessed as having no nursing needs. * Evaluate the home environment and make appropriate recommendations. * Consult with DON and nursing staff, after Oasis assessment and Physical Therapy evaluation, on needs for additional services, such as nursing, home health aide, occupational therapy, or speech therapy. * Effectively communicate with the referring physician and nursing staff concerning changes in the patient's condition, and necessary changes to the initial Plan of Care due to such changes. * Coordinate treatment with nursing staff, home health aides, occupational therapists, and speech therapists, as necessary. * Document evaluations, treatment sessions and services provided in the home according to agency protocols and Medicare guidelines, completing all documentation in a timely and effective manner for both communication with other health professionals and billing purposes. * Effectively involve the patient and caregivers in the development of the Plan of Care, and any changes indicated to the Plan of Care due to a change in condition. * Provide education to patient and caregivers through effective teaching methods in home exercises, safety with functional activities, and carryover of other therapeutic techniques as appropriate. * Provide supervision of Licensed Physical Therapy Assistants (LPTAs) involved in the patient's care according to guidelines for supervision established by the Commonwealth of Virginia state practice act and Medicare guidelines. * Participate in teaching of LPTAs through required joint visits and as needed in patient specific procedures and treatment techniques. * Maintains privacy and confidentiality of patient information at all times. * Implement infection control measures per agency protocols and policies. * Use equipment and supplies effectively and efficiently. * Participate in case conferences and discharge planning per agency protocols. * Complete Oasis recertification and discharge assessments if indicated. * Attend at least 4 staff meetings per year. * Attend agency mandatory in-services and maintains 100% compliance with mandatory data based training. * Participate in implementing systems for evaluating the home health agency's delivery of services to maintain the highest quality of patient care and satisfaction with services possible. QUALIFICATIONS • Graduate of culinary school or recognized culinary apprentice program, preferred • 3+ years of experience in culinary leadership positions; experience with production, senior living, or healthcare culinary environment preparing meals for individuals with varied dietary requirements, preferred • Valid Food Handler's Card required • SERVSAFE Manager Certification required within 30 days of employment. • Knowledge of therapeutic and restricted diets commonly used in Assisted Living settings (diabetic, cardiac, renal, dysphagia, etc.) KNOWLEDGE, SKILLS AND ABILITIES • Demonstrated experience in quantity food production within healthcare, senior living, hospitality, or fine-dining settings. • Knowledge of purchasing, inventory control, receiving, and storage procedures in compliance with regulatory and sanitation standards. • Understanding of nutrition principles, clinical diet guidelines, and menu planning for older adults and individuals with medical conditions. • Ability to guide production of meals that meet texture-modified and therapeutic diet requirements • Strong understanding of sanitation standards, state licensing requirements, health department regulations, and infection control protocols • Proficient computer skills (Word, Excel, Google Workspace); ability to learn food production systems and EHR/menu management platforms • Ability to work cross functionally and collaboratively with staff and other operational and clinical leaders to ensure resident and family satisfaction with culinary services • Excellent organizational and administrative skills and the ability to handle multiple priorities simultaneously in a fast-paced environment • Excellent oral and written communication and interpersonal skills to positively interact with residents and staff • Strong coaching and supervisory skills to help staff continue their professional growth and improve performance • Ability to lift and carry a minimum of 50 pounds • Ability to stand and walk for extended periods during meal preparation and service • Ability to use hands, arms and fingers for food handling, equipment operation, and utensil use; ability to talk and hear in a busy kitchen environment. • Ability to occasionally sit, climb, balance, stoop, kneel, crouch, or crawl. • Ability to work in environments exposed to heat, cold, steam, and cleaning chemicals for extended periods.
