The Nurse Practitioner Inpatient / Physician Assistant provides advanced health care to individuals, families or groups as an advanced or expert clinician. Performs assessment of individual health, analyzes data to determine medical and nursing care needed, plans a treatment strategy to meet identified needs, evaluates responses to nursing and medical interventions.
GENERAL DUTIES
Assessment: Assesses the complex advanced heart failure patients for urgent and chronic conditions. Uses relevant data to evaluate for any physiologic instability and utilizes data to formulate plan of care.
Clinical Interventions: Performs therapeutic interventions to stabilize acute and optimize chronic health problems. Specific interventions are listed in the Scope of Practice and approved as a part of the credentialing/privileging process. Analyzes the indications, contradictions, risk of complications and cost-benefits of therapeutic interventions.
Plan of Care: Manages the plan of care through evaluation, modification and documentation according to the patient's response to therapy. Manages changes in condition with a focus on optimizing the patient outcomes. Incorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute, critical and chronic illness. Facilitates the patients transition between and within health care settings.
Education: Serves as an educational resource to the patient/family, nursing staff and provider team. Acts as a consultant to multidisciplinary health care team members.
Professional Development: Participates in scholarly activities including research and education. Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Acts as a preceptor and mentor to support the development of nurses, APPs, students and other health care providers.

Essential Functions:
The Patient Care Technician works on basic care for adult and geriatric patients' needs including, but not limited to comfort, hygiene, safety, environmental cleanliness under the supervision of professional and practical nursing staff. Provides nursing care of skin/wound conditions involving the identification, assessment, management, and ongoing evaluation of patients with alterations in skin/tissue integrity, that includes but is not limited to draining wounds, pressure ulcers, and vascular ulcers, and conditions resulting from incontinence, surgical procedures, and/or trauma.
Provides patient care based on scientific principles/nursing theory and evidence-based practice.
Provides preventive and therapeutic measures for patients with wound/ostomy needs.
Collaborates with interprofessional healthcare professionals to develop appropriate plans of care for patients with wound/ostomy needs including nutritional and psychosocial needs.
Provides patient education regarding prevention and care of skin breakdown, wound, and/or ostomy management.
Provides education and delegation of wound/ostomy care management and interventions to staff nurses, LPN wound care specialists, and unlicensed assistive personnel adhering to the nursing scope of practice, policies, and procedures in the state of practice including those regarding delegating nursing interventions to qualified nursing personnel.
Uses evidence-based knowledge and research findings to guide practice and develop strategies/interventions to manage care to achieve appropriate goals and outcomes for patients with acute or chronic wounds.
Leads interprofessional teams to communicate, collaborate, and consult effectively; and ensure that safe, effective, efficient, timely, patient-centered, and equitable care is provided for the patient with an acute or chronic wound.
Evaluates processes, policies, procedures, and protocols/guidelines for care of patients with acute or chronic wounds; and recommends revisions when warranted.
Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

Certified Medical Assistant (MA) - American Association of Medical Assistants or

Certified Patient Care or Technician/Assistant (CPCT/A) - National Health careers Association or
Certified Patient Care Technician (PCT) - National Health careers Association
American Heart Association BLS certification.

Join our team as a NursingAssistant on the acute care unit, where you will assist patients with activities of daily living such as bathing, grooming, and feeding. You'll monitor vital signs, including temperature, oxygen saturation, and blood pressure, while accurately documenting patient observations and interactions in the electronic health record.
Your responsibilities also include maintaining a clean and organized patient environment, setting up medical equipment as needed, and providing compassionate support to patients and their families.
