Serves as primary case manager for 8 clients, provides skill training to meet client's assessed needs and to enable clients to live as independently as possible
Acts as client advocate to facilitate access to resources and services and works collaboratively with providers to achieve client goals and to aid clients in various situations.
Develops and maintains client IM+CAN's and completion or coordination of other required forms, and acts as liaison between healthcare resources and clients.

Requirements

POSITION SUMMARY:

An individual in the Care Coordinator position will be responsible for providing care coordination and support services to clients enrolled in the Pathways to Success program. The position involves working collaboratively with youth, their families, and community providers to achieve client goals. This full-time position works Monday through Friday, 8:00 am - 4:30 pm, with availability to work at least two evenings a week, 10:30 am - 7:00 pm. The hiring range for this position is $44,400 to $52,000 depending on experience.
