Active, unrestricted Physical Therapist (PT) license in the state of assignment
Current BLS certification (AHA)1+ year of recent home health experience (within the last 2 years)
Experience conducting PT evaluations and developing therapy plans

Experience with EMRs such as Homecare Homebase, Net
Smart, or Epic

Home Health Physical Therapist Contract Details:
$2,180 - $2,365 per week
12-week contract
Paid by the hour - not by the visit5-6 visits per day (45-90 minutes each, based on complexity)
Travel time factored into productivity expectations
Key Responsibilities:
Perform initial and ongoing PT assessments and develop individualized plans of care
Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
Conduct gait and balance evaluations, home safety assessments, and mobility training
Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
Document all care in compliance with Medicare, OASIS-E, and agency standards

Responsibilities
Our Speech Language Pathologists supervise, evaluate and provide speech therapy services to clients in a home setting as prescribed by the attending physician. The SLP treats patients with speech disorders, swallowing therapy, and hearing impairments that affect communication. In addition they participate in helping patients that may be recovering from a stroke or other neurological disorders.
This part-time salaried position works 24 hours/week, over 4 days in the week, and is eligible for our 60% paid days off program.

Summary
Responsible for the safe delivery and documentation of patient care through the nursing process of assessment// diagnosing// planning// implementation// and evaluation. Performs responsibilities in five areas of nursing practice utilizing critical thinking// management// problem solving and differentiation of urgency in the role of: clinician// teacher// researcher// leadership and professional development. Responsible for directing and coordinating nursing care for patients based on established clinical nursing practice standards subscribed to by American Nursing Association and in accordance with the Florida Nurse Practice Act and other regulatory standards. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patient's age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care. The nurse practice is guided by the code of ethics for nurses with interpretative statements.
Requirements
Education Requirements:Graduate of an accredited school of professional nursing required; Associates or Bachelors preferred.

Certification Requirements:Proof of American Heart Association Healthcare Provider Basic Life Support (BLS), current for a minimum of 90 days after hire date. Certification requirements may vary by department. Please reference the Nursing Addendums for department specific requirements.

The Opportunity
We're seeking a Band Program Lead to help grow a performance-forward band program. You'll collaborate with our Director of Programs and key Program staff, and work directly with talented youth to build an inspiring ensemble. Programming is for 8-13 year olds.
Responsibilities
Lead engaging band rehearsals that build technique, sight-reading, and performance fundamentals.
Co-develop rehearsal plans, repertoire, and performance calendars with the Program Director.
Prepare students for seasonal performances, community events, and showcases.
Maintain attendance, supplies, and learning materials.
Develop a curriculum for each session.
Collaborate on light outreach materials (e.g., flyers, forms, concert programs).
Spring 2026 Schedule
Days/Times: Mondays & Wednesdays, 3:30-6pm (subject to change based on enrollment numbers)
Qualifications
We encourage the application of candidates who have experience with and excitement for instrumental ensembles and performance events such as pep band, jazz/stage band, and marching band.
Required
Experience working with youth (grades 3-8)
Strong band background (stage band ensemble experience preferred)
Prior teaching or directing experience (≥1 year)
Excellent communication, classroom management, and mentorship skills
