General Purpose:
Responsible for the independent supervision of the delivery of care to a group of residents in a skilled nursing facility. Assess resident needs, develop individual care plans, administer nursing care, evaluate nursing care, and supervise Nurses, CNAs and other personnel in the delivery of nursing care.

Adhere to State and Federal rules and regulations concerning delivery of care and assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process.
Effectively interview the resident/responsible party to obtain information required for developing and implementing a plan of care.
Perform physical assessment through observation, inspection, palpation, percussion and auscultation.
Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes.
Identify rationale and anticipated outcomes for each nursing intervention.
Assume responsibility for care interventions consistent with skill level and professional standards of care. Utilize professional and sound judgment in delivery of care, which is cost efficient but does not jeopardize safe, effective care delivery.
Keep physician and/or other health care professionals (nurse practitioner, physician assistant, podiatrist, dentist, etc.) informed of resident's condition; and notify physician and/or other healthcare professionals immediately of significant change of condition.
Work in collaboration with physician and/or other health care professionals by sharing information relevant to changing plan of care.
Critique and take responsibility for evaluating nursing care delivered.
Utilize expertise of nurse colleagues to evaluate plan of care.
Elicit feedback from individual being served and/or family or responsible party to determine that their needs are being met.
Evaluate effectiveness of the plan of care in conjunction with the interdisciplinary care plan team.
Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
Transcribe physician and/or other health care professionals' orders appropriately and accurately.
Notify physician and/or other health care professionals if orders are not carried out and document event appropriately.
Follow company, facility and departmental policies and procedures and appropriately utilize nursing judgment.
Complete documentation of all medical records and reporting forms in an accurate manner.
Supervise CNAs and other personnel.
May supervise other Nurses, including other RNs.
Coach, counsel, and assign CNAs and other nurses to provide quality resident care.
Appropriately discipline or participate in the discipline of CNAs and other nurses for violations of work rules, policies or poor performance, including the recommendation of suspensions or terminations.
Recognize CNAs and other nurses for exceptional care and job performance on a regular basis and as part of their formal performance evaluation.
Assign CNAs and other nurses specific duties for resident care and direct their work.
Train and/or assist in the orientation of new employees.
Demonstrate safe practices in regard to resident's comfort and safety by applying knowledge of proper body alignment for self and resident.
Maintain updated knowledge base in pharmacology.
Demonstrate knowledge of drug reactions and sensitivities and nursing interventions.
Assure that narcotics are accounted for properly.
Recognize normal and abnormal lab values and communicate it in appropriate manner to physician, and other interdisciplinary team members as appropriate.
Attend in-service education programs in order to meet facility educational requirements.
Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill & Evacuation Procedures and know how to use the information.
Maintain confidentiality of resident and facility records/information.
Protect residents from neglect, mistreatment, and abuse.
Protect the personal property of the residents of the facility.
Others as directed by the supervisor or administrator.

Conduct and coordinate the development and completion of the resident assessment process in accordance with the requirements of the Federal and State regulations as well as Company policy and procedure.
Essential Job Functions

Minimum Data Set:
Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
Assemble information from the Initial Nursing Assessment, resident interview, and clinical record review to complete the nursing portion of the Minimum Data Set within 10-14 days of admission or annual review, and when there is a significant change in a resident's condition.
Notify all members of the interdisciplinary team at least one week in advance of the MDS due date for all new admissions, annual reviews, and significant changes in resident condition.
Monitor and follow-up with team members as needed to verify that all assigned sections of the MDS are completed, dated, and signed within designated time frames.
If a member of the interdisciplinary team is absent during the time frame for completion of a MDS, conduct necessary research and referral to confirm that all MDS sections and triggered RAPs are completed.
Review each MDS for accuracy, consistency, completeness, and signatures prior to submitting to the designated RN for final review and signature.
Verify that MDS documentation is placed in resident's medical record and that documentation is complete, including dates, signatures, and sections completed by all members of the interdisciplinary team. Complete, date, and sign MDS quarterly review sheets.
Verify the face validity of all Minimum Data Sets before electronic submission.
Participate in and oversee the timely electronic submission of all MDS.
Review the validation report and verify that appropriate action is taken.
Resident Assessment Protocols (RAP):
Review the Resident Assessment Protocols correlated with nursing issues and answer the questions as identified in the computer documentation system. Once all the questions have been answered, complete narrative summaries of the information, indicating the decision whether or not to include the identified problem on the Plan of Care.
Consult the RAP summary sheet and verify that all triggered RAPs and corresponding narrative summaries have been completed, dated, and signed by the appropriate disciplines.
For triggered RAPs included in the Care Plan, verify that any additional supportive documentation related to RAP issues is completed.
If a triggered RAP is not included in the Care Plan, verify that documentation in the RAP summary clearly indicates reasons for not proceeding.
Care Plans:
Schedule all interdisciplinary care plan meetings, and notify staff in advance which residents will be evaluated.
For Care Plan reviews, notify the resident's family in writing 30 days in advance of care plan meeting (except for care plans requiring immediate revision due to significant change or unforeseen circumstances.)
Identify and document nursing problems, goals, and approaches, and coordinate the development of an individual Plan of Care for each resident in cooperation with the physician, Medical Director, nursing staff, interdisciplinary team, and outside consultants (nursing, dietary, pharmacy, therapists, etc.) in accordance with corporate, state, and federal guidelines.
Correlate the information to update resident care plans quarterly and after each significant change. Verify that all updates are completed. Generate final copy, and verify that signatures from the physician, interdisciplinary team, and contributing resident or family members are obtained.
Make a copy of each resident's care plan accessible to CNAs.
Other Responsibilities:
Disseminate any new or updated materials involving the RAI process.
Create an opportunity for family participation in the care planning process.
Communicate with the Business Office Manager and Administrator on a regular basis regarding the case mix scores and how they impact reimbursement.
Coordinate the interdisciplinary assessment process for all residents of the facility. Verify that the Resident Assessment Instrument is individualized, complete, accurate, and timely for each resident.
Conduct and facilitate the Interdisciplinary Care Plan meetings. Educate peers on MDS, RAPs, and Care Plans.
Attend in-service education programs in order to meet facility educational requirements.
Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill & Evacuation Procedures and know how to use the information.

Prepare and maintain supplies and equipment for treatment.
Assist patients utilizing a Virtual Medicine Provider.
Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis.

Additional Requirements for Centers of Excellence:
Completes all required preceptor training in order to effectively onboard and train new hires in centers of excellence.
Engage with Learning and Development to manage onboarding and training of new employees.
Meet new employees on first day in clinic and facilitate welcome and introductions, facility tours, and clinic process overview.
Partners with Learning and Development and new hires establish a training plan and review training schedule. Communicates with learning and development as appropriate.
Submits weekly evaluations on your trainee's progress to Learning and Development. Communicates training progress or concerns as appropriate to operations leadership and learning and development.
Completes final evaluation and Proof of Proficiency form and submits to learning and development on final training day.
Experience Requirements and Preferences::
At least 1 year of experience as an X-Ray Technologist preferably in clinic setting, preferably Urgent Care.

In Indiana, Limited Scope License requires a State License and proof of ARRT License
