Job Description
Minimum Qualifications:
1. Master's degree in Social Work from an accredited institution.
2. Licensed Independent Clinical Social Worker (LICSW).
3. Five (5) years of related experience.
Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Provides psychosocial assessment of the individual and family, including limitations, deficits and strengths in their current situation and environment, as well as functional level. May conduct a psychiatric social work assessment according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). Develops treatment plans with appropriate and competent interventions.
2. Responsible for psychosocial assessment and intervention in crisis situations that often involve issues of suicide, homicide, family violence, elder or child abuse.
3. Demonstrates sound clinical judgment in assessing the patient's needs for long-term care, including evaluating the home and family situation, helping the patient and family to develop an in-home care plan, exploring alternatives to in-home care and arranging for placement if necessary.
4. Provides treatment, therapy, psycho-education and/or counseling utilizing individual, family or group modalities.
5. Provides information, referral and creative resource acquisition for specific individual and family needs both within the Hospital and in the community as indicated. Assists patients/families to understand psychosocial factors impeding their maximal utilization of hospital and community resources.
6. Collaborates with and provides psychosocial consultation to the health care team to promote a smooth, coordinated plan of care. Aids the team in understanding and integrating the significance of psychosocial factors in relation to patient's illness, treatment and recovery. Identifies psychosocial issues that may impede progress. Makes recommendations to the team regarding patient/family care and management. Assists health care team to assist patient/family level of understanding to make informed decisions.
7. Provide medical social services to the patient's family member or caregiver on a short-term basis when necessary to remove a clear and direct impediment to the effective treatment of the patient's medical condition or rate of recovery.
8. Treats patients and families with dignity and respect at all times. Offers time for questions and answers; demonstrates empathy in communication while working toward optimal health goals.
9. Provides outreach, case finding/screening for high-risk issues that may impact on patient's progress, participation in plan, discharge or ability to utilize resources.
10. Assists with discharge planning issues to ensure continuity of care. Formulates, coordinates and implements the psychosocial components of outpatient care.
11. Consults, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources.
12. Serves as a patient/family advocate with a variety of systems both internally and externally as required. May be called upon to testify in court.
13. Ensures clinical documentation is complete and according to departmental standards. Performs other administrative documentation and record keeping, such as accountability and statistical reports, billing forms, student intern evaluations, performance evaluations, etc. in complete and timely manner.
14. Initiates policy and program development in specific service area or team, utilizing knowledge of state-of-the-art programs and promoting social work values such as patient self-determination and social justice. Initiates, participates in and supports policy and program development in the Department of Social Work Services.
15. Actively engages in the regularly scheduled meetings, programs and activities of the Department. Serves on departmental committees. Represents the profession and Department in hospital and community meetings and committees.
16. Participates in social work professional education, supervision and teaching activities; supervises or trains staff social workers, student interns, house staff, nurses, medical students, and volunteers as required.
17. Maintains collaborative, team relationships with peers and colleagues in order to effectively contribute to the working groups achievement of goals, and to help foster a positive work environment
18. Continually monitors, assesses and modifies his/her own social work practice and programs. Conducts quality improvement activities and research regarding service delivery and effectiveness of interventions, as required.
19. Conducts or participates in professional organizations, grant writing, teaching, public presentations, seminars and workshops. May supervise staff social workers, social work associates, social work interns, parent consultants, house staff, nurses, medical students and volunteers as required.
20. Ensures compliance within guidelines set forth by regulatory agencies (DPH, ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.

Job Summary
Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds - including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service - for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.
The Massachusetts General Hospital seeks a dynamic Licensed Clinical Social Worker (LCSW)/Licensed Independent Clinical Social Worker (LICSW) to serve on the Special Operations Team. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. They will also be dedicated to supporting the Special Operations Forces (SOF) program and will provide coverage to other clinical operations as needed. The LCSW/LICSW may help cover approximately 2-3 weekend shifts per year.
Summary
This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
* Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
* Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
* Coordinate care for Special Operations Forces (SOF) veterans and military-connected family members seeking care at Home Base with the appropriate internal or external clinicians in a multi-disciplinary team, coordinated care model.
* Serve as a contact point for accepting and triaging SOF referrals from internal clinicians, outside clinicians and self-referred patients.
* Serve as primary case manager for patients in SOF admissions process in collaboration with Admissions Coordinator, including, but not limited to, the following responsibilities: conduct pre-screening to assess treatment and evaluation needs for SOF patients referred; coordinate signing of Release of Information forms, acquire relevant medical records, and coordinate with SOCOM where applicable; serve as point of contact for ongoing questions from the patient as they arise.
* Monitor patient's status in admissions process using appropriate documentation.
* May be asked to collaborate with SOF/ComBHaT team, patients, clinical team, and Resource Specialist to identify clinical and psychosocial needs in patients' home communities and identify appropriate resources.
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties.
* Collaborate with an inter-disciplinary team of clinical staff (social workers, psychologists, nurses, psychiatrists, physiatrists, physical therapists, etc.) to coordinate patient care and plan for discharge/aftercare when needed.
* Consult with team to facilitate mandated assessments when abuse is suspected (child, disabled adult, elder), and safety assessment when violence is reported. Collaborates with MGH resources (HAVEN and Child Protection Team).
* May assist with crisis intervention and management.
* Documents timely and relevant information in patient electronic medical record and Home Base database.
* Provide coverage for social work responsibilities in Outpatient Clinic as needed.
* For LICSWs: provide clinical supervision to LC-level and/or MSW students as needed.

Job Summary
Mass General Brigham Health Plan is hiring for two Behavioral Health Specialists to work as part of an interdisciplinary care team providing expert recommendations for managing behavioral health conditions and providing specialized coordination of care for enrollees with Serious Mental Illness (SMI) and Substance Use Disorders (SUD).
As the behavioral health clinical expert on the Interdisciplinary Care Team, the Behavioral Health Specialist partners with the Interdisciplinary Care Team, facilities, and providers to address behavioral health challenges, evaluate treatment options, and implement targeted strategies and interventions.

* Participate actively in ICT meetings, contributing insights on behavioral health concerns, treatment options, care strategies, and engaging in collaborative decision-making regarding enrollees' care.
* Receive and triage behavioral health and substance use referrals, coordinating with the ICT and primary care providers to refer enrollees to appropriate inpatient or outpatient services.
* Develop, update, and implement enrollee-centered care plans in collaboration with enrollees and care teams, promoting self-care, shared decision-making, and addressing identified needs through routine engagement and proactive outreach.
* Conduct outreach, assessments, and home visits via telephonic, electronic, or in-person contact for identified enrollees, including routine evaluations and post-transition care.
* Monitor clinical status to identify early signs of deterioration, intervening proactively with the ICT to prevent unnecessary hospitalizations and escalate behavioral health needs during weekly case conferences.
* Establish consistent communication and reporting schedules with providers and enrollees to review enrollee status, progress toward goals, care needs, utilization plans, and follow-up.

Job Summary
The Clinical Social Worker (LICSW) is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families.
Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high risk psychosocial factors of patients/families that impact health status and helps hospital staff understand the influence of those factors upon the course of medical care.
Assumes clinical evaluation, intervention and planning for patients with complex psychosocial risk (homelessness, protective services, frail elderly, disabled, psychiatric and substance abuse, etc.).
Works effectively as part of interdisciplinary health care teams.
Provides clinical services to patients/families that address psychosocial, environmental, age-specific and cultural issues in order to maximize emotional, social and physical well being and the effective use of health care and community resources.
Collaborates with and provides social work consultation to other disciplines within the setting and community.
Participates on Departmental, Hospital, Satellite, community task forces and committees.
The Clinical Social Worker reports directly to the assigned Social Work Manager.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Clinical Practice:
* Provides assessment of patients to evaluate mental health/psychiatric history/emotional issues/coping style, understanding of illness/adjustment/compliance, barriers to care, cultural issues, abuse/neglect and domestic violence.
* Provides psychosocial assessment of families to determine family relationships/systems as they relate to care of the patient. Identifies family decision makers and caregivers; family understanding of illness and trajectory of care. Identifies family coping style, family resources and cultural issues.
* Employs a range of clinical interventions such as individual, group or family counseling. Provides caregiver/family counseling/support to promote family cohesiveness to provide care to patient and prepare families for end of life. Advocates on behalf of patients and families to gain access to services and resources. Refers patients to other providers, as necessary.
* Develops comprehensive bio-psychosocial assessments responsive to age appropriate and cultural needs and concerns. Employs a range of clinical interventions such as psychotherapy (individual, couples, families, and group), psychosocial counseling, crisis intervention, care coordination, complementary therapies, information and referral and safety planning. Advocates on behalf of patients and families to gain access to services and resources.
* Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and safety assessment when domestic violence is reported. Files reports as indicated.
* Identifies patients' psychosocial, financial, legal, psychiatric or substance use that effect patient care management and collaborates with the team to facilitate patient care process.
* Works effectively as part of the interdisciplinary health care team, communicating regularly with the team and other members on cases and as issues arise. Documents timely and relevant information.
* Coordinates family/team meetings, as needed and when appropriate. Provides psychosocial consultation on patient care planning and patient/family management and community resources. Implements psychosocial programs based on patient/family identified needs.
* Facilitates the appropriate and efficient use of hospital and community resources.
* Participates in formal and informal clinical case reviews, clinical supervision, educational seminars and research projects.
Quality, Utilization Management: High Risk Psychosocial:
* Intervenes with appropriate individuals/departments/agencies regarding delays in service that may have an impact on quality of patient care, length of stay or inappropriate patient admissions.
* Reviews patient information for assigned caseload, determines anticipated length of stay and psychosocial barriers to plan of care transitions discharge plan in collaboration with the Nurse Care Coordinator
* Interacts with home care, community agencies and facilities to ensure safe and timely patient care transitions
* Negotiates with care coordination team follow up contact with patient/family, community agency or facility to evaluate the effectiveness of the patient care transitions and identifies problems in service delivery
* Ensures coordination of the communication process with patient/family concerning the plan of care, including coordination of family meetings and warm handoffs.
* Ensures that patient/family is involved in all phases of the care process to the greatest extent possible.
* Maintains current knowledge of and identifies needs in service delivery within social, governmental, protective services and legal agencies.
* Participates in data collection for departmental quality assessment activities in collaboration with the care coordination department.
* Participates in quality assessment/improvement activities designed to evaluate the appropriateness and effectiveness of the service delivery system in which care coordination operates.
* Ensures that the patient and family receive consistent information regarding all aspects of care.
* Communicates and collaborates with the Social Work Manager/Team to ensure efficient and quality patient care and equitable caseloads.
Leadership, Teaching and Education:
* Assesses patient/family learning needs, styles and readiness. Educates patients/families based on treatment plan, identifies barriers to care, diversity issues and learning styles.
* Mentors and may supervise students and staff. May teach in Departmental and Hospital seminars, workshops and rounds.
* Demonstrates expert social work clinical practice within the department and with interdisciplinary staff. Provides education and consultation to interdisciplinary health care providers, social work staff and community on psychosocial issues for patients.

* Participates in Departmental and Hospital committees. May participate in social work research.
Organizational/Administrative Skills:
* Takes responsibility for own administrative duties, including timely and appropriate documentation in patient medical records, timely and accurate daily reporting of activities and Hospital's scheduling systems, and accurate reporting of time worked.
* Provides clinical documentation including psychosocial assessment, progress notes, and billing compliance (if appropriate).
* Attends and participates in Staff Meetings and interdisciplinary meetings/rounds.
Professional Conduct:
Adheres to and fosters compliance with NASW Code of Ethics, and Department and Hospital clinical, quality, compliance and safety standards, policies and procedures.
Supervisory:
Expected to mentor, precept, teach social workers and social work residents
Fiscal:
Meets Department productivity and standards. Ambulatory staff, ED and ED on-call are responsible for billable hours.
Hospital-Wide Responsibility:
Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.

Education: Master's of Social Work Degree from an accredited program required.
Licensure: Require current Massachusetts Licensed Independent Clinical Social Worker (LICSW) required, or Licensed Clinical Social Worker (LCSW), or must obtain MA Licensed Clinical Social Worker (LCSW) within 6 months of hire date.

Job Summary
The Massachusetts General Hospital seeks a full-time, licensed social worker (LCSW or LICSW) for a one-year training program in Addiction Medicine starting in June 2026. The Integrated Substance Use Disorder Training Program (ISTP) is a yearlong fellowship that aims to train nurse practitioners, psychologists, and social workers in team-based addiction care, especially in community-based settings.
The Social Worker Fellow will work with other ISTP trainees to provide inter-disciplinary care for people with substance use disorders in a community-based health center setting. The training program will provide clinical experience in a variety of placements including an inpatient addiction consult service, a low-threshold walk-in clinic, and outpatient dual-diagnosis programs for adolescents and adults. Training and mentorship will be provided by experienced addiction practitioners who are faculty of this program.

1. SCHEDULING

Job Summary

The Newton-Wellesley Hospital Domestic Violence and Sexual Assault Program provides individual, community and systems responses to sexual assault and domestic violence through direct advocacy services, education, community linkages, quality improvement and program evaluation. · Provide direct services to primary and secondary survivors of sexual assault, domestic violence, and stalking. Services include but are not limited to crisis intervention, collaborative safety planning, advocacy, short and in some instances long-term counseling, as well as the provision of information, and supported referrals. · Holistically and organically assess the particular needs of each survivor and ensure that appropriate services are offered to the survivor and any safe family and friends, including children, elders and animal companions. · Provide on-call consultation to hospital and community-based health care providers. · Develop and co-facilitate support, empowerment and wellness groups for survivors of interpersonal violence. · Provide outreach in the hospital and in the community to increase awareness of the availability of services for survivors. The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.
