The PCC provides for the coordination of rehabilitation nursing care to patients in a specifice unit/program. This role functions as a clinical rehabilitation nursing resource to all nursing staff and interdisciplinary team members. The PCC coordinates the overall education plan for patients and assists the RN assigned to the patient in providing educational resources, consulation and guidance. In conjuction with direct care providers, the PCC provides guidance and consultaion to designated preceptos on the unit and assists the nurse manager in coordinating care planning, assessing the patient experience and participation in various clinical inter-disciplinary work groups including telehealth, skin care, fall prevention and other dashboard outcome measures. Additionally, in collaboration with the nurse manager, the PCC will provide direct care for a selected number of patients as needed. Experience: \t Must be a Registered Nurse licensed in Pennsylvania \t BSN and CCRN required \t MSN preferred \t Minimum 5 years of licensed professional nursing experience and minimum of 3 years in rehabilitation nursing.
