The Arc's Early Intervention Program, funded by the NJ Department of Health, is open to any child from birth to age 3, who has a developmental delay or disability, or who is at risk for developing a delay or disability. Following a comprehensive evaluation of a child's needs, the Early Intervention Team develops a coordinated approach to the provision of developmental intervention services, which includes physical, occupational and/or speech therapy, behavior intervention, and social work services. All services are provided within the child's natural environment.
This position provides services to children ages 0-3 with developmental needs and their families in-home and/or community settings throughout Union County. This role requires a compassionate professional who is dedicated to making a significant impact on a child's ability to learn new skills and help overcome challenges to increase success in school and life. This role requires a patient skilled professional who encourages and inspires our clients to work toward greater independence while meeting their developmental milestones.
Early Intervention is a family-training model. This position provides a unique approach to best support our clients and families' individual needs. As a Developmental Interventionist, this staff along with our collective Early Intervention team takes a person-centered approach and focuses on the child's strengths and individual differences, while developing an intervention plan that addresses his or her unique profile. The incumbent will be responsible for providing home and based early intervention services to infants and toddlers who have or are at risk for developmental delays and their families. The incumbent will partner with parents/caregivers to ensure service delivery is individualized, meaningful for the family, and grounded in evidence-based practice.
This position is to both educate and coach families on effective strategies to help their child reach developmental milestones and improve daily routines. Assist the family in understanding the special needs and development of their child. Perform Family Training with the family, caregiver and child in their home or daycare setting. Provide professional and responsible Service Coordination to meet the needs of the child and the family. Thos job assignment requires the practitioner to make home visits to train parents and caregivers in helping to improve developmental skills; us specific interventions based on the child's (birth to three) individual needs; acts as a role model for parents in the natural home environment; act as a liaison between professional licensed staff, parents, and agencies. Make verbal and written recommendations to parents, caregivers, staff, agencies for the purpose of assisting infants/toddlers towards successful implementation and management of in-home training.
Note: All completed NJEIS authorization logs, progress notes and accompanying bills for services are due to the agency no later than the 1st working day of each new month. All dates on the authorization must include the day, month and year. Each authorization should reflect the services provided to a single child and must include a valid parent verification, i.e., a dated signature attesting that the type and amount of services delivered on the given date. Authorizations must reflect only the current month in which services have been delivered and must be signed by the family at EACH session.
Scope and Limits of Authority:
This position is responsible for acting in compliance with a child's IFSP and in accordance with NJEIS requirements. This position does not have staff reporting responsibilities and functions under direct supervision and instruction from the Sr. Director, Early Intervention Services. Receives structured project direction and assignments from the Director, Early Intervention Services. Incumbents work within a framework of established goals and objectives. Incumbents in this classification are not authorized to assign, coordinate, supervise and or evaluate the work of others.

Qualifications

Coordinate and ensure that once an individual is discharged from a hospital stay, rehab center or ER, all medications are reconciled, accurate and documented consistently on the discharge documentation and/or prescriptions.
Ensure that Physician completed pre/re-admission/medical clearance form for hospitalized or rehab individual is complete and accurate before and/or at time of discharge and within appropriate periods.
Follow up and observe the individual in the home after hospital or rehabilitation discharge.
Assure that Agency protocol is followed when an individual is being discharged from a hospital or rehab facility.
Coordinate any therapeutic services needed - Physical therapy, speech, occupational etc.

Coordinate telephone calls and communications for residential programs with physician offices, pharmacy and other medical institutions on medical order request or clarifications etc. Log and respond to internal/external informational requests within agency guidelines and as directed by executive designee.
Observe and asses the assigned pharmacy's performance relative to the agency's medication processing and delivery efficiency and effectiveness relative to the agency's structured protocols, expectations, etc. including full adoption of the agency's centralized structure of prescription processing, structured weekly medication delivery at headquarters and timely deliveries of stats at all residential sites. Receive medications for all individuals served in residential programs and coordinate medication inventory with program supervisors. This includes but is not limited to reordering/processing with agency designated pharmacy for delivery.
Verify the refill date as indicated on the “Too Soon Notification”.
Verify the supply on hand in the residential group home upon receiving “too Soon Notification” from the pharmacy.
Contact Partners Pharmacy and find out when the next refill will occur upon receiving “too Soon Notification” from the pharmacy.
Compile and provide Executive Report quarterly featuring program, service recipient and medication that has featured refill too soon.
Request Partners Pharmacy to check again for refills and request Partners to contact the physician upon receiving “no refill notification from the pharmacy”.
Upon receiving “no refill notification from the pharmacy” factor the supply on hand in the residential group home, assess if supply of medication is inadequate for the resident's needs, request an emergency supply to be sent and inform the team for immediate follow up.
Follow up on Emergency Supply Request until it is resolved associated with “no refill notification from the pharmacy” etc.
Notify nursing, pharmacy, supervisor, Assistant Director that a Prior Authorization for medication needs to be completed by the physician for the agency nurse to write a letter to physician and the pharmacy as well.
If the supply of medication in the Group Home is inadequate to meet the needs of the resident, request from Partners Pharmacy an emergency supply to be sent to the main office within administrative hours or to be delivered to the Group Home after administrative hours or weekend. Follow STAT Medication Delivery Protocol if the emergency medication supply has to be delivered or picked up at Partners.
Inform the Nurses of the number of supplies available including Inadequate Supply.
If the supply of medication is inadequate to meet the needs of the resident, will request an emergency supply to be sent to the main office within administrative hours or to be delivered to the Group Home after administrative hours or at the weekend. Follow STAT Medication Delivery Protocol if the emergency medication supply must be delivered.
If situations arise resulting in inadequate medication's supply, initiate STAT (immediate delivery request).
Address and resolve by Thursdays all medication supplies for residents since Residential Group Homes' Supervisors are off on Friday to address arising issues related to medication recount etc., it is recommended that all medication supplies for residents' need are addressed and resolved on Thursday. Factor 6-day Supply-Rule to prevent last-minute requests and STAT.
Process all medication requests. Scan all “too soon communications” from pharmacy into Therap for support to understand how to restructure the process.
Remind staff as necessary to print New MCRs from the Pharmacy app.
Provide programs with MCRs as soon as the pharmacy receives e-scripts so that we can have updated scripts on the MAR. Alternatively, print MCR when requesting STAT delivery as well.
Follow up with the pharmacy on missing medications from the delivery and/or any discrepancies.
Maintain a record of all admitted individuals and ensure that medication refills are stopped and resumed as needed after discharge from inpatient external stays.
Maintain record of medication delivery for accountability.
Maintain inventory of medical supplies.
Plan for scheduled and impromptu closings by anticipating medication refill needs during holiday closures, inclement weather closings, etc. Notify pharmacy of closing.
Observe and asses the assigned pharmacy's performance relative to the agency's medication processing and delivery efficiency and effectiveness relative to the agency's structured protocols, expectations, etc. including full adoption of the agency's centralized structure of prescription processing, structured medication delivery at headquarters and timely deliveries of stats at all residential sites. Communicate with your supervisor and/or intervene promptly of any deviation pharmacy to honor its fiduciary responsibility obligation.

Administrative Duties
Submit 025 progress reports within a proper timeline as mandated by the agency and state regulations.
Submit daily sessions into the EIMS system in real time for claim processing requirements as mandated by the agency and state regulations.
Submit monthly billing on the first of the month or on designated date assigned as mandated by the agency billing procedures.
Quality Control
Review and revise reports and assessments as needed.
Individual and or management quality review daily, weekly, monthly service documentation to ensure all appointments have thorough and accurate supporting documentation.
Sign all documentation under the supervision of the Sr. Director of Children's Services monthly.
Submit semi-annual program progress report to management as required.
Client Relations
Schedule, coordinate and/or conduct individual or group meetings with parents related to specific topics or areas of interest.
