Position Overview
In collaboration with a supervising physician, the NursePractitioner provides comprehensive in-home clinical assessments, develops individualized care plans, and coordinates primary and preventive care services for an assigned panel of members residing in the community.
You will play a critical role in reducing avoidable hospitalizations, supporting safe transitions of care, and improving the quality of life for health plan members.
Key Responsibilities
Conduct comprehensive clinical assessments in the home, including physical, psychosocial, and functional evaluations.
Develop and update individualized care plans using evidence-based guidelines.
Coordinate with Care Managers and IDT members to ensure timely follow-up, appropriate services, and continuity of care.
Facilitate safe transitions from hospitals, subacute rehab, or nursing facilities using the modified Coleman Model.
Order diagnostic tests, therapeutic services, and preventive care interventions as appropriate.
Reconcile medications, monitor polypharmacy, and optimize pharmaceutical management.
Evaluate the need for acute inpatient care and communicate findings with collaborating physicians.
Educate members and families on disease management, preventive care, and end-of-life planning when appropriate.
Participate in interdisciplinary team meetings and serve as a clinical resource to staff.
Maintain accurate, timely documentation in EMR systems.
Qualifications
Master's Degree in Nursing and graduation from an accredited NursePractitioner program.
Active NYS RN / NP license, registration, and DEA certificate required.
Minimum 1 year of NP experience, including geriatric care.
Prior experience as an RN in acute, subacute, or long-term care strongly preferred.
Managed care experience is a plus.
Strong communication, independent decision-making, and care coordination skills.
Comfort working field-based with regular home visits (reliable vehicle required).

DUTIES & ESSENTIAL JOB FUNCTIONS
Work closely with Infectious Disease Attending physicians to provide evaluation and follow-up services for outpatients in the ID clinic. The scope of service includes managing patients receiving outpatient intravenous antibiotics, patients with HIV or hepatitis C, and patients with latent and active tuberculosis.

Treatment plans are documented as progress notes in the electronic medical records system. Provides women's health and wellness services including breast and cervical cancer screening / PAP tests and coordinating a respective Clinical Quality Improvement (CQI) project,
Provides and monitors continuity of care between patient visits and evaluation and management of emergency situations.
Provides psychosocial counseling and support to patients and families and makes community referrals as necessary.
Identifies treatment-related toxicities and initiates medical interventions.
