The Mental Health Worker is responsible for an expansive caseload of clients; for designing, developing and implementing therapy and skill training groups, modifying the curriculum as needs change, while meeting state regulations with regard to charting requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Assume overall treatment responsibility for assigned caseload, which includes presenting cases to the Treatment Team, developing assigned clients' treatment plan, providing input to the Team regarding other cases being presented.
2. Identifying client needs/problems, strengths, goals and developing a plan to meet the goals as part of the treatment planning process.
3. The compilation of and maintenance of timely and accurate case records on assigned clients which include daily progress notes on clients, treatment plan reviews, appropriate referral forms, chart audits, intake and other forms as needed.
4. Conduct individual therapies as needed with assigned clients and assume the role of group leader in group therapy sessions and other skill training as assigned.
5. Establish and maintain communication with other NTC treatment staff of staff from other agencies regarding mutual clients when it is appropriate and necessary to do so to meet the treatment needs of the client.
6. Attend and participate in inter-agency meeting regarding mutual clients as needed.
7. Assist in referring assigned clients to other needed community services, such as Sheltered Workshop employment, Community Residential Rehabilitation and Intensive Case Management.
8. Participate in assigned Quality Assurance activities.
9. Participate in Staff Development activities to enhance skills and education.
10. Assist assigned clients in accessing transportation through the Endless Mountains Transportation Authority.
11. Utilize NTC's Electronic Health Records (E.H.R.) during tenure of employment. Initial training and on-going training will be provided to employee by respective supervisor and/or assigned team member.
12. Employee will be proficient in their job position within six (6) months. If at any time an employee feels they need more training/education, employee is to submit a request for such through their supervisor, manager, and/or director.
13. Other duties and functions as assigned.
