Responsibilities
* Responsibilities include, but are not limited to:
* Under collaborative practice:
* Perform comprehensive medication management by initiating, adjusting and discontinuing medications (ex. supportive care, antimicrobials etc.)
* Order and interpret pertinent labs and clinical parameters to guide therapy
* Manage adverse drug reactions, drug-drug interactions and therapeutic drug monitoring (ex. tacrolimus, sirolimus etc.)
* Clinically review IV and oral transplant and cellular therapy treatment regimens and supportive medication orders

* Coordinate transitions of care for patients moving outpatient

Qualifications
EDUCATION
Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree
EXPERIENCE
The applicant must be a graduate of an accredited College of Pharmacy and CT pharmacist licensure eligible. Completion of an ASHP accredited PGY1 residency or equivalent hospital experience is required. Completion of an oncology PGY2 residency or equivalent experience is required.
LICENSURE
Current Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut
SPECIAL SKILLS
This individual should demonstrate in depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.

Identifies and resolves basic order entry and prescribing problems. Makes appropriate interventions to ensure patient safety and regulatory compliance.
b. Dispenses accurately labeled medications in the appropriate packaging in a timely manner. Understands all drug preparation techniques and can perform or oversee the timely preparation of medications to ensure product and patient safety.
c. Ensures all sterile compounded medications and IV solutions are handled and labeled according to applicable regulatory standards and policies.
d. Understands and complies with required Pharmacy Department documentation in all areas and do so in a clear concise manner.
e. Understands the purchasing process and assists in inventory control to reduce waste. obtain drugs in select emergency situations.

● Conduct benefits investigation and validation of insurance coverage for requested specialty medications
