The Wounded Warrior Project (WWP) Physical Health & Wellness (PH&W) Specialist develops and executes multiweek programs and events to educate, coach, and empower wounded service members in pursuit of achievement of their physical health and wellness goals.
DUTIES & RESPONSIBILITIES
Identify, screen, and recruit warriors for participation in quarterly virtual and in person Physical Health & Wellness events and coaching programs.
Plan agenda and execute multiday PH&W coaching program kick off events, providing fitness, nutrition, mobility, sleep, and health-focused education to warriors with limited to no supervision from manager.
Create individualized coaching programs to help warriors achieve their physical health and wellness goals.
Manage warrior participation throughout 90-day coaching program, providing encouragement, support, and connection to resources through regular coaching sessions delivered by telephone or Zoom.
Provide instruction, guidance, and coaching using motivational interviewing skills to encourage behavior change and support the progress of warrior's health focused SMART goals.
Develop and execute in person events, virtual education, competitions, and wellness challenges for warriors and their families.
Develop and facilitate virtual programming content via Zoom, Facebook Live, and other technology-based platforms.
Collaborate with physical health and wellness teammates to develop new, innovative content and curriculum.
Identify and recommend new programming opportunities aimed at increasing overall impact.
Learn and implement various modalities to execute during events and follow up, including Learning Management Systems (LMS).
Establish and maintain community relationships to build an ecosystem of PH&W support for warriors.
Collaborate with multiple WWP departments and other key stakeholders to deliver programming and services to warriors and their families.
Conduct public speaking activities to raise awareness of PH&W programming and discuss the importance of starting and maintaining a healthy lifestyle.
Maintain budgets for PH&W programming within assigned region in support of leadership.
Manage information within various systems, ensuring accuracy while maintaining data integrity/security, e.g. Salesforce.
Utilize validated metrics to meet strategic outcomes and key performance indicators.
Manage multiple tasks, assignments, and functions, being comfortable in a fast-paced, innovative environment.
Participate in workgroups for special projects and programming initiatives as assigned.
Assist with onboarding and training of new teammates as needed.
Attend and actively participate in required training and/or meetings, including but not limited to New Teammate Orientation, WWP Cares, ASIST Suicide Prevention training, Leadership training, culture/team based training, or departmental huddles.

EXPERIENCE
Requirements
Four years of experience working in the fitness industry, allied health profession, health management, wellness coaching, strength and conditioning, or personal/athletic training.
Two years of experience with longitudinal plan design, teaching, guiding, and coaching in the categories of fitness, nutrition, wellness, and/or athletics.
Preferences
Two years of experience working with and providing support to wounded service members dealing with health issues and invisible injuries.
One year of event planning experience.
One year of experience with customer relationship management and event management systems, e.g., Salesforce, CVENT.
EDUCATION
Requirements
Bachelor's degree in exercise science, exercise physiology, kinesiology, nutrition, human performance, or a related health science field.
Preferences
Master's degree in exercise science, exercise physiology, kinesiology, nutrition, human performance, or a related health science field.
CERTIFICATIONS & LICENSURE

Valid state-issued driver's license.

Position Summary
The HOPE Program is seeking an Employment Coach who will provide one-on-one and group based support, connecting HOPE program participants to job opportunities, ensuring placement and continued employment while offering career advancement opportunities that lead to increased hourly wages. The position receives daily supervision from the Employment and Retention Manager and reports to the Director of Employment and Business Services. The Employment Coach is expected to work a minimum of four days from the office across all HOPE and partner locations and work schedule will include evening and weekend hours.
The Employment Coach will work with program participants to assess their level of job readiness, understand and guide their career plans, review and finetune their baseline resume, cover letter and interviewing skills, and assist them in searching for in-person and remote work to connect them to jobs that are aligned with their career plan.
Key Responsibilities
Build relationships and manage a caseload of up to 150 program participants and alumni annually, ensuring that 80% of them secure employment within 90 days after program completion for first time placement and within 30 days for re-placements. This equates to 10 job placements per month. Report progress towards goals and outcomes on a weekly basis.
Ensure a minimum 85% 90-day retention rate, 75%180-day retention rate, and a 65% 365-day retention rate, by maintaining regular contact with participants, assessing job satisfaction, performance and sustainability of the job, providing consistent coaching, and partner with other HOPE staff to make referrals for other support services as needed. Report progress towards goals and outcomes on a weekly basis.
Through the use of a holistic, student-centered approach, provide consistent and high quality employment-focused group based and one-on-one engagements and services to ensure that students are qualified candidates for the job opportunities that they seek. More specifically,
Develop an engaged coaching relationship with participants.
Partner with participants on their short and long term career goals and provide guidance in understanding their potential career pathways.
Perform job readiness assessments and co-develop coaching plans that will address barriers to employment.
Ensure all students have a complete and updated employment portfolio, including resumes, cover letters, career profile, and references.
Support in tailoring materials and practicing for interviews for open positions and ensure they are equipped for self-directed search.
Screen for HOPE-developed internships and job opportunities with trusted employers.
Maintain communication with students regarding applications, interviews, hiring, new hire follow-up, and documentation.
Secure proof of employment for new hires as well as for capturing retention milestones, either directly from program graduates or from the employer partner.
Develop and facilitate group-based work that includes (peer-to-peer) job search and applications activities, interview preparation, networking events, hiring events, panel discussions and other sessions and events that among others include employer partners or provide participants and alumni with valuable resources that support job placement and retention.
Coordinate activities designed to help graduates retain jobs and develop new skills to advance in their careers, including facilitating discussions on topics such as: retention incentives, conflict management, dealing with difficult employees/employers, employee rights, and financial planning.
Make career advancement training opportunities available that eventually will lead to increased hourly wages and salaries.
Conduct outreach to engage graduates for participation in day-time, evening, and weekend workshops, groups sessions, and events.
Engage program alumni as ambassadors for The HOPE Program and as contributors for providing services and facilitating events and activities.
Support the Business Development Managers with identifying and stewarding employer partner relationships.
Enter all required data into the Salesforce database and document activities in a timely manner to include employment and retention verification documentation, case notes, up to date participant contact, demographic, and other profile data,
Participate in case conferencing with other employment and program staff
Document participant and employer feedback and testimonials.
Support a culture of HOPE for Life where participants remain engaged with The HOPE Program and pay it forward when they can.
Perform other duties as assigned.
Minimum Qualifications

At least two years of case management or workforce development experience.
Demonstrated ability to support clients in meeting their personal goals (this position is measured against placement rate).
Ability to adapt and course correct with focus on meeting goals and outcomes.
Excellent oral and written organizational and communication skills.
Experience documenting interactions with clients within a 48 hour period and using a CRM (HOPE uses Salesforce).
Google Suite, Zoom and Internet research proficiency.
Ability to build rapport with a wide range of clients and practice client-centered counseling.
Must be a strong team player who works well independently.
Problem solver, flexible and adaptable.
Experience in developing processes is preferred.
Work Environment
This is a hybrid position working at least 4 days per week at HOPE and partner locations and work schedule will include evening and weekend hours.
Work hours for this position may be subject to change based on organizational needs. Employees may be required to adjust their schedules, including working outside of standard business hours, as necessary to fulfill the responsibilities of the role.
Employees serve as ambassadors of The HOPE Program and may be called upon to represent the organization at community events, partner meetings, public forums, and other external engagements, helping to advance our mission and strengthen relationships with stakeholders, including employers and community partners.
Salary and Benefits

The goal of the CHIME study (Comparing Health Interventions for Maternal Equity) is to compare three different approaches to overcoming barriers to maternal healthy weight and chronic disease prevention. The community-based arm of the study will provide culturally relevant nutrition and physical activity and Health Coach support for pregnant and postpartum patients to improve maternal healthy weight in pregnancy and postpartum to prevent adverse maternal infant health outcomes.
Summary of Position :
PHS is seeking a health coach to fulfill the following duties:
Provide health education and home visiting services to pregnant participants throughout pregnancy and through 12- months postpartum.
Facilitate curriculum-based group health education workshops and support enrolled participants in the CHIME study with maternal & child health resources in their community.
Responsible for working with participants on their nutrition, physical activity, and lifestyle goal setting, as well as a detailed social needs screening, assessment, and referral.
Meet with participants on a weekly basis (at their home, in the office, or in the community) and keep detailed notes on their progress and activities, providing in-person support, text-message resources, and referrals to community services.

Qualifications and Requirements:

General Description
Behavioral Health Recovery Coach provides individuals who are diagnosed with mental health, substance use and/or chronic health conditions and are eligible for Home and Community Based Services (HCBS) or Community Oriented Recovery & Empowerment (CORE) Services with recovery-oriented activities and interventions, using a trauma-informed, non-clinical approach. Activities and interventions include individual's recovery goals, including education and vocational objectives.
Essential Duties and Responsibilities
Assist individuals with learning adaptive behaviors and responding to or avoiding identified precursors such as cravings or triggers that result in relapse in unhealthy behavior or functional impairments.
Provide training to individual/family on treatment regimen choices including but not limited to: recovery support options; recovery concepts; advocacy; understanding substance use disorder/mental illness or chronical health conditions and treatment; medication education; effects of trauma; psychosocial rehabilitation; habilitation; crisis intervention and related tools and skills including but not limited to Individual Recovery Plans, self-care, emotional validation, communication skills, boundaries, emotional regulation, behavioral relapse prevention, violence prevention and suicide prevention.
When appropriate, assist family members in the acquisition of knowledge and skills necessary to understand and address the specific needs of the individual who is Medicaid eligible in relation to their substance use disorder/mental illness and treatment.
Collaborate with family and caregivers to develop positive interventions to address specific presenting issues and to develop and sustain health and stable relationships to support the individual's recovery.

General Description
Behavioral Health Recovery Coach provides individuals who are diagnosed with mental health, substance use and/or chronic health conditions and are eligible for Home and Community Based Services (HCBS) or Community Oriented Recovery & Empowerment (CORE) Services with recovery-oriented activities and interventions, using a trauma-informed, non-clinical approach. Activities and interventions include individual's recovery goals, including education and vocational objectives.
Essential Duties and Responsibilities
Assist individuals with learning adaptive behaviors and responding to or avoiding identified precursors such as cravings or triggers that result in relapse in unhealthy behavior or functional impairments.
Provide training to individual/family on treatment regimen choices including but not limited to: recovery support options; recovery concepts; advocacy; understanding substance use disorder/mental illness or chronical health conditions and treatment; medication education; effects of trauma; psychosocial rehabilitation; habilitation; crisis intervention and related tools and skills including but not limited to Individual Recovery Plans, self-care, emotional validation, communication skills, boundaries, emotional regulation, behavioral relapse prevention, violence prevention and suicide prevention.
When appropriate, assist family members in the acquisition of knowledge and skills necessary to understand and address the specific needs of the individual who is Medicaid eligible in relation to their substance use disorder/mental illness and treatment.
Collaborate with family and caregivers to develop positive interventions to address specific presenting issues and to develop and sustain health and stable relationships to support the individual's recovery.
Assist individuals to develop skills necessary for community living and recovery with ongoing assessment of their functional status and creation of (re)habilitative goals.
Develop and implement behavioral heal and/or health education support groups for individuals with substance use, mental health, or chronic health conditions.
Instruction in self-advocacy skills including activities designed to facilitate individuals' ability to access social service systems (e.g. health care, substance use treatment, employment, vocational rehabilitation, entitlement/benefits, self-help groups, etc.) and other recovery-oriented systems of care.
Assistance with increasing social opportunities and developing social support skills that improve life stressors resulting from the individual's disability and promote health, wellness and recovery.
Assist Individuals with linkage to and system navigation within behavioral health and human services systems to access appropriate care.
Serve as a professional resource and provided leadership within the department and among staff.
