Job Summary Roy Street Transitional Housing is a 40 unit transitional housing program that works with 18-24 year olds in need of longer term housing, located in Capitol Hill. We are a 24/7 site offering services to young adults on life skills, housing management and employment. We also are offering a community based portion of the program that you will be trained on and experience community based base case management. This position is with the Y Social Impact Center, the social services division of the YMCA of Greater Seattle (YGS), serving around 5,000 youth, young adults, and families every year in communities across King County, Pierce County, and various counties across WA. Through our programs we aim to strengthen young people's ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; establishing stability, safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Center's programs include homelessness prevention, housing, behavioral health, crisis response, foster care licensing, violence prevention, and employment. Since many of the young people we serve have been involved in the foster care, criminal justice, behavioral health, and/or homelessness systems, we truly value relatable lived experience of our staff and those applying for our positions as an asset. Position Type/Expected Hours of Work These are full-time, onsite roles. There are currently openings for three shifts at Roy St. and the shifts available are: * Wednesday-Sunday 2PM-10PM, or as needed. Ability to speak any language in addition to English may be helpful. * Some Case Manager positions required Bachelor's degree or higher. Within 30 days of employment or first available training. Authorized Driver Preference This role has the potential to transport clients as an "authorized driver" for the YMCA of Greater Seattle (YGS) using an employer-provided vehicle. Driving duties for this role are optional, but preferred.* Driving roles are safety-sensitive. Job Summary: Share's Housing and Essential Needs (HEN), Rapid Re-Housing (RRH) and Permanent Supportive Housing (PSH) programs use the Housing First model to move the most vulnerable houseless individuals and families into housing, provide short and long-term rental assistance and case management support to help people stabilize in housing and not return to houselessness. The Housing Case Manager works with clients to obtain housing and ensure long-term housing sustainability. Case management is specifically focused on housing navigation, housing maintenance, stability, and retention: paying rent, interacting positively with landlords and neighbors, assisting clients in finding supports that help them manage behaviors that may accompany mental illness or substance use, developing crisis plans, connecting with appropriate community resources, and supporting clients' individual housing goals. Key Responsibilities: Work with each client to develop and progress on goal plans according to individual goals and objectives. Work with clients to identify housing opportunities in the community. Maintain contact with all housing clients on a scheduled basis, appropriate to their status and needs. Ensure that client interactions are person-centered, goal-focused and reflect each client's goal plan. Provide transportation to clients when required. Make referrals to supportive services in our community as appropriate and support client in navigating this process. Effectively use the HMIS database and other tools to ensure compliance with data standards and record-keeping as required by Federal, State, County and City funders Document all client interaction and maintain accurate paperwork/reports. Perform re-certifications and housing inspections for each client and accurately update files regularly. Attend regular case management meetings and other specified meetings/trainings as required. Establish and maintain effective working relationships with a wide and diverse range of co-workers, supervisors, community organizations, and other human service providers. Operate proficiently a personal computer, scanner, printer, and copier. Must Have's: Bachelor's degree in related field, or 2 or more years working with the population and an understanding of the strengths and needs of people experiencing houselessness, low-income, elderly and differently abled individuals. A dedication to supporting and empowering the client utilizing best practice interventions including a housing first philosophy, harm reduction, and trauma informed care. Ability to hold clients with severe clinical needs in unconditional positive regard and provide services without judgment. Deep respect for clients' autonomy and inherent right to make their own choices about their own lives, while also helping them understand and navigate those choices.
