Joshua's House is the West Coast's first home for individuals with terminal illness who are at risk of homelessness. Overseen by YoloCares, the program extends exceptional care to those most marginalized and neglected by traditional healthcare systems-those with no place to call home.
In coordination with Joshua's House staff, volunteers, and YoloCares' clinical staff, the Caregiver assists residents in all aspects of their end-of-life journey, including personal care, food service, housekeeping, laundry, behavior management, socialization, activities, orientation, and information/communication needs.
Joshua's House is located in South Natomas, CA and will be the primary work location. Training will be conducted at the YoloCares office in Davis, CA.
Requirements:
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assists with or provides personal care, e.g., shower, tub bath, bed bath, oral hygiene, shaving, shampooing, cleaning nails, assists to toilet, etc. as desired by a resident, directed by a specific resident's written Plan of Care, and supervised by YoloCares' clinical staff.
2. Provides homemaker services by cleaning resident's room, washing resident's personal laundry, preparing and assisting with a resident's meal, and other homemaking duties, when the resident's Plan of Care directs.
3. Changes resident's clothing and bed linen as requested.
4. Increases resident's comfort and safety by prescribed rehabilitative measures such as positioning, ambulating and assistance with transfers. Uses basic adaptive equipment with debilitated patients when the Plan of Care requires.
5. Performs routine procedures such as feeding non-compromised residents, making occupied and unoccupied beds, and cleaning resident's equipment.
6. Demonstrates and teaches care techniques to volunteers and interns when appropriate.
7. Records vital signs when requested by the Plan of Care and/or as requested by clinical staff.
8. Follows universal infectious disease precautions when delivering all resident care (gloves, handwashing, etc).
9. Assists residents in taking self-administered oral OTC and prescription medications as directed by the clinical staff.
10. Encourages simple measures for resident's bowel and bladder regularity.
11. Observes/monitors resident's physical and emotional condition and reports changes to the Joshua's House Manager and clinical staff in a timely manner.
12. In compliance with state and federal regulations and with YoloCares policies and procedures, documents all resident care, including observations of significant changes: reactions to care, symptoms observed, mental and physical status changes in the patient, and coping changes noted in residents.
13. Participates in all Joshua's House team meetings, training sessions, and evaluation processes.
14. Other duties as assigned by supervisors.

PREFERRED QUALIFICATIONS
1. Experience working a NOC or graveyard shift.
2. Experience working with adults, the elderly, and or people experiencing homelessness.
3. Bilingual skills in any language are a plus!
4. Able to use Microsoft Office Outlook and Word.
5. Strong attention to detail, time management and organizational skills.
REQUIRED QUALIFICATIONS
1. Possesses effective verbal and written communication skills to effectively report and document patient care observations and facilities-related matters.
2. Ability to perform effectively under stress and when confronted with critical or unexpected events.
3. Ability to work independently and exercise sound judgement in a care setting.
4. Must have CPR certification. If not, must obtain one within 90 days if offered the position.
4. Compliance with YoloCares' mandated reporting policies.
5. Commitment to confidentiality and protection of resident information in compliance with HIPAA.
6. Ability to successfully meet YoloCares' background screening requirements.
7. Acknowledgment of Yolo Cares' mandated reporting policy.
8. Familiarity with and commitment to the YoloCares “Hospice Aide Core Values”, the “Statement on the Scope and Standards of Hospice and Palliative Nursing Assistant Practice,” and “Competencies for Hospice and Palliative Nursing Assistants” developed by HPNA.
PHYSICAL AND OTHER JOB REQUIREMENTS
1. Considerable walking and standing.
2. Occasionally stoop, kneel, crouch, or crawl.
3. Ability to lift to 30 pounds.
4. If driving on YoloCares business, must have a valid California driver's license and state-required automobile insurance.
5. Pass random drug testing.
SHIFTS AND PAY RANGES
Day: Monday through Friday NOC Shift- 11:00 pm to 7:30 am.
Pay: $45,760 - $49,920/Yearly

Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality

Requirements:
CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
Must have completed a minimum of 75 hours of classroom and supervised practical training
Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
Willingness to submit to a criminal background check prior to hire and annually thereafter
Current CPR certification

Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record

Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
Fulfill the obligation of requested and/or accepted assignments
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
Complete all other duties as assigned
Requirements:
Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Must have current nursing licensure in State
Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
Must be a licensed driver with licensed automobile that is insured in accordance with
organization/state requirements and is in good working order

An Average Day:

Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings
Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties; provide crisis intervention when necessary
Assess for, and educate interdisciplinary group, on any special needs related to the culture of the patient and family, including communication, role of family, space, and any special traditions or taboos
Maintain clinical records on all patients referred to social work
Educate patients and families on, and assist in, preparation of advanced directives
Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs
Provide information to patients and families/caregivers and community agencies
Serve as liaison between patients and families/caregivers and community agencies
Maintain collaborative relationships with organization personnel to support patient care
Maintain and develop contacts with public and private agencies as resources for patient and personnel
Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses on symptoms of the terminal illness
Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
Actively participate in quality assessment performance improvement teams and activities
Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
Perform other duties as delegated by the Clinical Supervisor
Requirements:
Must have a master's degree in social work from an accredited university.
If you hold an undergraduate B.A, Degree in Social Work, you are eligible to apply. Please know the hirable rate of pay will be based on degree type.
Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
Must understand hospice philosophy, and issues of death/dying.
Certified Hospice and Palliative Social Worker is desirable

Perform tasks associated with operational planning and budgeting
Ensure organizational compliance with legal, regulatory and accreditation requirements
Monitor business operations to insure financial stability

Requirements:
Bachelor's degree in Business Administration or Health Care preferred, or equivalent direct work experience
• Must have three (3) years of experience in health care management, five (5) years preferred
Ability to communicate and facilitate training via phone, WebEx, or in-person
Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
Must demonstrate an ability to supervise and direct professional and administrative personnel
• Must possess an ability to deal tactfully with the community
• Must possess a knowledge of corporate business management
• Must understand hospice care and the services provided to patient and family/caregiver through an interdisciplinary group
• Must possess an intimate knowledge of Medicare Hospice Certification

Will perform after hours and weekend visits and admissions as needed.
Follow up on phone calls by either directing the phone call to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or taking care of the need immediately per scope of practice

Joshua's House is the West Coast's first home for individuals with terminal illness who are at risk of homelessness. Overseen by YoloCares, the program extends exceptional care to those most marginalized and neglected by traditional healthcare systems-those with no place to call home.
In coordination with Joshua's House staff, volunteers, and YoloCares' clinical staff, the Caregiver assists residents in all aspects of their end-of-life journey, including personal care, food service, housekeeping, laundry, behavior management, socialization, activities, orientation, and information/communication needs.
Joshua's House is located in South Natomas, CA and will be the primary work location. Training will be conducted at the YoloCares office in Davis, CA.
Requirements
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assists with or provides personal care, e.g., shower, tub bath, bed bath, oral hygiene, shaving, shampooing, cleaning nails, assists to toilet, etc. as desired by a resident, directed by a specific resident's written Plan of Care, and supervised by YoloCares' clinical staff.
2. Provides homemaker services by cleaning resident's room, washing resident's personal laundry, preparing and assisting with a resident's meal, and other homemaking duties, when the resident's Plan of Care directs.
3. Changes resident's clothing and bed linen as requested.
4. Increases resident's comfort and safety by prescribed rehabilitative measures such as positioning, ambulating and assistance with transfers. Uses basic adaptive equipment with debilitated patients when the Plan of Care requires.
5. Performs routine procedures such as feeding non-compromised residents, making occupied and unoccupied beds, and cleaning resident's equipment.
6. Demonstrates and teaches care techniques to volunteers and interns when appropriate.
7. Records vital signs when requested by the Plan of Care and/or as requested by clinical staff.
8. Follows universal infectious disease precautions when delivering all resident care (gloves, handwashing, etc).
9. Assists residents in taking self-administered oral OTC and prescription medications as directed by the clinical staff.
10. Encourages simple measures for resident's bowel and bladder regularity.
11. Observes/monitors resident's physical and emotional condition and reports changes to the Joshua's House Manager and clinical staff in a timely manner.
12. In compliance with state and federal regulations and with YoloCares policies and procedures, documents all resident care, including observations of significant changes: reactions to care, symptoms observed, mental and physical status changes in the patient, and coping changes noted in residents.
13. Participates in all Joshua's House team meetings, training sessions, and evaluation processes.
14. Other duties as assigned by supervisors.

Requirements:
Must have a master's degree in social work from an accredited university.

On an Average Day You Will:
(includes, but not limited to)
Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
Fulfill the obligation of requested and/or accepted assignments
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
Complete all other duties as assigned
Requirements:
Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Must have current nursing licensure in State
Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
Must be a licensed driver with licensed automobile that is insured in accordance with
organization/state requirements and is in good working order

Will perform after hours and weekend visits and admissions as needed.
Follow up on phone calls by either directing the phone call to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or taking care of the need immediately per scope of practice
Dispatch field nurses and other appropriate staff to patient's residence in a timely manner

Requirements:
Must be an LVN/LPN with knowledge of hospice, telephone triage preferred

Must be flexible in work hours
Must show aptitude of computer data entry and the use of current software systems, such as word processing, spreadsheets and projects
Must possess the ability to deal with stressful situations, as well as grieving families

Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality
Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.)
Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned
Assist in dressing and undressing patients, as assigned
Plan and prepare nutritious meals, including shopping, as assigned
Assist in feeding the patient, as assigned
Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency
Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence
Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues
Assist in ambulation and exercise as instructed by the hospice nurse or therapist
Provide normal range of motion and transfers/positioning, as directed
Assist with self-administered medications as allowed by state regulations, such as “reminders”
Offer and assist with bedpan and urinal aid, as assigned
Aid with light laundry needs if necessary
Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency
Provide respite for patient's family/caregiver when on-site, as appropriate
Maintain patient's living area is clean and orderly, as assigned
Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
Participate in quality assessment performance improvement teams and activities
Respect patient and family/caregiver environment and patient's personal needs
All other duties as assigned
Requirements:
Must be a Certified Nursing Assistant (
applicable for all States, additional requirements for California residents, see below).
CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
Must have completed a minimum of 75 hours of classroom and supervised practical training
Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
Willingness to submit to a criminal background check prior to hire and annually thereafter
Current CPR certification
