Skills Required: Minimum of 5 years in a healthcare-related field supporting patient care, public health, or population health. At least 2 years of supervisory experience. Strong organizational, time management, and communication skills. Proficiency in Microsoft Excel for data analysis and reporting. Experience or interest in working with underserved populations, particularly in community health. Valid driver's license and auto liability insurance. Ability to perform home visits and outreach. Knowledge of community resources and services.
