Outpatient physical therapist job at Youth Advocate Programs
Status: Full Time Salary FLSA Classification: Exempt
Summary of the Position: Licensed Professional of Healing Arts (LPHA) is responsible for providing individual, family, and group counseling to youth with serious emotional disturbances and their families.
Services provided in a participants home or appropriate community setting and may include Psychological Diagnostic Interview Examinations, treatment planning, and counseling in a group, individual, or family setting.
The Licensed Professional of Healing Arts will deliver services targeting increasing the youth's natural supports and strengths, fostering resilience and recovery.
The Licensed Professional of Health Arts is responsible for accurately documenting services, attending Wraparound Team meetings as needed, and communicating participants programs to supervisors, team members, and the referring authority.
All services delivered in compliance with the Texas Medicaid Provider Procedures Manual, the Texas Resilience and Recovery Utilization Management Guidelines, YAP Standards, and all state and federal regulations governing the delivery of service.
Annual Salary: $55,000-$65,000
Qualifications/Requirements: Master's degree in one of the following human services fields required; psychology, criminal justice, sociology, social work, counseling/therapy, or behavioral health.
Active Professional Licensure in the practice of psychology, counseling, or social work required. Valid Texas Licensure (LCSW, LPC, LMFT) Required.
Experience in the provision of home and/or community based mental health services.
Foster care experience is a plus.
Play Therapy experience is a plus.
Experience in community work and knowledge of community resources.
Positions requires valid driver's license, reliable transportation, and current auto insurance coverage.
Bi-Lingual/Spanish Speaking Applicant is required.

PURPOSE:
The purpose of a Trauma Systems Therapy (TST) Therapist in Preventive Service Department is to provide the TST model for families with child(ren) (ages 5 and over) who are impacted by trauma and there may be risk for foster care placement. The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. Prevention is an essential part of the child welfare continuum and requires a commitment to the wellbeing of all members of a client family. All services are provided in a family's home and community.
POSITION OVERVIEW:
The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. They collaborate with other service providers to ensure a comprehensive approach to the family's care. The therapist also plays a crucial role in monitoring the family's progress, adjusting treatment plans as necessary, and working towards successful case closure.
The Therapist is a key figure in helping families overcome challenges, strengthen relationships, and create a safe and nurturing environment for children within the child welfare system. The Therapist provides all services in a client's home and community.
KEY ESSENTIAL FUNCTIONS:
Provide Child Welfare Preventive Services exclusively or primarily within the client's home and community
Learn and implement the TST model
Improve caregiver-child relationship as a way to restore the child's functioning
Integrate case management services with an assigned evidence base model to help families meet their basic needs to promote stability and reduce stress
Collaborate and coordinate services with all providers working with the family to ensure the family's needs are adequately being addressed
Identify and evaluate risk

CFTSS Clinical services are provided to children/youth and are intended to help identify and prevent the progression of behavioral health needs through early identification and intervention. Responsible for direct supervision to patients, indirect supervision of rehabilitation technicians. General supervision of PT assistants in accordance with State Practice Acts. Qualifications:
Education: Bachelor, Master of Science in Physical Therapy or Doctor of Physical Therapy
Licensure: Current/Temporary license of Physical Therapy in state of practice
Certifications: Must have a current American Heart Association Healthcare Provider card or complete a course within their probation period.
Preferred Experience: Six months to one year experience is preferred. Prefer experience with care for adult and geriatric patients
Skills and Abilities:
Must have general knowledge of evaluation and treatment principles for a diverse number and types of diagnoses.
Working Conditions, Mental and Physical Requirements:
Patient care may include heavy lifting and extensive bending and standing.
Must demonstrate competency in equipment used for the delivery of modalities.
Must be able to push 40 lbs., pull 60 lbs., grip 50 lbs. and pinch 15 lbs.
