If unable to attend, initiate communication with the National Clinical Director to get information and resources provided. •Provide oversight for Registered Behavior Technician on a 1:1 basis working directly with clients - in accordance with the BACB, funding, and Center guidelines, to include documentation requirements. •Maintain documentation of services in accordance with the funder's requirements utilizing the appropriate reporting formats and submitting within the timelines required. •Understands and adheres to all external accreditation review standards, applicable state, local and Federal laws and/or regulations including maintaining patient confidentiality through abiding by HIPAA laws/regulations. •Performs additional duties as assigned. Maintain an average of 3 completed visits or attempted visits/completed visits per day * Facilitate the identification of family goals and strategies to achieve identified goals as desired by families with appropriate referrals and connections to community resources * Provide appropriate referrals with each family, assisting with access to community resources and collaborating with community agencies as appropriate * Respond appropriately using crisis intervention and problem solving techniques with families as needed * Conduct child development, depression, and health screenings; monitors and supports health promotion * Utilize prenatal, child development and parent education curricula with families during home visits at a minimum of 75% of face to face visits * Assist participants in establishing and maintaining medical/dental homes to obtain perinatal care; and comprehensive health care particularly for infants and/or toddlers * Provide information, advocacy, support and access to resources for breastfeeding, family planning and interconceptional care * Maintain accurate documentation of records and adhere to data entry guidelines, including participant evaluations * Implement and maintain Healthy Start and Empowerment Projects' Best Practices and Standards * Attend monthly case management task force meetings * Participate in team meetings, supervision, case consultation, staffing, and training as required by Healthy Start and Empowerment Projects * Manage time, organization, and scheduling to meet client needs as well as other job duties and responsibilities * Use a family-centered; culturally sensitive approach to provide support and assistance to families * Utilize language interpretation/translation services and obtain pertinent information according to the culture and primary language of each pregnant or interconceptional woman, infant or toddler and their family as appropriate * Connect families to a wide-range of community resources and services responsive to the needs of families with pregnant women and/or young children * Engage in on-going personal professional development and training * Other duties as assigned by supervisor Qualifications: * High School Diploma/GED with 10 years' experience with young children, parents and/or families or an associate degree and at least 5 years full time work experience in Nursing; Sociology; Social Work; Child and Family Studies; Psychology or other human service field required. * Bachelor's degree in nursing, Sociology, Social Work, Child and Family Studies or Psychology highly preferred * Experience with home visiting preferred * Fluent written and verbal bi-lingual communication skills in English and Spanish, Vietnamese, Arabic, or Burmese preferred * Ability to accept and work with diverse populations * Ability to complete an assessment, care plan and tracking forms * Good written and oral communication skills * Ability to physically carry out various aspects of providing services in the participant/client's home in a variety of environments over which there is very limited control * Good working knowledge of community resources and accessing these resources * Complies with accepted professional standards and practice * Ability to work effectively with a multi-disciplinary team * Ability to work with the management information system and complete data entry * Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire) * Must have valid driver's license, auto liability insurance and reliable transportation if driving is required * Successful completion of pre-employment physical, drug screen, 2-step TB test and background check Working Conditions: * Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects. * Physical activities: Climbing, Balancing, Stooping, Kneeling, Crouching, Crawling, Reaching, Standing , Walking, Pushing, Pulling, Lifting, Talking, Fingering (picking, pinching, typing, etc.), Hearing, Grasping (applying pressure with fingers and palm), Repetitive motions, Feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations) * Environment: Exposure to varied temperatures, communicable diseases, dust, and stairs * Time: Ability to be flexible with a varying schedule and programmatic requirements * Travel: Required between locations These characteristics are representative of those an employee encounters while performing the essential functions of this job.
