Methodist Home for Children is seeking a full-time Youth Care Worker for its Transitional Living Home in New Bern, NC.
Salary: $15.00+ depending on education and experience.
FIRST THINGS FIRST... WHAT IS A TRANSITIONAL LIVING HOME?
These group homes serve teens who are on probation but cannot return home safely from a youth development center or other residential placement. Referred by juvenile court counselors, they stay for six to 12 months, working to finish school, identify career goals, get a job, volunteer, earn a driver's license, and learn to budget, cook and manage bills.
WHAT DO YOUTH CARE WORKERS DO?
The primary responsibility of Youth Care Worker is to provide program services to intervene with adolescents being served in the residential group home. Key responsibilities include:
Implementing direct treatment services through use of the agency's model of care.
Providing residential care services.
Working with adolescents, families, volunteers, and community representatives in the youth home and community settings.
QUALIFICATIONS:
Must be at least twenty-one (21) years of age.
Minimum of a High School Diploma and experience working with youth.
Must maintain a valid Driver's License and an acceptable driving record.
Must be able to work a flexible schedule, including evenings, weekends, holidays, and extended periods of time.

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.
Additional state specific requirements include:
NC, VA- QMHP or ability to register as a QMHP
MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.

The primary purpose of this Income Maintenance Caseworker II position is interviewing, gathering verifications and determining eligibility for applications and/or recertifications for Food & Nutrition recipients in a timely manner, in accordance with the Food & Nutrition manual, Federal and State rules and regulations. The caseworker will be conducting face-to-face and telephone interviews to accomplish this. Good customer service skills and communication skills are essential to perform this job function. The purpose of the interview is to obtain pertinent information, which is verified to determine eligibility of benefits. Thorough documentation in the case file of the information gathered during the interview is vital. This worker will make sure all information taken is in the record, keyed into NC FAST and processed in a timely manner. This worker is responsible for printing and verifying client information through the State computer system, or a third-party source, to avoid duplications of benefits and to get a better understanding of the client's situation.
This position explains program requirements and options and refers the customer to other units for assistance as appropriate.
This position is from 8:00 A.M. until 5:00 P.M., Monday through Friday. Workers may be required to stay past 5pm on occasion to finish meeting with a client, process an application or recert that is due on that date, or other duties that have a deadline that cannot be missed. Overtime may also be required at times and, if needed, is worked during the week and/or weekends.
* This worker interviews, gathers verifications and determines eligibility from applicants applying for Food & Nutrition services received via email, fax, mail-in, drop off, or submitted via epass. The worker in this position will determine eligibility for all applications using the NCFAST system.If the case is determined to be consideredexpedited service, this worker will process the application on the same day that the application is taken. Worker will process all cases within a timely manner.If during an Interview it is determined that the customer is in need of Supportive Services, Protective Services, Work First Family Assistance, Medicaid or Transportation Services a referral is made to the respective unit.
* The worker in this position will screen applications and late recertifications received via email, fax, mail-in, drop off, or submitted via epass for expedited services. Worker will be proactive in screening for expedited services to ensure application process can be disposed timely to ensure client has access to benefits by the 7th day. In normal application processing, timeframes ensure the eligibility is determined to ensure client is eating on or before the 30th day. Worker will also assist client by retrieving any information that would aid application to be processed the same day.
* The workers in this position will interview, gather verifying information and determine eligibility for Food and Nutrition recertifications received via email, fax, mail-in, drop off or submitted via epass.Re-certification will be based on program guidelines.
* This worker is responsible for timely completion of system generated tasks, changed decisions, county transfers, and issuing food referrals. Ability to react timely to changes reported by the household to reduce potential fraud and/or agency errors. Second party reviews are completed often and corrections that ensue are to be completed by the 2nd business day. Ability to retrieve state reports to give account for all pending applications and/or recertifications.
* In cases that lead to Court involvement, the worker would prepare the preliminary summary to be used by the Fraud Investigator to prepare the case for court. The worker in this position would be expected to testify as to the validity of the information taken at the time of the interview. This worker is also responsible for preparing the summaries and presenting them for the appellants in local and state appeals. If the recertification has been completed, then the on-going worker would take case to court. Submission of program integrity referral when and if suspected fraud is detected is also required of this worker.
* Employee will be expected to work in emergency shelters and/or perform related disaster tasks and perform other reasonable tasks requested by the immediate supervisor, program manager, deputy director or director.
* Other Work Characteristics
* Calculators, copiers, personal computer, telephone, fax machine, Audio visual equipment and other office equipment will be used in the performance of the duties of this position. Worker in this position must have access to the Eligibility Information system, NC FAST, and inquiry access to Nash County Clerk of Court, Child Support System, Department of Motor Vehicles, Social Security and Employment Security System. Included in materials used in work are case management reports, pending application reports, EIS Manuals, NC FAST, Food & Nutrition Manual, Integrated Manual and Administrative letters as well as other verbal and written instructions.
* The responsibility of this caseworker position must be accurate and exact as well as timely to avoid delay in benefits and ineligible participation.
* Close visual attention is needed in every phase of the job since reading, writing and using machines make up the duties. The computer screens are very hard on the eyes. Intense mental concentration must be maintained not to make errors, which could have a negative impact on the applicants/recipients and the agency. NC FAST is referred to daily as the additional responsibility of entering data into the computer system in order to generate or adjust benefits requires precise attention to detail and accuracy.

3. Assist with the on-going observation of the patient's physical and mental health and home environment by: (a) gathers and documents specific data according to the patient's individualized plan of care and skilled nursing report, (b) recognizes existing relations between data gathered and patient's health status and treatment plan, (c) determines a patient's need for immediate nursing intervention based upon data gathered and in compliance with parameters on the Plan of Care.
4. Documents in the medical record electronically and downloads patient information in NetSmart periodically throughout the day: (a) those services rendered to the patient; (b) clinical and progress notes, conferences held with the physician and other disciplines, physician, or community resources; (c) changes noted in the patient's condition and/or family home situation; (d) revisions in the care plan with supervision by the RN (e) and submits downloads all paperwork including time and travel daily (f) Updates medication profile with each medicinal change. (g) Keeps up-to-date on reimbursement criteria, hospice criteria, PPS, and charting needs for all patients under care including private authorization. Responds within 48 hours to requests for documentation correction/completion.
5. Collaborates with supervisor to meet agency's clinical needs. Rotates weekend, after hours, and holiday on-call: with an RN on back up call. Assists with coverage of visits in other offices as assigned.
6. Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basis following chain of command. Communicates information using current process and technology available to 3HC.
7. Stays abreast of overall patient care and improves nursing care skills: (a) participates in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences/ IDG at least 75% of the time; (c) attends agency mandatory in-services and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings
9. Military and Family Life Counselor - Assignment Ready Counselor

These positions provide short-term support to the MFLC program either working on a base with military members and their families or in local schools with high populations of military families attending. Candidates must be local to either Camp Lejeune, Camp Geiger, MCAS New River, MCAS Cherry Point, Fort Bragg, or Seymour Johnson AFB or be willing to relocate to become local. Candidates must be licensed at the independent level as a Social Worker (LCSW), Therapist (LMFT), or Mental Health Counselor (LPC/LCHMC).
The Assignment Ready Counselor (ARC) will provide coverage in short-term, surge and on demand situations resulting from the Military needs such as a post-deployment event or to cover an existing assignment for counselors who are absent. The Summer ARC counselor position covers assignments during the May 15th through September 15th time period, coinciding with the school summer break. With minimal guidance and oversight, provides the full breadth of Adult or Child and Youth Behavioral (CYB) counseling services to military service members and their families at military installations. These services may include non- medical counseling, training/health and wellness presentations, consultation with parents, personnel at child daycare centers and schools, and consultation to installation command regarding behavioral health issues affecting military personnel and their family members. ARC CYB counselors may be assigned to DoDEA schools and community schools and will need to have experience working in a school setting to qualify for ARC role in backing up a school position. CYB counselors may also be assigned to youth programs, summer camps, and on demand assignments. ARC adult counselors may be assigned to support MFLC services in a variety of military community and readiness centers, as well as on-demand and surge assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation.
* Provides non-medical, short term, solution focused, counseling directly to adults, children, and youth of service members. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
* CYB counselors will also have a focus on supporting the staff and personnel of CDCs, DoDEA and community schools, youth programs and summer camps as well as providing parent training and guidance.
* Provides training and health and wellness presentations, participate in health fairs and other base/installation activities.
* Enters counselor activity data daily through smart phone or web application assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service / family member.

Stays abreast of overall patient care and improves nursing care skills: (a) participates in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences/ IDG at least 75% of the time; (c) attends agency mandatory in-services and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings
9. Demonstrates an interest in personal and professional growth for self and staff: (a) attends and participates in workshops, seminars, webcasts to keep abreast of current changes in rules, regulations, relating to job functions and department; (b)does individual reading and research; and (c) recommends educational opportunities for staff and encourages participation.
10. Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
11. Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.
Requirements:
Licensed Practical Nurse currently licensed to practice nursing in the state of North Carolina.
Minimum of two-years of experience as a nurse in an acute care setting preferred.
Maintain current CPR certification (BCLS)
Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized a plan of care.
Possess valid Driver's License and reliable transportation.
