The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Monitor and document the status of the consumer's progress.
Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects.
Qualifications
Minimum of a Bachelor's Degree in human services or related field is required.
Two years' experience working with youth or families.
Current Family Centered Treatment Certification is preferred.
Additional state specific requirements include:
NC, VA- QMHP or ability to register as a QMHP
MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.

ALL ABOUT ON DEMAND
You can work as much or as little as you like as an On-Demand Team Member (TM) and your schedule may vary depending on shift availability and store needs. This role is ideal for those that are looking for a personalized work schedule.
As an On-Demand TM, you will not be included on the posted weekly schedule, but rather will have the opportunity to create your own schedule by picking up shifts posted by leaders or other team members (via our my Time mobile scheduling app) that work best with your schedule.
When you choose to pick up a shift, reliable and prompt attendance is necessary.
We also ask for your active engagement by picking up and working shifts once every 4 weeks (minimum of four hours). Flexibility can be granted in certain circumstances, but you need to respond to our attempts to contact you to confirm your interest in working shifts at Target. If you do not respond to our attempts at contact, your employment may be terminated. Additionally, if you do not work at least one shift within 5 months, your employment with Target will be administratively terminated.
Your communication and ability to work when our business demands it most are critical to your success in this role.
Should you be offered a position as an On-Demand TM, you will be required to attend a Target Welcome orientation and commit to a short-term structured training schedule to ensure you are properly prepared for your new role. After that, the my Time mobile scheduling app is where you can pick up the shifts you desire to work.

As a Guest Advocate, no two days are ever the same, but a typical day will most likely include the following responsibilities:
* Demonstrate a service culture that prioritizes the guest service experience by delivering the service standard.
* Create a welcoming experience by greeting guests as you are completing your daily tasks.
* When guests need assistance, engage with guests in a welcoming way, to help solve their specific needs.
* Be knowledgeable about the tools, products, and services available in the total store, and specific to your area, to solve issues for the guest and improve their experience.
* Thank guests and let them know we're happy they chose to shop at Target.
* Scan and bag all guest items efficiently, accurately and in compliance with food safety standards and company best practices.
* Work efficiently to minimize guest wait time while maintaining guest service and accuracy.
* Make the guest aware of current and upcoming brand launches, store activities and events.
* Know and speak to the benefits of Target Loyalty programs with every guest and assist with application or sign-up as needed. Be familiar with all fulfillment services, and know how to direct the guest to enroll, activate and use them.
* Understand and show guests how to use Wallet and the other features and offerings within the Target App.
* Attempt every return and follow register prompts, partnering with immediate leaders as needed to help solve for the guest while following Target's policies and procedures.
* Partner with leaders as needed to de-escalate any situations and recover the guest shopping experience while following Target's policies and procedures.
* Deliver easy and accurate service to all Order Pick Up, Drive Up, and Registry guests.
* Maintain a clean, clutter free work area (including gathering abandoned items, baskets, and hangers).
* Stock supplies during store open hours while being available for the guest.
* Demonstrate a culture of ethical conduct, safety and compliance.
* Work in a safe manner at all times; comply with all safety policies, best practices, and training; report hazards and correct where possible.
* Support guest services such as order pick up (OPU), Drive-up (DU) Orders, DU Returns and maintain a compliance culture while executing those duties, such as compliance with federal, state, and local adult beverage laws.
* Support Cash Office processes as needed, including management of cash systems.
* All other duties based on business needs

* Accurately handle cash register operations, cash transactions, and support cash office operations as needed
* Scan, handle and move merchandise efficiently and safely, including frequently lifting or moving product up to 10 pounds and occasionally lifting or moving merchandise up to 44 pounds without additional assistance from others.
* Flexibility when picking up shifts (e.g., nights, weekends and holidays) to help meet store needs
* Active engagement by picking up and working shifts as well as responding to our attempts at contact. You must work at least one shift within 5 months or you will be administratively terminated.
* Capable of working in and exposure to varying temperatures, humidity, and other elements while performing certain job duties including but not limited to Drive-Up, carryout, etc.
* Ability to remain mobile for the duration of a scheduled shift (shift length may vary).

TYPICAL WORKING CONDITIONS
Work involves the supervision of children age 8 weeks to 12 years old. Employee must have the ability to provide a safe, constructive environment for children while parent/guardian is participating in activities elsewhere in the facility.
PRINCIPAL DUTIES
Communicate and promote the YMCA mission, goals and objectives to children, parents and community at large.
Provides excellent service to members, guests, and program participants.
Supervise children in the day-to-day operations of the Y-Tikes and Y-Zone center, which includes the changing of diapers, wiping noses, playing with children, offering a drink or snack.
Provide for and lead children in structured activities.
Adhere to policies related to boundaries with children/youth.
Report suspicious or inappropriate behaviors and policy violations.
Follow mandated abuse reporting requirements.
Help with special event programs.
Communicate supply and maintenance need to Y-Tikes and Y-Zone Center.
Communicate with Childwatch Coordinator any concerns.
Communicate with the front desk to ensure smooth flow of information concerning programs, holiday hours, special events and facility rentals.
Ensure that all areas are clean and safe at all times.
Ensure that all safety inspection reports are complete and accurate.
Ensure that all paper work is completely filled out and properly filed.
Provide a quality experience for children and families.
Open the child watch center on time each shift. This requires that child watch staff will be at their duty station 10 minutes before each shift begins.
Ensure that an accurate sign in/out log and file log sheets will be completed at the end of each shift.
Responsible for any other duties as assigned by the Youth and Family Services Director.
Qualifications
EXPERIENCE/PERFORMANCE REQUIREMENTS (Knowledge, Skills, and Abilities)

What you'll be doing:
Provide outpatient case management services to the seriously mentally ill population.
Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about continuum of care.
Provide training on money management as it relates to meeting needs of the client to obtain and maintain psychiatric stability residential setting.
Provide follow-up progress as per need of client to treatment team members.
Evaluate client's residential stability and assist those clients who are in need of more stable environment.
Provide Basic Living Skills training when necessary as it relates to clients immediate needs while preparing client for less intensive level of care within the community.

Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness.
