Job Summary: The Residential Counselor is responsible for providing direct clinical services to individuals who have been affected by chemical dependency, psychiatric problems and/or co-occurring disorders. Functions as a case manager for patient caseload. Maintains clinical record including biopsychosocial, SNAP and BAM assessments, treatment plans, clinical summaries, progress notes, case consult notes, discharge summaries, and continuing care plans. Ensures clinical record is compliant with all licensing, regulatory and accrediting agency requirements. Completes continued stay reviews using ASAM criteria. Coordinates services including aftercare planning and makes referral recommendations when necessary. Maintains regular communication with referral sources, legal systems, and outside providers. Responds to and addresses unusual or emergency situations. Encourages patients to understand the benefits of recommended level of care. Operates as part of multi-disciplinary treatment team. Assesses healthcare equity for current and future patient needs. Conducts educational seminars and lectures to patient population in a professional and competent manner. Facilitates individual, group and family counseling sessions. Assists with the collection of random urine drug screen samples from patients as needed. Participates in clinical supervision, department meetings and trainings as required. Protects the confidentiality of patient information. Performs other related duties as assigned.
