Coach parents on self-advocacy skills, ensuring their voices and preferences are heard within the care team. * Resource Connection: Identify and connect families with relevant community resources, support groups, and financial/insurance information. Develop and maintain a directory of current, vetted resources. * Education & Information: Help re-state clinical information into everyday language for families and assist them in understanding their child's diagnosis and treatment plan, without providing medical advice. * Support Group Facilitation: Coordinate and facilitate support groups for parents and caregivers, offering a safe space for shared experiences and mutual support. * Collaboration with Care Team: Function as an integral member of the interdisciplinary team (physicians, nurses, social workers, child life specialists), communicating family-identified needs and ensuring a family-centered care approach. * Crisis Planning Support: Assist families with crisis and safety planning by leveraging personal experience with stressful situations and hospitalization. * Documentation: Maintain accurate and confidential records of interactions and services provided, adhering to HIPAA and other regulatory requirements. Minimum Qualifications/Work Experience: 1+ year social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience. *Billingual skills may be required depending on assignment and patient population. Preferred: Lived experience as a parent of a child with Congenital Heart Disease (CHD) or a child who was in the NICU, along with experience navigating the California High-Risk Infant Follow-up (HRIF) Program and Regional Center services. This position is for our DMC funded, residential substance abuse treatment program. Key Responsibilities Monitors client's progress toward achieving treatment plan goals and provides treatment plan input. Utilizes trauma informed care while applying motivational interviewing, cognitive behavioral therapy, and evidence-based practices. Works collaboratively with other members of the treatment team including the Case Manager, Clinical Therapist, Medical, and Psychiatrist. Maintains frequent communication with the treatment team and engages in regular consults, clinical team, case conferences, and plan development meetings. Delivers services in outpatient treatment setting. Responsible for treatment oversight of the clients on their caseload. Maintains compliance with due dates for all clinical documentation. Writes client progress letters and court reports and obtains final approval from the supervisor. Documentation must maintain in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards.
