Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay. Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC's Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown. The Case Manager I is responsible for assisting clients being served in one of PHC's social service programs (those who are living with HIV/AIDS) through needs assessment, case management, and advocacy with community partners. The Case Manager I provides services according to grant guidelines. Contact may occur in non-traditional settings. Demonstrates iCare values in daily work. What You Will Be Doing * Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services. May assist clients with application process for entitlement programs. * Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures. * Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies, including emergency and crisis services. * Advocates for clients with community agencies, schools, significant others and other appropriate agencies. * Ensures clients have access to transportation to necessary appointments as appropriate, assisting with client transportation in accordance with PHC transportation policy. * Communicates with other agencies on behalf of clients using professional discretion with client information, ensuring that appropriate releases of information are obtained. * Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as a liaison between PHC and community resources. * Performs other duties as assigned. Qualifications You Will Bring * Bachelor's degree in psychology, counseling, mental health, human services or other related field or equivalent combination of education experience. * Excellent customer service skills. * Effective written and verbal communication skills. * Team oriented. * Proficiency using Microsoft Office applications including Excel and other database applications. * Excellent interpersonal skills with ability to establishes and maintain effective working relationships with a diverse group of individuals. * Detail oriented with high degree of accuracy. * Demonstrates professionalism with ability to maintain composure in all situations and show empathy. * Problem solving skills. * Organization and prioritization skills. * Licenses & Certifications: Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the State of Iowa; Active Chauffer's license from Iowa Department of Transportation. Both must be obtained within introductory period if not current. PREFERRED QUALIFICATIONS AND SKILLS * Six months of work or volunteer experience in human services related activities. (Will consider experience acquired while obtaining the degree.) * Work experience with vulnerable populations, including people experiencing homelessness. * Demonstrated competency using HMIS or CareWare as applicable to department. * Working knowledge of community resources related to homelessness or HIV/AIDS in Des Moines. * Knowledge of program terminology. * Ability to effectively use HMIS or CareWare system to document all client contact. * Demonstrate language proficiency in Spanish. The hiring range for this position is typically $17.00 - 21.25., although highly qualified candidates could earn up to $25.50.
