Our Brands Responsibilities: The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model. Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements. FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA). Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system. Deliver FCT services to children, adolescents, and their families, primarily in their living environments, with a family focus. Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule. Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards. Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision. Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan. Provide psychoeducation as indicated in the Service Plan. Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents. Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals. Make referrals to services and resources on behalf of the families and children. Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs. Coordinate and oversee the initial and ongoing assessment activities. Convene the Treatment Team for Service Plan. Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process. Ensure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations. Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members. Monitor and document the status of the consumer's progress. Participate in Quality Assurance/Quality Improvement activities to include audits, corrections and designated improvement plans and projects. Qualifications: Minimum of a Bachelor's Degree in human services or related field is required. Two years' experience working with youth or families. Current Family Centered Treatment Certification is preferred. Additional state specific requirements include: NC, VA- QMHP or ability to register as a QMHP MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi. Responsibilities will include responding to emergency and non-emergency calls with police officers to engage clients that are experiencing mental/behavioral health crisis, meeting clients in the community, assessing clients for safety, determining appropriate level of care, and providing clinical or non-clinical treatment. Work also involves a follow-up component to connect identified individuals with available community resources. Work is performed under the regular supervision of the Community Division Commander. Essential Tasks & Responsibilities: Respond to referral requests from the police department or other designated partners. * Respond within 30 minutes to calls for service where an alternate response team is requested. Depending on the call type and safety assessment, response may be independent or co-response with a police officer. * Determine the type of response needed including field-based crisis intervention, phone counseling, information, follow-up, and/or referral. * Engage with clients to develop rapport and gain cooperation for the completion of assessments. * Conduct clinical assessments to determine the appropriate safety plan and level of service referral interventions needed by collecting information from clients, providers, supports, and advocates. * Provide evidence based therapeutic interventions as indicated on the assessment (clinical or non-clinical). * Conduct limited case management services for high service utilizers as directed by the Lead Clinician. * Establish and maintain healthy work relationships with community partners and resources. * Participate in an after-hours on-call schedule. * Provide accurate documentation within allocated time frames. * Operate standard office equipment including calculator, copier, personal computer, printer, and related software. * Enter and edit data in databases, spreadsheets or word processing software as needed. * Utilizes specialized software to ensure HIPAA compliance and maintain case management notes. * Work outside the office and within the community as needed. * Other duties as assigned. Qualifications: Required Qualifications * Master's degree in mental health counseling, social work or human related fields. The FSC assists families in identifying and reaching their own goals. You'll have direct responsibility for tasks associated with the Eligibility, Recruitment, Selections, Enrollment and Attendance (ERSEA) requirements of the Head Start Program Performance Standards, including the proper determination of family eligibility, ongoing recruitment and selection of families, preservation of full enrollment and support for regular attendance of children. As a front line representative of Save the Children, the FSC is required to ensure the safety and security of Head Start children and families with whom he/she has contact, and adhere to the agency's values of Accountability, Ambition, Collaboration, Creativity and Integrity. Support each family in developing a Family Partnership Agreement so the family is able to understand and use goal development as a valuable life Have regular home visits with families to build on competence in the areas of: understanding attachment and child development, self and family advocacy, developmental transitions, parents as a child's first teacher, life management skills and family Make and consistently follow up on referrals related to: Family Partnership Agreement goals, strengths/needs assessment items, children with special needs, family health, job and education services and other services related to wellness and family Strengthen community collaborations to support families by: maintaining current knowledge of community resources, sharing community service information with families and building relationships during attendance of interagency meetings, community events and other scheduled meetings with community Promote a unified approach to sharing child information with families by engaging in joint planning with teachers and other specialists to prepare for home visits and other family Participate in multi-disciplinary team Meet program documentation requirements by maintaining accurate, objective, complete, timely and well-organized child and family records, both electronic and hard Actively participate in opportunities for continuous professional development Required Qualifications Associate's degree in family and child development, early childhood development, social work or adult learning; or within eighteen months of hire, at a minimum, a credential or certification in social work, human services, family services, counseling or a related Professional proficiency in MS Office suite Professional proficiency in written and spoken English. Demonstrated knowledge of administration, budgeting, purchasing systems management, and public relations skills. Demonstrated experience developing consistent, stable and supportive relationships with young children. Proven ability to exercise professional judgment and evaluation before making decisions. Proven ability to establish and maintain effective working relationships with staff, children, parents and outside agencies. Demonstrated successful ability to communicate and collaborate with individuals and teams at all levels-both internal and external Proven successful problem solving and time management skills. Preferred Qualifications Bilingual preferred (English/Spanish or English) Additional Qualifications Physical Requirements related to essential functions: Ability to perform medium to heavy physical work exerting up to 50 pounds of force occasionally, up to 20 pounds frequently and up to 5-10 pounds constantly, performing such activities as sitting, crawling, stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting and grasping. This also includes some duties that require repetitive action and motion skills, keyboarding and computer use. Clear pre-employment background check requirements, including local, state, and national criminal records check, sex offender registries, employment, and education verifications (including child abuse registry check if required by state regulations) and pass all state and local health requirements required post job offer and prior to employment. This position will require the incumbent to operate a motor vehicle to fulfill certain essential functions of the job. The incumbent should maintain a valid driver's license and remain in compliance with any Save the Children policies pertaining to the safe operation of motor vehicles. Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process. Individual and group supportive counseling, monitoring of symptoms and medications, linking the individual supported to the appropriate services, agencies and supports in the community, monitoring their development of appropriate behaviors in his/her home and other community settings, teaching the individual supported skills necessary for independent community living and facilitating them in the practice and transfer of these skills to his/her environment, providing support to his/her family. • Assume responsibility for writing, implementing, evaluating, and revising Person Centered Plan and overall treatment goals and service plans that are based on evaluations and recommendations from assessments by various professionals and input from the individual supported. • Function as a member of a multi-disciplinary team to develop and implement treatment goals. • Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community and assess individual needs on a daily basis. • Coordinate services with the program, staff, inpatient facilities, and detoxification facilities and establish linkage to residential programs, self-help programs and community resources. • Review treatment progress and monitor and provide continuous consultation with other Assertive Community Treatment Team staff and community agencies to ensure the consistent therapeutic management of the individual's needs. • Act as liaison with the program after hours staff on a daily basis through exchange of participant information; briefly detailing the presenting problem and after hours plans for the individuals being served by the Assertive Community Treatment Team. • Meet regularly with the other Assertive Community Treatment Team members to formulate and disseminate information and be available by telephone and/or pager. Meet regularly to review the total caseload, problem-solve, develop plans, and determine daily duty assignments. • Complete an intake assessment including: medical history, general physical condition, alcohol and drug histories and current use, educational issues, family structure and relationships, vocational history and goals, legal history and issues, psychiatric history (including, onset, course, effect of illness, treatment history, medication history risk behaviors). • Ensure immediate and appropriate interventions are provided in response to changes in mental status which may put individual at risk. • Documents participant activities consistent with the service records manual. Write an event note that reflects participant activities, goals, interventions, and outcomes. • Developing service plans and/or person-centered plans as appropriate, and screens referrals in coordination with the other Assertive Community Treatment Team members. • Attend and actively participate in meetings and training as required.
