Summary
The Social Worker position is full-time and works out of the Chico CA office. This position serves the agency by supervising and coordinating children placed in certified foster homes, recruiting, evaluating and certifying potential foster homes, provide and arrange training for foster parents and liaison with other agencies. The position will be working with post adoptive families that are in need of resources, case management, and planning and facilitating groups and events. This position provides coordination and case work with the Adoptions, THP, THP Plus FCS, and THP Plus populations.
Essential Duties and Responsibilities
Provide support and services to ensure stability of client placement in foster homes
Monitor foster homes for compliance with Title 22 regulations
Provide support and services to ensure stability of youth and families in adoptive homes
Plan and facilitate family events and groups along with monthly respite nights
Maintain timely progress, quarterly and/or court ordered reports
Screen and evaluate intake admissions to appropriately match children in emergency placements
Experience and/or Education
Master's in Social Work (Preferred) or related field, a Bachelor's at minimum is necessary.
California driver license and clean driving record
Experience working with at-risk children and families
Understanding of the child welfare system
Knowledge of group processes and the ability to apply
Knowledge of 24-hour crisis intervention
The estimated base pay for the Social Worker position is $22.00 to $38.00 an hour; additional individual compensation may be available. Base pay is only a portion of the total rewards package, and a comprehensive benefit program is available for qualifying position.

Summary
The Community Supports Housing Transition Navigator works as a member of the 6th Street Center for Youth team. The position involves providing 6th Street youth with assistance to promote independence and well-being through connection to resources and advocating for their needs related to housing. This support includes individual and in-group sessions to assist in overcoming barriers for finding and sustaining permanent and stable housing.
Essential Duties and Responsibilities
Conduct tenant screening and housing assessment to identify client preferences and barriers to housing
Develop individualized Housing Support Plans based on assessment.
Develop client's short- and long-term goals related to housing stability and search for housing opportunities in the community.
Assist clients to secure housing, including applications and other documents.
Assist with benefits advocacy and securing required documentation (e.g., social security, birth certificates, prior rental history)
Identify and assist with rental subsidy programs (e.g., TBRA, HUD Section 8)
Identify and assist with other financial resources related to housing sustainability.
Engage and develop working relationships with landlords.
Assess living environments for safety.
Develop a Housing Support Crisis Plan with clients.
Identify and coordinate client transportation needs.
Education and/or Experience
Bachelor's Degree in public health, human, social welfare or related field; or equivalent experience, training, or coursework.
Minimum one year of progressively responsible and direct work experience working with the essential duties and responsibilities described above.
Strong understanding of community resources and services
Excellent communication and interpersonal skills
Ability to work independently as an as part of a team
Experience working with diverse populations including homeless individuals.
Strong organizational and time-management skills.

What you will be doing in this role:
Under the supervision of the Chief Radiation Therapist and the professional direction of a Radiation Oncologist, the Radiation Therapist job is expected to administer radiation treatments to cancer patients; Verification of treatment plans & Quality Checklist; Documentation of treatment, daily and weekly linear accelerator QA, portal images, and CBCT; Coordinates and schedules patients in Mosaiq system; Notifies appropriate person of EPIC scheduling changes; Follows physicians prescription and carries out all instructions; Verifies mathematical accuracy of treatment prescription; Detects equipment malfunction, troubleshoots and reports problem to appropriate person; Demonstrates appropriate medical knowledge when caring for patients with urgent/emergent patients; Demonstrates effective time management; proactively assists other team members to meet teamwork goals. Performs some special treatment procedures, such as Stereotactic Radiosurgery (SRS), Stereotactic Radiation Therapy (SRT), Stereotactic Body Radiation Therapy (SBRT), Brachytherapy.
Performing weekly portal images, and daily CBCT, KV, VisionRT prior to delivering treatment.
Accurately completing and charting all simulation and treatment summaries, report and documentations, along with photographs of set up and tattooing of patient as required

Patient management and education
Documenting daily pain assessment
Verifying correct side/site; Patient name; DOB prior to treatment

What You'll Do:
Handle incoming and outgoing non-professional level phone inquiries and transfer calls to appropriate staff.
Update patient demographic information in the Electronic Medical Record (EMR).

Department/Unit Summary
The Critical Care Supervisor works as an advocate for the physical and emotional wellbeing of both the patient and nursing staff on the Intensive Care Unit. Position Summary:
As a senior member of the Diagnostic Imaging Department, this position produces clinical diagnostic radiographic images. What You'll Do:
Conduct nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions.

Starship Explorer is a 36-bed, acute care unit with a patient population comprised primarily of surgical patients. Explorer has central monitoring capabilities and 4 rooms with negative pressure capabilities. Explorer utilizes Valley Children's Healthcare L.E.G.A.C.Y. Nursing Care Delivery Model which embraces a philosophy of professional practice that supports an environment of excellence in patient care. Position Summary:
As a senior member of the Diagnostic Imaging Department, this position produces clinical diagnostic radiographic images. Operates diagnostic imaging equipment, prepares room, equipment, supplies and medications. Provides leadership to Rad Tech II technologists or other staff as is needed. Provides patient care services using general and specialized imaging modalities under the direction of a physician but not requiring constant technical supervision. Provides diagnostic imaging services to patients of all ages. Applies ionizing radiation in the performance of a variety of technical procedures for radiological diagnosis. Responsible for patient care safety. Assists physicians in the performance of interventional procedures and the administration of contrast media. Assumes responsibility for designated procedures and equipment. Participates in orientation of Radiology Department staff. Works cooperatively with members of the health care team to maintain standards for professional Diagnostic Imaging Technologist practice in the clinical setting.

What You'll Do:

Summary:
Peer Support Specialists in the Sutter-Yuba Behavioral Health Transition Age Youth (TAY) program actively participate in an interdisciplinary treatment team of behavioral health professionals supporting the mental health well-being of persons aged 16-25 years old. Peer Support Specialists use their own lived experience in recovery and receiving behavioral health services, or as a close family member of someone who has, to support, inspire, and model to others that recovery from mental health challenges and/or substance abuse does happen.
