Our Company: Community Mental Health Center, Inc. is a provider of comprehensive mental health services, offering inpatient, outpatient, home-based, school, and community-based programs to individuals and families in Dearborn, Franklin, Ohio, Ripley and Switzerland counties in Southeastern Indiana. Position Responsibilities: The Wraparound Facilitator is responsible for assessing the appropriateness of the referral to the program; to engage a youth and family in the wraparound process; to engage, develop, and prepare a team; to create a crisis plan and stabilize crises; to explore strengths, needs, culture, and vision with the family; to establish an integrated care plan that is determined by the child and family driven by underlying needs that have been prioritized by the team, and grounded in a strength’s perspective; monitor progress toward those goals and objectives established; ensure the team effectiveness by collaborating with team members to stay on track with the assigned action steps and by inspiring them to work as a cohesive group toward the family vision; to create a transition plan including post-transition crisis management; and to celebrate successes.

Qualifications: Bachelor Degree in social work, counseling or related field; a minimum of three years of experience, excellent organizational, and communication skills, commitment to wraparound principles (Family voice and choice, Strength-Based, Individualized, Outcome Based, Team Based, Culturally Competent, Community Based, Unconditional Care, Collaboration, and Natural Supports).

Day-to-Day Expectations:

• Consultation and collaboration with community systems to facilitate linkage-referral, and advocacy ensuring caregivers and youth have access to the services and resources needed to reach goals and objectives supporting the family vision.
• Teach youth and families the skills needed to facilitate their family team toward reaching their vision.
• Teach youth and families how to identify underlying needs and mitigating strengths to facilitate the development of an individualized plan that supports the family vision.
• Monitor progress toward meeting established goals, and adherence to action plans.
• Assists in the development of natural supports and non-traditional community resources needed for youth and their families to reach their vision.

Salary Range: $12.56 - $18.70 hourly

Youth Enrichment and the Rodarte Community Center promotes a positive presence in our community. You will be working with youth of all ages in our Fun in the Sun summer program. Essential Functions:

• Utilize various technology platforms in Microsoft and Brightwheel
• Be able to handle stressful situations
• Ability to follow all COVID-19 guidlines
• Ability to work mornings and evenings
• Ensures the safety of all team members and guests at the facility.
• Supervises, guides, and assists participants during art/crafts, recreational activities, and other activities.
• Assists Youth Service Specialist Leads in implementing programs.
• Executes instructional plans using accepted teaching methods, coaching methods, and individual instruction.
• Effectively communicates with participants, parents/guardians, and public effectively and appropriately.
• Sets up and takes down necessary equipment prior to and after programs, which includes cleaning and organizing.
• Assists in the organization and administration of special events.
• Performs other related duties as assigned or requested.

Working Environment and Physical Requirements:

• Swimming will be require.
• Multiple hours outside in the heat.
• Physical activity with youth.
• Is able to stand for long periods of time.
• Can be outside and active for long periods of time.
• Can lift 30-50 pounds.
• Has the ability to participate in activities with participants.
• Can swim 50 meters or more.

*GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES: *

The Service Specialist has two primary roles; one as Resource Specialist connecting with service providers and maintaining a list of all community resources from each town in the catchment area and the second as Intervention Specialist facilitating cognitive-behavioral and Life Skills groups to youth. The Service Specialist works closely with the Family Advocate in helping to identify what is available in the youth's and family's community in order to make a meaningful connection.

*MINIMUM QUALIFICATIONS/REQUIREMENTS: *
* Associate Degree in Criminal Justice, Community Organizing, Human Services, Sociology OR at least 3 years' experience and knowledge working with juvenile justice system and/or working with communities
* Bilingual (English/Spanish) a plus
