The American Lung Association has an excellent opportunity for a Specialist, Health Promotions. Working as a member of the Health Promotion & Education department, you will join an outstanding group of professionals dedicated to our mission: to save lives by improving lung health and preventing lung disease through research, education, and advocacy.
The person in this role will support implementation and evaluation of lung health programs throughout assigned market. Complete deliverables as provided by funder, utilize Lung Association resources and programs that elevate outcomes, and work with grant team to meet deliverables on time and within budget. This is a 100% grant-funded position.
Location: The position can be located anywhere in Arizona and will be a hybrid of in-person and virtual work.
Responsibilities:
Work within a multi-faceted team to meet deliverables of newly awarded Community Innovations grant.
Support the build-out of new coalitions and contribute to the efforts and outcomes of the work.
Assist in the build-out of the new Arizona Community Tobacco Council, prepare for council meetings and follow-up on their next steps.
Identify potential external community partners that can elevate the work
Prepare and organize convenings such as cohorts, meetings, education sessions, community education activities etc. as appointed by Manager.
Research and identify resources that pertain to tobacco, existing smoke-free and commercial tobacco-free environments, tobacco policies, coalition management, and more to share out to council members, stakeholders, and partners.
Promote cessation services and resources
Support distribution and evaluation of community assessments. Contribute to next steps according to assessment outcomes.
Contribute to required American Lung Association and funder reports.
Work well within a team setting, collaborate and support team members.
Qualifications:
Bachelor's degree in Public Health, Health Promotion, or related field required with a minimum of two years of programing/health education experience.
Two years of similar non-profit programming experience highly preferred
Prior experience in Community outreach, tobacco education, and/or coalition management
Must be a self-starter with excellent communication skills both written and oral
Ability to multitask in a fast-paced environment
Positive attitude with the ability to work independently and in a team environment
Ability to lift approximately 25 lbs
Able to work with minimum direct supervision, make decisions, and take initiative
Proven ability to cultivate and steward relationships across a diverse population
Strong verbal and written communication skills
Strong computer skills with knowledge of Microsoft Office Suite
Must have a valid Driver's license and your own reliable transportation with the ability to travel within assigned area 10% of the time for meetings and conferences, as well as the flexibility to work irregular hours, including evenings and weekends with some overnights required.
Required to travel statewide to provide technical assistance, meetings and conferences as required by assigned grants.

Compensation: Exact compensation may vary based on skills, experience, and location. The target hiring range for this position is between $45,500 and $51,500 per annum.

The Firewood Outreach Conservation Information Manager will work with the Forest Pest and Pathogen team to slow the spread of damaging forest pests and pathogens in the US with a focus in the Pacific Northwest (PNW) through support of TNC's longstanding outreach campaign, āDon't Move Firewoodā. They will communicate and engage with governmental agencies, Tribal Nations, academic institutions, non-profits, and private entities. They will conduct work in relation to some or all of the following: online informational pages, campground reservation systems, outreach resource library, social media presence, supporting technical documentation, and layperson science communications.
Your Position with TNC:

Job Summary: The Community Health Worker (CHW) is a peer who has experience in navigating Rhode Island's health system for themselves, a family member or through previous employment. This CHW will work in RIPIN's Hospital Care Transition Program, which supports Rhode Islanders who may be good candidates to discharge from the hospital back to their homes or other community settings but need a little extra assistance to make that possible. The CHW will be a critical part of a comprehensive team providing options counseling, resources and referrals for post-hospital care. CHWs will engage with consumers in hospital settings providing person centered, culturally sensitive support, and building on the values, strengths and preferences of the patient. The CHW will also serve as an effective role model and mentor.
Essential Functions:
⢠Assist patients and families in understanding and accessing informal and formal options for post-discharge care benefits including copay and cost of care.
⢠Review and educate on benefits and eligibility for Medicaid Fee-For-Service, Medicaid/Medicare Managed Care, Medicare Advantage Plans, and any available private insurances.
⢠Assist the consumer in completion and submission of enrollment or benefit applications. Refer consumers to other services and public or private agencies for additional supports as needed.
⢠Utilizing motivational interviewing skills and culturally sensitive methods to collaborate with patients to explore preferred post-discharge supports and identify social determinants of health and/or areas of need within their community environment.
⢠Review care options including natural supports, home care services, medical equipment, adult day health programs, senior centers and assisted living communities.
⢠Coordinate with hospital discharge and health plan staff to enable post-discharge home and community supports to be established in a timely manner.
⢠Assist consumers as they transition to independence/case closure by engaging with consumers and providing follow up support.

Literations is an affiliate of AARP Foundation Experience Corps, a national leader in engaging older adult volunteer coaches to improve grade 1-4 student literacy.
GENERAL STATEMENT OF POSITION
The Outreach Coordinator, under the supervision of the Deputy Director, is responsible for recruiting new volunteers in Boston to serve as Literacy Coaches. The Outreach Coordinator will build relationships with community partners and influencers, attend community events, conduct information sessions, and promote Literations volunteer opportunities online and in-person, with an immediate emphasis in Dorchester, Roxbury, and Mattapan. Willingness to work occasional evenings and/or weekends to represent Literations at community events as needed
The Outreach Coordinator is part of the Recruitment and Training Team and will work closely with the Volunteer Coordinator and the Training Specialist. They will also interface regularly with Literations's Operations and Program Teams.

Outreach
In partnership with Literations Marketing VISTA, create a schedule of social media outreach and develop engaging promotional materials to use for online and in-person outreach opportunities

Research and attend community events, schedule and conduct in-community information sessions
Develop and manage a Volunteer Ambassador program to engage existing volunteers in Literations's recruitment efforts

The Programming and Outreach Coordinator (OC) position is a three-season position that is on a full-time 6 month contract from mid-April to mid-November, and part-time contract from January to April. This position is partially office-based and partially field-based during the high season.
The fundamental responsibility of the BCP is to protect the surrounding natural resources by offsetting recreational impacts, and to provide maximum benefit to the public who use the AMC managed backcountry campsites. The OC plays a critical role in caretaker training and backcountry public education, specifically with respect to organized groups. The OC position is collaborative not only within the Trails Department, but with other AMC departments such as A Mountain Classroom, Accounting, AMC Research, and AMC PR. The OC is also responsible for maintaining a variety of databases and entering and analyzing program use and financial data.
The OC should anticipate spending 40% of their time in the field while ensuring to fulfill all their administrative duties throughout the season. The OC largely designs their own schedule based on the needs of the program, and they should expect to work a regular 40-hour week (although at times they may need to work more than that). The OC reports to the Backcountry Resource Manager but works collaboratively with the BCP Field Coordinator.

Assists in seasonal caretaker staff hiring, interviews, and training
Supports with pre-season airlifts and bark prep (human waste composting system)
Opens and closes backcountry campsites as needed
Creates and manages backcountry caretaker schedule and caretaker binder resources
Manages Group Notification System (GNS)
Collaborates with AMC IT to ensure form visibility and functionality on outdoors.org
Weekly notifications to BCP leadership team
Maintains open lines of communication with and contact info for organized groups
Communicates with site users about Leave-No-Trace best practices, USFS rules, and trail advice
Collaborates with AMC PR to create social media posts and educational blogs
Responsible for data entry and graphic visualization of BCP Site Use and Finances
Creates framework for training schedule and midsummer appreciation day
Site visits caretakers at least once throughout the season to model expectations (projects, visitor interactions, site maintenance) and support caretaker wellbeing
Fills-in as site caretaker when needed
Contributes to field projects and outstanding field needs (overdue runs, shelter projects, etc.)
May support or lead specialized programs/initiatives such as site phenology plots, privy outreach, Bear Canister campaigns, SOLSA liquid separator system, alpine steward and trail volunteers, etc.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Essential duties include the following but are not considered an inclusive list.
Assures all clients assigned to caseload receive annual physicals, including physician prescription for Day Habilitation Services.
Completes Yearly D&E Assessments on clients within two (2) weeks of receiving physical/prescription for Day Habilitation; client's annual assessment information assists in development of client's Individual Program Plan (IPP).
Develops IPPs for all assigned clients, according to all licensing guidelines, including Adult Development goals/objectives specific to client needs and individualized schedules.
Completes Quarterly Reports every 90-day period from time of client's IPP Yearly Meeting, including progress/lack of on goals depending on data from Daily Goal Sheets, behaviors, accomplishments, medical information, etc.
Conducts Quarterly Meetings with assigned clients to relay progress/change on goal objectives and provide feedback on past 90 days. Send reports to client's families to communicate progress and concerns.
Assures all goal/objective changes are completed as needed on Daily Goal Sheets and distributed to instructors on bi-monthly basis.

Job Summary:
The Community Health Worker (CHW) is a peer who has experience in navigating Rhode Island's health system for themselves, a family member or through previous employment. This CHW will support children or adults with special needs and their families. The CHW will be a critical part of a comprehensive team providing peer support, care coordination, resources and referrals. CHWs will engage with consumers in the home and community settings providing person centered, culturally sensitive support, and building on the values, strengths and preferences of the member. The CHW will also serve as an effective role model and mentor.
Essential Functions:
Utilizes motivational interviewing skills and culturally sensitive methods to help consumers to achieve goals
Provide emotional support, serve as a role model, and guide consumers to practice positive, responsible healthy behaviors
Assist consumers as they transition to independence/case closure by engaging with consumers and providing follow up support
Maintain timely, accurate records, documentation, and reports as required
Assist in data collection, surveys, assessment and reporting as required
Actively participate and complete training and professional development activities
Assist in statewide system analysis, planning and coordination with state agencies, state and local boards, community-based organizations, and community rehabilitation programs
Accept other duties and responsibilities as assigned
Qualifications
