This state-of-the-art, 60,000-square-foot facility will offer a wide array of services, including primary care, urgent care, advanced imaging, and specialists. You will be joining a visionary project that establishes a new standard for healthcare delivery in suburban areas. Position is day shift, Monday - Friday, no holidays or weekends. Responsibilties: A day in the life of a Physical Therapist at Hackensack Meridian Health includes: Prepares for the evaluation in accordance with department policies/procedures and standards of practice, including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses. Selects and administers tests and measures. Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care. Establishes treatment plan based on evaluation results, goals and best practice. Collaborates with interdisciplinary team, patients and patient's support system. Assesses and reassesses need for modifications to plan of care and goals. Identifies need for consultation and/or refers the patient to other health care professional or health care services. Initiates and completes discharge plan. Provides education/training to patient and/or patient's support system. Provides functional Training: This may include but is not limited to gait training and/or locomotion training; therapeutic activities (e.g. bed mobility, transfers); community reintegration, ADLs/IADLS; work injury management; equipment prescription. Provides Musculoskeletal Interventions: This may include but is not limited to, therapeutic exercise; manual therapy; joint mobilizations; Provides Neurological interventions may include but are not limited to, handling techniques; neuro-developmental training; neuromuscular re-education; cognitive rehab; behavioral management; spasticity management; vestibular therapy. May provide additional assessments but is not limited to cardiac rehab, medical monitoring and splinting. Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care. This includes, but is not limited to, examination, evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team. Additionally records daily billing and follows department guidelines for productivity. Other duties and/or projects as assigned. Adheres to HMH s Organizational and Managerial competencies and standards of behavior. Lifts a minimum of 5 lbs., pushes and pulls a minimum of 5 lbs. and stands a minimum of 1 hours a day Qualifications: Education, Knowledge, Skills and Abilities Required: Graduate of an APTA accredited Physical Therapy education program or equivalent. Outpatient orthopedic experience is preferred.*** Responsibilties: A day in the life of a Physical Therapist at Hackensack Meridian Health includes: Committee Representative: Participates on committees as requested by Supervisor. Customer Service: Delivers the highest level of service to all residents, families and visitors. Delivery of Care: Models and maintains a safe environment in which to deliver services and employs correct body mechanics during activities. Demonstrates and implements appropriate treatment techniques and skills with residents. Completes resident treatments as scheduled. Discharge Planning: Discharge planning established at initial evaluation and updated throughout the course of therapeutic treatment. DME Assessment: Demonstrates appropriate knowledge in assessing the need for DME for residents to improve their functional independence safety. Follows through with appropriate professionals to ensure proper fitting of the wheelchair to the resident. Educates the resident/family about the proper use and care of the wheelchair. Documentation Compliance: Prepares written documentation such as Progress Notes, Reassessments and Discharge Summaries. Progress notes and Reassessments are completed as per departmental policy and procedure, and monitor the resident's status and progress towards goal achievement. Short term goals are updated and treatment plans are modified, as appropriate. The Discharge Summary reflects the resident's current status, outcomes achieved, and recommendations for further services or interventions, as appropriate. Documentation of Re-Evaluations: Re-evaluations are completed in a timely manner. Goals and treatment plan updated as per departmental policy. Effective Communication: Develops a rapport and mutual respect with the resident to foster an optimum environment for goal achievement. Provides education to residents/families based upon the identified learning needs, in a manner that the residents/families can demonstrate their understanding. Evaluation Information Gathering: Gathers all information necessary to properly evaluate the resident's needs as per department policy, including testing and consultations with other health care professionals, as appropriate. The content of the evaluation contains data as outlined by departmental policies and is completed within time frames as specified in departmental policies. Other Duties As Assigned: Handles special projects and rush assignments as needed, in a timely manner. Patient Rapport: Develops a rapport and mutual respect with the patient/caregiver and team members to foster an optimum environment of goal achievement. Patient/Caregiver Education: Provides and documents education to patients/caregivers based upon identified learning needs in an understandable manner. Plan of Care: Develops and implements an individualized treatment program based on established goals.
