Position Requirements Required Skills/Abilities: Knowledge of the dying process; knowledge of policies and procedures of the hospice organization. Knowledge of regulations surrounding compliance with Medicare Conditions of Participation. Ability to function as a resource to patients and families during times of high stress and volatility. Excellent interpersonal and communication skills, including providing support for patients and families, resource providers, members of the clinical team and employees. Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing. Well organized to manage patient and family needs, provide resources and complete timely documentation Ability to work both independently with sound decision-making, and as a part of an interdisciplinary team. Strong assessment and documentation skills. Proficiency in operating a computer; including a working knowledge of Microsoft applications (Word, Excel, and Outlook) and an electronic medical record. Education and Experience: Required Education: Master's Degree in Social Work from a Graduate School of Social Work accredited by the Council on Social Work Education, Current N.J. License Preferred Education: Required Experience: One year of post-Masters Social Work or counseling experience. Preferred Experience: Prior experience in a healthcare setting.
