    We are seeking a hardworking, passionate and caring individual to work with clients that have been diagnosed with a mental illness. As an Adult Case Manager you will help clients gain access to services needed and support them through their recovery. Responsibilities Maintain a caseload of clients. Coordinate and make referrals for client services that will help the individual meet their goals. Complete and accurately update all documentation as required. Work collaboratively with all members of the treatment plan as often as needed. Follow-up on referrals and monitor the clients progress. Provides crisis assessment and intervention services to clients as needed. Qualifications Requirements & Qualifications Bachelor's degree in Social Work, Psychology or a related human services field. Minimum of two years providing services to individuals with a mental illness or substance use disorder required. Must qualify as a Mental Health Practitioner in the State of MN Experience working with severe emotional disturbances in adults is preferred. Valid driver's license and proof of auto insurance.
