Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following RAI guidelines for completion, computer encoding and electronic transmission of the MDS.
Identify changes in patient condition in conjunction with the interdisciplinary team and coordinate scheduling and completion of indicated assessments.
Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff.
Attend educational programs to maintain knowledge with current RAI/Prospective Payment System regulations. Qualifications: *Must be a Registered Nurse currently licensed by the state and in good standing.
*Previous experience in a hospital, long-term facility, or other healthcare related facility is required.
*Prior experience completing MDSs and Care Plans is required

Waypoint's Overnight Young Adult Shelter provides a safe, low-barrier space for young adults ages 18-24 experiencing homelessness across New Hampshire. Overnight Shelter Staff play a critical role in creating a calm, supportive, and respectful environment where young people feel physically and emotionally safe.
This is an awake overnight position focused on relationship-building, safety, and crisis support, while meeting young adults where they are using trauma-informed, harm reduction, and positive youth development approaches.
Key Responsibilities
Maintain a safe, welcoming, and non-judgmental shelter environment
Build trusting relationships with young adults and connect them to case managers
Provide crisis intervention, de-escalation, and emotional support as needed
Complete intakes, documentation, and shift notes
Support shelter routines, boundaries, and policies
Communicate effectively with teammates to ensure continuity of care
Assist with light cleaning, inventory, and reporting maintenance needs
Participate in required trainings and supervision
Support staffing coverage as needed

Comfort working overnight and remaining alert
Basic computer skills and reliable transportation
Qualifications
High school diploma or equivalent
At least one year of related experience (lived experience welcomed and valued)
Experience with vulnerable populations a plus
Criminal background check required (prior legal involvement does not automatically exclude candidates)
Physical Requirements
Ability to remain awake and alert throughout overnight shifts
Ability to stand for extended periods and navigate stairs
Light cleaning and housekeeping duties required

Under the direction of the Supply Chain Management Manager, responsible for daily monitoring, re-ordering of inventory items for the hospital, receipt, inspection and delivery.
Education

Supply chain experience preferred.
Responsibilities
Establish and maintain supply areas.
Inventory product and upload requisitions in system.
Manage supplies in LogiD areas and monitor tags on RFID boards to prevent stock-outs.
Distribute supplies and use mobile delivery device for product tracking as needed.
Follows up on order confirmations, short ships, back order issues and returns.
Oversees parcel management inclusive of receipts and distribution which includes CC-ID and mobile
delivery.
Establish and maintain list of supplies and supply levels for assigned department(s).
Supports off campus corporation entities.
Maintain Expired Product Management for assigned department(s). This includes management of
Nonconforming product bins in supply areas.
Demonstrates ability to prioritize and process work effectively and efficiently.
Demonstrates competency to perform role utilizing available references, tools, systems, and equipment,
to ensure quality results while following department policies and procedures.
