- Coordinates care coordination services for the Health Home program
- Supervises care coordinators and other direct reports
- Participates in the hiring and termination process of direct reports
- Manages aspects of program and system development for the Health Home program
- Conduct Monthly team meetings
- Assure care coordination services are delivered to HARP-eligible clients in compliance with NYSDOH HARP (Health & Recovery Plan) guidelines and policies
- Develops and implements quality assurance activities including chart review and client survey data for the Health Home program
- Conduct monthly 1:1 case specific supervision with Care Coordinators
- Reviews monthly services delivered to clients to assure the delivery of Health Home Core Services
- Maintains and analyzes statistical and claims data for the Health Home program
- Ensures written policies and procedures for the Health Home program are being implemented
- Facilitates and participates in Case Conferences
- Assists in the implementation of an on-going training program for Care Coordinators
Qualified Candidate will have a Masters of Social Work, Masters of Mental Health Counseling, or Clinical Psychologist with one (1) year of professional experience in a supervisory or management position supervising clinicians or staff serving people living with Severe Mental Illness (SMI) and Substance Use Disorder (SUD) OR Masters in a related field (Psychology, School Counseling, etc.) with three (3) years of supervisory or management position experience supervising clinicians or staff serving people living with Severe Mental Illness (SMI) and Substance Use Disorder (SUD). Possession or eligibility to obtain New York State licensure (LMSW, LMHC, LCSW, etc.) within six (6) months of beginning position, preferred. Experience developing and implementing direct client services and case management services, required. Must demonstrate high level clinical skills, including performance of clinical assessments, person-centered and recovery-oriented care planning, crisis intervention and strong clinical judgment and decision making skills. Candidate must possess valid NYS driver license and an insured, dependable car to use for work-related activities. Excellent written and verbal communication skills, ability to work independently and sensitivity toward cultural diversity, people living with HIV/AIDS and lifestyle and addiction issues essential.
