Cornerstone Care is seeking a Physician for our Sto Rox and Hilltop locations. POSITION SUMMARY: To care for our patients in a variety of ways, from treating and managing chronic illnesses, interpreting multiple in-house screenings to advising about preventative healthcare. The physician will work to diagnose and treat injuries and illnesses by performing physical exams, taking medical histories, performing and interpreting diagnostic tests and recommending a plan of treatment. JOB DUTIES AND RESPONSIBILITIES: Patient Care Assumes leadership of the care team. Manages clinical aspects of patient care. Understands and provides health literacy appropriate disease management education. Takes into consideration barriers that may impact the patient's understanding or treatment management. Works with the care team to collaboratively identify and set patient treatment and self-management goals. Participates in quality improvement initiatives as identified by patient surveys, feedback, or quality measurement/outcome data. Examines, diagnoses, and treats patients utilizing the Cornerstone Care offices. Ensures that medical care is completely and accurately documented in the medical record. Ensures that their PAQ (provider approval que) is checked minimally daily. Addresses all urgent out of range testing in a timely manner and everything in the PAQ is signed off within 10 days. Initiates requests for laboratory procedures and/or diagnostic tests that are not on standing orders; follows up test results as needed and ensures that the care team notifies the patients of results. Performs triage, when necessary, on in-coming patients; provides emergency care as needed; arranges for transfer to the appropriate facility if needed. Provides physician supervision for mid-level practitioners as needed. Prepares and signs all forms needed for patient care and ensures that all necessary information to submit a claim for the visit is in the EHR when the patient leaves the exam room or shortly after. Participates in after-hours patient call rotation. Administrative Assists as needed, in maintaining the Medical Patient Management/Health Care Delivery Policies of the Principles of Practice. Assists in development of performance improvement standards and procedures, which insure safe and therapeutically effective, acute, chronic, and preventative health care and continuity of that care for patients and their families. Assists to assure that adequate equipment and supplies are in place to provide optimal and efficient patient care. Assists as needed, in formulating the Annual Health Care Plan. When necessary, attends meetings of the Board of Directors as determined by the CEO. Participates as requested or needed on FPAT, CORE Team and/or Special Meetings. Community Service Monitors the health care needs of the service area and attempts to fulfill them. Participates in community health education and projects as needed. Acts as an advocate and liaison for the health center with the public, the medical community, the hospitals, the nursing homes, and with local, state, and national medical societies as determined by the CEO. Maintains membership in medical and specialty societies as needed. Participates in after hour center activities as available. WORKING CONDITIONS/ESSENTIAL FUNCTIONS: Full scope of family practice in a community health center setting with emphasis on teamwork and patient-center medical home approach. See Attachment B Communication with, and examination, diagnosis, and treatment of a wide range of patients, including all ages and chronic conditions. Orders and interprets diagnostic tests. Collaborative work with other providers and Care Teams. Duties may include administrative tasks, committee participation, and community events.
